tion with limited physiologic reserves. Nevertheless, sedation and analgesia can provide a safe

Management of perioperative arrhythmias requires an appreciation of the unique mechanisms
and toxicities associated with antiarrhythmic pharmacology in surgical patients. This chapter
reviews the molecular pharmacology of both old and new antiarrhythmic drugs, and provides
a rational approach to using these agents to treat supraventricular and ventricular arrhythmias
in the perioperative period.

Experience with percutaneous central venous access has endorsed its usefulness and relative
safety in the care of critically ill patients. Unfortunately, a variety of minor and major com¬
plications are associated with the landmark guided techniques to central venous catheteriza
tion. This review describes the more prevalent techniques and the anatomy of central venous
cannulation, followed by a discussion of related complications. An overview of factors that
complicate central venous access and alternative techniques, including ultrasound guided
approaches, is presented.

Pulmonary artery hypertension is underrecognized in the perioperative period and can be a
life threatening complication. This chapter reviews the presentation of pulmonary hyperten¬
sion and diagnostic tests and management strategies for treating it. Understanding its physiol¬
ogy, early recognition of the complication, avoidance of factors that can worsen it, and judi¬
cious treatment are all essential. New therapies, such as inhaled nitric oxide, may also help
improve outcome.

A guide to blood transfusion for the cardiac patient undergoing noncardiac surgery is given in
terms of the relevant physiology, and also as a set of guidelines for clinical practice. The
patient with diminished cardiac reserve requires particular care with fluid balance, which may
precipitate cardiac failure, and anemia, which may increase cardiac work. A guide to calcula¬
tion of some important parameters of oxygen carriage and hemodynamics is given.

munications have already made digital echocardiography an attractive and affordable alterna¬
tive to tape based recordkeeping. We provide an example of the usefulness of a system under¬
going clinical evaluation in a regional health care network.
