Eclampsia is the most dramatic, if not serious, manifestation of
the severe pre eclamptic state. Significant maternal, as well as
fetal, morbidity and mortality occur. Recognition of the patho
physiology allows the anesthesiologist to make rational choices
of anesthetic technique and drugs. Airway management, as well
as possible compromised cardiovascular and neurologic state, test
the limits of the anesthesiologist s working knowledge of both
anesthesia and critical care.
HELLP SYNDROME: An Anesthesiologist s Perspective 331

Hemolysis, elevated liver enzymes, and low platelet count
(HELLP) syndrome is a disease of pre eclamptic women. It is a
part of a continuum of a disease process that includes pre eclamp
sia and eclampsia, and may develop either antepartum or post
partum. This uncommon form of pre eclampsia may initially have
a misleading, benign clinical appearance. However, these patients
progress rapidly to a fully developed HELLP syndrome associ¬
ated with multisystem failure, which can be life threatening. The
use of regional anesthesia in HELLP patients is controversial

because of thrombocytopenia and platelet dysfunction. Thrombo
elastography studies may help determine the safety of regional
anesthesia in HELLP patients. Successful management of criti¬
cally ill HELLP patients requires expert obstetric, anesthesia, neo
natology, nursing, and critical care teams.

Altered hemostasis, both congenital and acquired, has important
implications for anesthetic practice in the obstetric patient. In this
article, pathophysiology, appropriate investigations, and manage¬
ment options are described and are illustrated by case reports.
Epidural or spinal anesthesia in these patients with particular
reference to the risk of epidural hematomas is discussed. New
methods of assessing overall clotting and thromboelastography
are included.

The drug abusing parturient presents many challenges to the
anesthesiologist. Substance abuse in pregnancy has emerged as
a major public health problem of the 1990s. Because of this,
obstetricians, pediatricians, and anesthesiologists are now en¬
countering an increasing number of pregnant patients who use
illicit substances, in particular, cocaine abuse. This article dis¬
cusses the effects of cocaine, amphetamine, narcotic, alcohol, and
cigarette abuse in the parturient.

This article focuses on the management of human immunodefi¬
ciency virus positive patients and related concerns because the
operating room and the obstetric suite are now a common inter¬
face with these patients. The unique medical problems of this
group of patients has significant effects upon anesthetic manage¬
ment. Although this discussion focuses on the parturient and her
child, much of this information is equally pertinent to the general
operating room as well as labor and delivery.

The presence of cardiac disease makes pregnancy more difficult
and risky, and the anatomic and physiologic alterations of preg¬
nancy tend to exacerbate many forms of heart disease. The man¬
agement of these women in pregnancy and through delivery
requires an interdisciplinary approach involving obstetricians, an¬
esthesiologists, and cardiologists. This article reviews some of
the forms of heart disease more commonly seen in women of
childbearing years. Medical and anesthetic strategies are dis¬
cussed that can increase the chance for a successful outcome for
pregnancy even in women with significant cardiac pathology.
Anesthesia for the Pregnant Diabetic Patient 441

Diabetes is a multisystem disease and is the most common medi¬
cal problem of pregnancy. Maternal complications include preg¬
nancy induced hypertension, ketoacidosis, hypoglycemia, pro¬
gression of diabetic retinopathy, and infection. Fetal complications
include early spontaneous abortion, prematurity, respiratory dis¬
tress syndrome, congenital anomalies, macrosomia, and neonatal
hypoglycemia. Significant improvements in maternal and fetal
outcomes have been achieved by screening for gestations diabe¬
tes, control of maternal glucose, and fetal surveillance near term.
The anesthetic considerations of the diabetic parturient are dis¬
cussed with emphasis on optimizing outcome for the high risk
mothers and their babies.
Neurologic and Neuromuscular Disease in the
Obstetric Patient 459
Angela M. Bader
This article reviews the more commonly seen neurologic disor¬
ders in the parturient and will address known effects of the
physiology of pregnancy on the particular disorders discussed.
These disorders include headache during pregnancy, multiple
sclerosis, epilepsy, myasthenia gravis, myotonic disorders, muscu¬
lar dystrophy, spinal cord injury, brain neoplasms, cerebral hem¬
orrhage, cortical vein thrombosis, maternal hydrocephalus with
shunt, and commonly seen mononeuropathies. Issues affecting
obstetric management as well as the available data regarding
implications for anesthetic care are presented.
