Practical Aspects of Mohs Surgery 251

A potpourri of well illustrated practical suggestions to make Mohs surgery
easier, faster, and better for patients is presented. Major topics include
streamline excision of Mohs layers, useful hemostatic agents, how to flatten
tissue containing cartilage for laboratory processing, closures using skin
staples and tissue glue, and special instruments such as the oilman and
Shaw thermocautery scalpels. From a historical prospective a brief review
of chemosurgery (fixed tissue technique) is presented.
Mohs Surgery Fixed Tissue Technique: Current Approach and
Indications 267

The current indications for Mohs surgery fixed tissue technique are de¬
scribed, including the main indication, malignant melanoma. Retrospective
data, using historical control comparisons demonstrating a significant im¬
provement in 5 year survival for fixed tissue surgery relative to conventional
surgery in the treatment of high risk melanoma is presented. An experimen¬
tal study indicating an immunologic adjuvant effect to the tumor by zinc
chloride is discussed. A simplified technique is explained.
Mohs Micrographic Surgery for Basal Cell Carcinoma 275

The basal cell carcinoma (BCC), because of its limited ability to metastasize,
and because it grows in a contiguous fashion, is ideally suited for Mohs
micrographic surgery (MMS). The indications, limitations, advantages, and
disadvantages of MMS are reviewed along with clinical examples. The
possible pitfalls associated with MMS that can lead to tumor recurrence
include improper mapping and tissue orientation, poor quality histologic

preparations, and disconnected foci of tumor. The three most important
variables to consider when deciding whether or not a BBC should be man¬
aged by MMS are anatomical location, histology, and whether or not the
tumor has been previously treated. The indications for and the importance
of a multidisciplinary approach when managing difficult BCCs are discussed
in detail.

prognosis for perineural squamous cell carcinoma of the skin.
Mohs Surgery for Lentigo Maligna and Melanoma 309

Mohs micrographic surgery (MMS) is a valuable therapeutic modality for
the treatment of lentigo maligna and malignant melanoma. It is particularly
useful in cosmetic areas or where excessive surgical excision would result
in disfigurement or loss of function, such as the face, hands, feet, or genital
areas. This technique is also superior in tumors in which there are subclinical
extension beyond the clinically visible margin, a common dilemma in lentigo
maligna. With experience and quality frozen sections, MMS provides com¬
plete removal of the tumor with a lower recurrence rate and maximal
tissue sparing.
Mohs Micrographic Surgery for Dermatofibrosarcoma Protuberans 319

Dermatofibrosarcoma protuberans (DFSP) is an intermediate grade fibro
histiocytic neoplasm with a marked tendency for recurrence. This tumor
spreads locally in an aggressive manner but rarely metastasizes. Surgical
excision has historically been the treatment of choice for DFSP. Unfortu¬
nately, the tumor has marked tendency to recur after standard surgical
excision. Therefore, Mohs micrographic surgery has become an attractive
therapeutic alternative to standard excision with wide surgical margins.
The Treatment of Merkel Cell Carcinoma with Mohs Micrographic
Surgery 329

Appropriate therapy for Merkel cell carcinoma (MCC) is still in the process
of being defined. As with all tumors with great propensity to metastasize,
the physician hopes to extirpate the tumor before metastasis. The prime
goal for definitive excision of MCC is to remove the entire primary tumor.
MCC, however, is known to frequently have subclinical contiguous tumor
extension. Mohs micrographic surgery (MMS) is a meticulous method of
excision that assures that all contiguous subclinical tumor is identified histo
logically and excised. This article discusses therapy of MCC in general and
specifically the role of MMS in the treatment of the primary tumor.
Rapid Immunoenzyme Mohs Technique for Skin Cancer Removal:

Mohs micrographic surgery is an extremely effective method for removing
complex tumors. Since Tromovitch and Stegman developed the fresh tissue
technique in the 1970s, few new developments have been added to the
technique. Rapid immunocytochemistry micrographic (RIM) surgery offers
increased sensitivity for patients with complex tumors or those hidden by
inflammation. This article describes the technique and specifies optimal
methods for multiple types of tumors.
Limitations and Recurrences With the Mohs Technique 347

Up to 77% of local recurrences after Mohs micrographic surgery may be
caused by technical problems during the surgical procedure, specimen han¬
dling, frozen section tissue processing, or histopathologic interpretation.
Meticulous attention to each step of the technique can reduce these recur¬
rences. The remaining recurrences develop because of multifocal tumors
with skip areas. At times, tumors are so invasive that a tumor free plane
cannot be obtained with acceptable morbidity and mortality. Deeply inva¬
sive tumors benefit from a combined approach to collaboration with a head
and neck surgeon to preserve vital structure.
The Paramedian Forehead Flap for Nasal Reconstruction 365

The paramedian forehead flap has no worthy rival when reconstructing
significant nasal defects. As an axial flap with a richly vascularized pedicle,
it is incredibly reliable. As a source of tissue to recapitulate the bulk and
texture of the nose, it is without parallel. No other flap offers equal versatility
when using subunit reconstructive principles to provide aesthetic results.
When properly lined and supported, even defects encompassing loss of soft
tissue, cartilage, and bone can be repaired.
Reconstruction Issues After Mohs Surgery 379

Reconstruction issues after Mohs surgery often focus on the technical aspects
of the surgical repair. This article reviews this but also provides a discussion
of several equally important issues that are perhaps not initially considered.
These include the approach to preoperative planning, the timing and loca¬
tion of the Mohs excision and reconstruction, other logistical issues, and
also the oncologic and pathologic issues that can arise. The perspective of
the Mohs and head and neck and reconstruction surgeon is provided.
The Multidisciplinary Approach to the Management of Advanced
Nonmelanoma Skin Cancer 387

The safety and efficacy of Mohs surgery in the management of nonmelan¬
oma skin cancer have been well described. Advanced tumors, however, of
ten require the cooperation of colleagues from various medical specialties
to improve the patient s chance for disease free survival. The characteristics
of advanced skin cancer are reviewed and the importance of a multidiscipli
nary approach to the management of these tumors is emphasized.
