Cervical Flexion Extension/Whiplash Injuries

CONTENTS
Preface xi
Gerard A. Malanga
The Pathophysiology of Whiplash 209
Leslie Barnsley, Susan M. Lord, and Nikolai Bogduk
Many aspects of whiplash remain controversial. Controversies stem largely from
preconceived attitudes and beliefs that stand in contrast to the ever increasing body
of scientific knowledge about this phenomenon. However, research into whiplash,
particularly during the last decade, has dispelled many of the myths and concerns
about it. This chapter reviews the impressive body of knowledge about its biome
chanics, pathology, and the mechanism of symptoms.
Automotive Design and Whiplash Injury 243
Thomas Gibson and Nikolai Bogduk
This chapter examines whiplash injuries from the perspective of automotive en¬
gineering. The regulatory interventions and available biomechanical knowledge
are identified and engineering measures able to prevent whiplash injuries are
discussed.
The Clinical Picture of Whiplash Injuries 257
Robert W. Teasel! and Allan P. Shapiro
Whiplash injuries continue to be a controversial clinical condition characterized by
neck pain and headache. However, in many cases patients experience additional
symptoms of dizziness, tinnitus, visual disturbance, memory and concentration dif¬
ficulties, and arm and low back pain. This chapter comprehensively reviews all
clinical symptoms associated with whiplash injuries.
A Summary of the Quebec Task Force on Whiplash associated
Disorders: Classification, Evaluation, and Treatment Guidelines 271
Richard Leclaire and Guy Bouvier
This summary presents the basic approach of the first board of international experts
requested to make specific recommendations to assure fairness and compassion to
persons with whiplash injuries following a motor vehicle collision. A comprehen¬
sive management care is proposed based on preeminence of evidence and expert
consensus.

Prognosis after Whiplash related Injury 287
Jay Smith and Clifford R. Everett
Given the high prevalence of motor vehicle accidents and whiplash injuries, it is
important for clinicians to understand the anticipated clinical outcome of patients
with whiplash associated disorders (WADs). This chapter reviews and summa¬
rizes studies pertaining to prognosis in WAD. Specific discussions regarding tem¬
poral course of recovery and the influence of cervical spondylosis and litigation
follow.
Cervical Zygapophyseal Joint Pain in Whiplash Injuries 301
Susan M. Lord, Leslie Barnsley, and Nikolai Bogduk
The clinical importance of cervical zygapophyseal joints has only recently been re¬
alized as they were proven to be the source of neck pain in about half of chronic
whiplash patients. The anatomy of these joints, their innervation, possible mecha¬
nisms of injury, diagnosis, and treatment are reviewed.
Cervical Intrinsic Disc Pain and Radiculopathy 323 ,
Christopher Rogers, Anand Joshi, and Paul Dreyfuss
The management of cervical intervertebral disc and spinal nerve injury following
whiplash is challenging and controversial due to the lack of adequate studies. The
anatomy, pathophysiology, history, physical examination, differential diagnosis, ra
diographic evaluation, physiologic testing, and treatment options for these painful
conditions are discussed.
Myofascial Pain in Whiplash Injuries: Diagnosis and Treatment 357
Scott Nadler and Paul Cooke
Myofascial pain is most likely a key player in whiplash associated soft tissue injury
to the neck and needs to be thoroughly evaluated and treated. This discussion fo t
cuses on the pathophysiology and treatment of whiplash injuries related to the de¬
velopment of myofascial pain.
Postconcussive Headaches 377
Lawrence J. Horn
Headache is the most common physical postconcussive complaint. This chapter
focuses on the most common causes of postconcussive headache, particularly those ;
germane to cervical acceleration deceleration injury. Treatment of postconcussive
headaches is also discussed.
Minor Traumatic Brain Injury Following Whiplash 395
Richard T. Katz, John DeLuca, and Stacey Lee Smith
This chapter focuses on the supposed relationship between whiplash injury and
mild traumatic brain injury (MTBI). It provides a concise overview of MTBI, ana¬
lyzes the current research in this field, and explores the questions of why certain
patients continue to complain of ill defined symptoms often attributed to MTBI
while the majority do not. i
Contents v
Psychological Distress Following Whiplash Injury 409
Barbara J. Wai I is
Psychological distress is a concomitant of chronic neck pain following whiplash
injury. There is a characteristic profile of distress which resolves if the pain is re¬
lieved. However, not all cases can be diagnosed and treated, and psychological
treatment should be part of a multidisciplinary management approach.
Imaging of Whiplash Injuries 419
Joseph D. Fortin and Edward C. Weber
Imaging needs of whiplash victims are complex and it is imperative to have a sys¬
tematic approach to the various imaging modalities that can be applied. This chap¬
ter focuses on the imaging of cervical spine, since it is the primary region of
interest in the investigation of whiplash injuries.
Manual Medicine Approach to Whiplash 437
Ronald Tolchin and Scott Nadler
This chapter addresses whiplash syndrome from a manual medicine holistic ap¬
proach. The mechanism of injury is discussed along with an evaluation and treat¬
ment overview. Specific examples of soft tissue, mobilization, myofascial release,
and muscle energy techniques are discussed and demonstrated as they apply to the
cervical spine in whiplash syndrome.
Rehabilitation of Whiplash Injuries 453
Frank J. E. Falco and Gerard A. Malanga
This chapter provides an overview of rehabilitation techniques that are often used
to treat whiplash injuries. Rehabilitation plays a key role in the treatment of pa¬
tients with these injuries and is typically divided into three phases: acute, restora¬
tive, and maintenance. Rehabilitation is one component of the multidisciplinary
approach for successful relief of symptoms and functional restoration after
whiplash injuries.
Treatment of Whiplash Injuries 469
Nikolai Bogduk
This chapter outlines the principles of evidence for efficacy and summarizes the
evidence for and against physical therapy, medical therapy, and surgery for acute
and chronic neck pain. Most treatments used for whiplash are unsupported or re¬
futed by the available scientific evidence. Evidence based recommendations for
the treatment of whiplash are also provided.
