Soft tissue wound healing is a highly orchestrated cellular response from
tissue injury. If it is impaired, scarring frequently results that disrupts normal
architecture and function. To optimize the healing of wounds, understand¬
ing the normal repair process is essential. In the future, by modifying the
wound environment to be more similar to fetal wounds, it may be possible to
reduce scarring. This article describes the mechanisms of soft tissue wound
healing and its relation to scarring.

The face is one of the most important functional and aesthetic units of the
body. Soft tissue injuries can disrupt not only the functional integrity of
the face but also compromise its aesthetic qualities, with far reaching per¬
sonal and social ramifications. The appropriate management of facial soft
tissue injuries can lessen or eliminate many of the negative sequelae of facial
injuries. This article describes reliable techniques to maximize outcome for
facial soft tissue injuries.

Not all scars need to be revised. The appropriate indications, timing issues,
and psychologic considerations all need to be taken into consideration to
ensure a satisfactory outcome from scar revision surgery. This article details
some of these critical concerns in guideline fashion to help ensure favorable
results when undertaking revisional surgery for facial scars.

Simple and Serial Excisions 141

Simple fusiform excisions of facial scars are indicated when the scar lies
parallel to or less than 30 degrees to the relaxed skin tension line (RSTL),
when the resultant scar will be well hidden in a wrinkle or parallel to the
RSTLs, and the wound is able to be closed without the need for advancing
or rotating adjacent tissue. Scars less than 5 mm in diameter can be excised
using a dermatologic punch and closed primarily without the need for
extensive undermining. When the scar (such as a facial burn) is too large
for a one stage excision, serial excisions of the central scar tissue are indi¬
cated. This technique sequentially removes the scar tissue as it simultane¬
ously advances normal adjacent tissue. Ultimately the entire scar will be
excised and normal skin approximated.
Z Plasty Technique 149

The Z plasty technique is a versatile tool for reconstructive surgeons. Com¬
plete understanding of the geometry and complexity of this procedure is
necessary to maximize its applications. Once the surgeon is familiar with
the basic technique, variations can be used to broaden the range of applica¬
tions of tissue transposition. Ultimately, the true versatility of the Z plasty
is limited only by the surgeon s ingenuity.

Surgical improvement of prominent facial scars continues to be a challenge.
W Plasty is one of the simpler means of revising a conspicuous scar and is
based on the principles of scar excision and replacement with irregulariza
tion. The pattern and scar reorientation tend to contribute toward scar
camouflage. There are several subtleties incorporated in the design of the
W plasties closure and these are detailed in this article.

Geometric broken line closure (GBLC) camouflages unaesthetic scars run¬
ning against the relaxed skin tension lines (RSTLs) of the face by a deliberate
randomizing of geometric interposed flaps. Arranging combinations of tri¬
angles, rectangles, semicircles, and squares along the scar in random order
disguises the scar by creating an unpredictable pattern. In addition, the
placement of the maximum number of wound edges parallel to the RSTLs
enhances scar camouflage, as does the interdigitation of the GBLC represents
its primary advantage over other techniques of scar revision, such as the
running W plasty, which may result in repetitious, predictable scar patterns.

Medical laser technology and applications for skin resurfacing and the
aesthetic laser market continue to advance at an unprecedented rate, driven
by an increased demand by physicians and patients for better and less
invasive skin rejuvenation procedures. The aging of the baby boomers and
their history of early sun exposure in an atmosphere with a diminished
ozone layer have produced a population that is in need of skin rejuvenation.
This article discusses modern and contemporary carbon dioxide laser resur¬
facing and Erbium laser resurfacing.

Although soft tissue fillers have a limited role in scar revision, they can
have specific applications in certain clinical conditions. This article reviews
the several available soft tissue implants and the advantages and disadvan¬
tages associated with their use.
The Treatment of Hypertrophic Scars and Keloids 191

Hypertrophic scars and keloids of the head and neck are common entities
in the noncaucasian races. The primary difficulty in their treatment is to
prevent recurrences. A treatment algorithm is presented incorporating the
CO2 spot laser and secondary healing of the keloid wound bed. Injections
of triamcinolone acetonide and hyaluronidase are used in the initial treat¬
ment of hypertrophic scars and for early collagen depositions in the postop¬
erative excision of keloids. Scar revision is an important treatment modality
for hypertrophic scars.

Design of geometric broken line closure (GBLC) can be challenging. This
article demonstrates a simplified method of designing the GBLC using the
concept of the half square. The advantages of this method over the more
traditional W plasty are discussed.

The relevance of the midface in aesthetic and reconstructive surgery has
only been realized recently. Traditional aesthetic surgery has concentrated
on the aged lower eyelids and lower facial structures but has neglected the
multicontoured changes that occur in the midface. Within the past decade,
the paramount importance of the midface has come to light. The position
of the midface affects not only the lower eyelid contour but also the naso
labial and even the geniomandibular regions. This article reviews the midfa
cial aging changes and midfacial anatomy. Advanced techniques in midfa
cial repositioning are discussed. Experience with these techniques is limited,
and they are still in evolution. As a better understanding is gained of the
midfacial structures and changes occurring in this region, these techniques
will continue to improve and will most likely become part of the aesthetic
surgeon s armamentarium of surgical procedures.

A common complication of lower eyelid blepharoplasty is lower eyelid
retraction and lateral canthal dystopia manifesting as round eye and scleral
show. The problem often resides in the three lamellae (layers) of the eyelid.
Reconstruction must, therefore, address the anterior, middle, and posterior
lamellae of the lower eyelid. This should be performed in conjunction with
a myocutaneous and cheek elevation. The Madame Butterfly procedure is
described in detail in this article.
