Acute renal failure (ARF) presents a challenge to all critical care practitioners because of the consistently
high mortality rate of 50% to 80% despite advances in many other areas of caring for the critically
ill. This article provides a comprehensive review of and update of the current advances in the treatment
and care of the critically ill patient with ARF. Among the advances to be discussed are the role of
nitric oxide and atrial natriuretic peptide in the pathophysiology and treatment of ATN. Several
suggestions are made to assist the practitioner to integrate this information into practice.

Many patients in acute renal failure (ARF) are now being treated in the critical care setting with
continuous renal replacement therapy (CRRT). It is essential that health care professionals caring for
patients undergoing CRRT have clinical and technical expertise to assure appropriate management
of the patient, as well as the technology. This article describes a program designed through the
continuing collaboration of the American Association of Critical Care Nurses (AACN) and the National
Kidney Foundation (NKF) to provide critical care nurses an opportunity to learn about CRRT.

Continuous arteriovenous hemofiltration is a form of continuous renal replacement therapy (CRRT) in
which an extracorporeal circuit containing a hemofilter is placed between arterial and venous access
catheters for the purposes of fluid andsolute clearance in the critically ill patient. Anticoagulation therapy
and filter replacement fluid are components essential to the management of CAVH. Nursing implications
for the patient undergoing CAVH include being able to problem solve and troubleshoot for patient and
technical complications. Multidisciplinary development of standards of care for CAVH can facilitate
implementation of therapy as well as ensure uniformity of patient care delivery.

The first major variation to CAVH was continuous arteriovenous hemodialysis (CAVHD). In the
adaptation for CAVHD, dialysate is added to the circuit to achieve a countercurrent flow to the blood
path. By the mechanism of diffusion, improvement in solute and uremic toxin clearance was achieved.

With the addition of the dialysate to the circuit, accommodations must be made in the calculation
of filter replacement fluids. Institutional standards of care for CAVHD must be developed to comple¬

Today, continuous venovenous hemofiltration (CVVH) is noted as a promising advancement in the
treatment of acute renal failure because it offers several advantages over traditional dialysis modalities
when used in critically ill, unstable patients. CWH uses the same basic convection and hemofiltration
concepts of CAVH, but has the added benefit of a pump driven circuit. The scope of this article is
CWH and the care of the pump, the patient, and the circuit.

modern technologies, including dialysis. The ethical question that care providers must answer in
order to be accountable to the patient and society is: Am I using dialysis to sustain life or to prolong?
To assist clinicians in exploring this question more explicitly, the following are included in this article:
a perspective on the costs associated with dying in the hospital; physiologic and psychologic criteria that
have been proposed to help guide dialysis discontinuation; components related to patient autonomy; a
brief overview of related ethical principles, decision making models, and dilemmas; and a perspective
on patient advocacy in relation to the question posed.
