The recent increase in the number of so called cumulative trauma disorders has led
to controversy among treating physicians. The author reviews various upper ex¬
tremity musculoskeletal disorders to determine which complaints are truly work
related. By differentiating between physiologic and psychological etiologies,

lems of misdiagnosis, mismanagement, and inordinate disability.

a systems approach and the risk factors for musculoskeletal disorders. The structure
of an ergonomics program, including medical management, is also detailed.

This chapter deals with the management of patients who have minimal or no signif¬
icant findings on physical examination but have complaints of upper extremity pain
or neurasthesias that are attributed to repetitive work activities. Case reports illus¬
trate how the treating physician can use clinical assessment, occupational therapy,
and management of paresthesias in these so called cumulative trauma disorders to
ensure resolution of symptomatic complaints, minimize unnecessary time lost from
work, and prevent disability in these workers.

This chapter focuses on the neurologic examination of the upper extremity to iden¬
tify the cause of symptoms. However, it is intended to identify clinically useful
neurologic examination techniques rather than to be a treatise on the neurologic ex¬
amination. The chapter covers the general physical examination, including
provocative symptom testing, and addresses sensory alterations and weakness and
other motor manifestations in an increasing order of objective examination find¬
ings. A section on the functional neurologic examination is also included.

The first step in controlling the so called epidemic of upper extremity work related
disorders is to understand these conditions. To this end, the authors have provided
history, physical examination, anatomy, diagnostic study, and treatment informa¬
tion on the most significant upper extremity compressive neuropathies, including
carpal tunnel syndrome, ulnar nerve entrapment at the wrist, and compression
neuropathies of the elbow and forearm, such as pronator tunnel syndrome,
Wartenberg s neuropathy, and thoracic outlet syndrome.

This chapter examines the anatomy, pathomechanics, evaluation, and treatment of
ligament injuries of the hand and wrist. Injuries to the proximal and distal interpha
langeal joints, metacarpophalangeal joints of the fingers and the thumb, and car
pometacarpal injuries of the fingers and the thumb are detailed as are the
radiocarpal and intercarpal joints of the wrist. Disorders associated with these in¬
juries are also presented.

Musculoskeletal complaints are the most prevalent of work related injury symp¬
toms. The myofascial pain syndrome (MPS) is increasingly being applied as a

diagnostic label to explain some of these symptoms. Despite an abundance of liter¬
ature, most data have not been subjected to rigorous controlled study but rely
mostly on anecdotes and case reports. This chapter evaluates the objective fac¬
tors purported to support the MPS construct and addresses the relationship of MPS
to occupational factors. Epidemiology, the relationship between MPS and fi
bromyalgia, trigger points, treatment, and psychology are addressed.

Clinicians disagree about the diagnosis, treatment, and mechanisms of reflex sym¬
pathetic dystrophy (RSD). To achieve some type of consensus, the authors review
what is currently known about this perplexing condition, including terminology
and definitions, involvement of the sympathetic nervous system, diagnosis, and
treatment.
Upper Extremity Musculoskeletal Complaints: A Return

This article details a systematic way to achieve good outcomes in patients with po¬
tentially work related upper extremity musculoskeletal complaints. The author puts
these types of complaints into historical perspective and encourages the physician
to understand workers compensation legislation and the employer s perspective.
The author then details the guiding principals of successfully returning the patient
to work: understanding the patient s job, using objective diagnostic protocols, ad¬
dressing psychosocial considerations early on, and clearly communicating the
prognosis, treatment plan, and return to work expectations.

Allegations of work related upper extremity injuries represent one of the largest
areas of workers compensation litigation. In the past, these claims primarily in¬
volved soft tissue injuries, fractures, crush injuries, problems with bone healing,
and management of the diabetic patient. Recently, though, there has been a dra¬
matic increase in injuries allegedly derived from ergonomic or cumulative
trauma injuries. This chapter addresses legal concerns involved in the litigation of
these cumulative trauma claims.
