This article discusses the significance that contusions, fractures,
or ligament injuries (involving the small bones, joints, and soft
tissues of the forearm, wrist, or hand) can have on the athlete.
The concern for proper management of upper extremity injuries
is addressed in order to decrease the possibility of long term
functional consequences for the athlete.
Epidemiology of Hand and Wrist Injuries in Sports 401

This article addresses the prevalence of various hand and wrist
injuries in a variety of sports. Multiple resources and references
have been utilized to draw a perception of the types of injuries
and the sports with which they are most commonly associated.
It is hoped that this article will give the physician a greater
understanding of the types of hand and wrist injuries they may
expect to encounter in athletes.

Both the weekend and professional athlete are at risk for devel¬
oping tendinitis and overuse syndromes. This article focuses on
the inflammatory mediated afflictions most commonly experi¬
enced in the upper extremity. A detailed explanation of the
causes, diagnostic criteria, and treatment is outlined.

Tendon injuries in the hand and wrist are common in the athletic
population. The key to treatment is diagnosing these injuries
early so that appropriate treatment may be promptly initiated. A
good result can usually be obtained when these injuries are
treated early. Most can be treated with splinting, although some
of these injuries may require operative intervention. When these
injuries go untreated and are diagnosed late, a permanent disabil¬
ity may result.

Wrist fractures are frequent in athletes. Optimal management
begins with a prompt, accurate diagnosis. Awareness of predict¬
able, highly specific fracture types for particular sports leads to
greater precision in early diagnosis and skillful treatment. Ana¬
tomic and stable fracture reduction, with restoration of normal
anatomy followed by comprehensive rehabilitation leads to a
highly favorable outcome and return to competition.

The high demands placed on the upper extremity in sporting
activities subject the competitive athlete to common injuries of the
hand. Treatment options are based on the fracture configuration,
associated extremity injuries, and status of the surrounding soft
tissue. Metacarpal and phalangeal fractures may usually be
treated by closed, nonoperative methods, and most athletes may
quickly return to play with a protective orthosis. Supplemental
methods of fixation, such as percutaneous pins and tension band
wires, may be used for unstable fractures. When required, open
reduction and internal fixation can provide optimum stability to
the fracture, which allows immediate range of motion and early
return to play.
Joint Injuries of the Hand in Athletes 513

The variety of injuries that occur in the joints of the hand range
from skin abrasions to displaced intra articular fractures. This
article discusses serious injuries involving ligaments, volar plate,
and bones at specific anatomic regions of the joints of the hand.
Ligamentous Injuries of the Wrist in the Athlete 533

Wrist instability most commonly results from ligamentous disrup¬
tion between bones of the proximal carpal row. Scapholunate and
lunotriquetral dissociation are forms of this instability pattern.
Carpal instability can also occur due to loss of the normal liga¬
mentous restraints between the carpal rows. Ulnar midcarpal
instability is an example of this pattern. The treatment of wrist
instability depends on the specific type and degree of carpal
disruption and the presence or absence of degenerative changes.
Options include soft tissue reconstruction, partial wrist fusion,
limited carpal bone excision, or a combination of methods.
Thumb Injuries in the Athlete 553

Sports related injuries of the thumb, including fractures and dislo¬
cations, are very common. The anatomic basis for these injuries
is discussed. Treatment options include functional bracing and
early operative intervention, thus hastening a return to sporting
activities.
Ulnar Sided Wrist Pain in Athletes 567

A previously unpublished report of seven professional athletes
who were treated for ulnar sided wrist pain is presented in this

article. This report is followed by an introduction to the anatomy,
examination, and imaging of the ulnar side of the wrist. Specific
sources of ulnar sided wrist pain, including triangulofibrocarti
lage complex tears, lunotriquetral instability, distal radioulnar
joint pathology, extensor carpi ulnaris subluxation, calcific flexor
carpi ulnaris tendinitis, and pisotriquetral arthritis are presented.
Neurovascular Problems in the Forearm, Wrist, and Hand 585

Peripheral nerve injuries in athletes are rare. Musculoskeletal
injuries have increased in frequency as our population is more
involved with athletic participation. Most peripheral nerve injur¬
ies are related to repetitive stresses or direct trauma. An accurate
understanding of neurovascular anatomy and mechanism of in¬
jury is paramount to the diagnosis and treatment of these injuries.
An accurate neurovascular examination and provocative testing
is essential to their detection. Clinical suspicion can be supported
by electrophysiologic testing, including electromyography and
nerve conduction studies. Proper equipment, proper technique,
and an understanding of the biomechanics of participation can
aid in the prevention of these injuries. The hallmark of treatment
should be directed at rest, splinting, physical therapy, and anti
inflammatory medications. Occasionally, surgical decompression
is warranted to return the athlete to productive participation.
Gymnastic Wrist Injuries 611

Wrist injuries in the gymnast are due to the transformation of the
upper extremity into a weight bearing entity. Both acute and
chronic disorders occur and are difficult to manage in these
athletes because of the intensity of training and competition.
Treatment is dependent on the injury as well as the expectations
of the patient, coach, and family—a situation that at times may
alter the usual management program.
The Role of Arthroscopy in the Treatment of Wrist Injuries
in the Athlete 623

Wrist arthroscopy provides an ideal means of evaluating intra
articular soft tissue injuries of the wrist. Many lesions such as
tears of the triangular fibrocartilage complex can be treated by
arthroscopic means as well. Reduction of articular fractures of the
distal radius and pin fixation, reduction of scaphoid fractures and
intramedullary fixation, and arthroscopic reduction and transcu
taneous pin stabilization of acute carpal dissociation patterns can
all be accomplished with minimally invasive techniques under
arthroscopic control. These measures often provide the athlete
with shorter periods of immobilization and earlier return to ath

letic competition. Wrist arthroscopy provides a very useful ad¬
junct to the treatment armamentarium of all sports medicine
physicians.

Hand and wrist injuries are common among athletes. Although
the hand does not often bear weight and these injuries do not
always sideline an athlete, careful attention must be paid when
treating injuries of the hand and wrist. Rehabilitation is given for
common injuries of the hand and wrist, including exercises and
protective splints and braces. Details on how to fabricate a play¬
ing cast for athletes are also discussed.
