Proton MR Spectroscopy in Neoplastic and Non Neoplastic Brain

The basic principles of proton MR spectroscopy as well as newer and more
advanced related techniques are reviewed. Spectroscopy is capable of differ¬
entiating normal from pathologic brain and provides tissue specificity
greater than that of imaging in many instances. Spectroscopic mapping
allows for the visualization of the concentration of different metabolites and
their distribution within a lesion. This article emphasizes the utility of proton
MR spectroscopy in the diagnosis of brain rumors, postradiation therapy
changes, infections, degenerative brain disorders, hepatic encephalopathy,
ischemia, and demyelination.
MR Spectroscopy in the Evaluation of Epilepsy 21

MR spectroscopy may be a helpful, reliable, noninvasive test with which
to lateralize the seizure focus in patients with partial complex seizures. MR
spectroscopy is complementary to other studies, especially MR imaging,
and both may be performed in a single integrated examination and result
in improved surgical management and outcome.

Magnetization transfer (MT) imaging has become a focus of recent basic
science and clinical investigations because of its unique physical basis of
MRI CLINICS OF NORTH AMERICA

contrast, and the potential to quantify aspects of the biochemical structure
and composition of tissues. In this article, basic theories of MT are discussed,
and several practical MT pulse and sequence considerations are addressed.
The clinical utility of MT techniques in MR angiography, postgadolinium
Tl weighted scanning, and spine scanning are discussed. Highlights of
quantitative MT studies for pathologic tissue characterization and for studies
of patients with white matter disorders such as multiple sclerosis are in¬
cluded, giving a comprehensive review of the neuroimaging application
of MT.
FLAIR and HASTE Imaging in Neurologic Diseases 53

The need to develop faster imaging techniques has led to the implementation
of fast spin echo imaging, EPI, and hybrid imaging techniques. This article
discusses two of these faster imaging techniques, fast fluid attenuated
inversion recovery (FLAIR) imaging and half Fourier acquisition single
shot turbo spin echo (HASTE) imaging, and their potential clinical applica¬
tion in neurologic diseases. FLAIR imaging, in particular, has demonstrated
considerable promise for the evaluation of intracranial pathology. HASTE
imaging can be used for rapid imaging of the brain or spine in those patients
who are claustrophobic or who have difficulty remaining still, but it does
not yet have a primary role in clinical neuroradiology.
Cortical Activation MR Studies in Brain Disorders 67

Principles and clinical applications of functional MR imaging activation
studies are presented. The physical and physiologic basis for cerebral blood
volume , cerebral blood flow , and oxygenation based functional MR im¬
aging techniques are reviewed. The methodology of blood oxygen level
dependent (BOLD) functional MR imaging is emphasized. Applications to
presurgical mapping of sensorimotor and language function in patients
with vascular malformations, tumors, and other lesions near critical cortical
areas are illustrated. Important new applications to neuropsychiatric dis¬
eases are highlighted.
Functional MR Imaging in Patients With Epilepsy 95
Kenneth L. Weiss, Ramon E. Figueroa, and Jerry Allison
Recent advances in MR imaging have significantly enhanced the diagnosis
and management of epilepsy. This article focuses on the expanding role of
functional MR imaging in our imaging armamentarium. Diffusion, perfu
sion, and BOLD functional MR imaging techniques are discussed in detail
with emphasis on epilepsy applications. Functional MR imaging seizure
focus localization, eloquent cortex mapping, and the potential for functional
MR imaging to replace Wada testing are explored. With anticipated future
developments, functional MR imaging holds great promise for epilepsy pa¬
tients.
Contrast Dosage in the Neuroimaging of Brain Tumors: Principles
and Indications 113

No single dose of MR contrast agent is optimal or adequate for the evaluation
of all types of CNS lesions. This article discusses the fundamental principles
and various background suppression techniques for lesion detection and
delineation. An understanding of all factors influencing lesion conspicuity

is important in deciding the optimal dose, if any, of contrast agent and the
technique necessary for the procedure.
Overview of Diffusion Imaging 125

Diffusion is a major advance in the continuing evolution of MR imaging.
It provides contrasts and characterization between tissues at a cellular level
that may imply differences in function as well as framework. With the
development of echo planar imaging (EPI) diffusion the quality of the data
will be such that clinically relevant information can result, and thus clinical
usage of EPI diffusion will increase. Many potential applications of diffusion
have not yet been explored.
High Resolution Imaging of the Brain 139

Phased array imaging at 1.5 T is a practical means of obtaining high
resolution images of the brain. Our novel coil designs and image intensity
correction algorithms, which are critical for the accurate interpretation of
phased array images of the brain, are described. High resolution imaging
of the brain has been proven to be clinically useful in the evaluation of
patients with partial neocortial epilepsy and has potential clinical applica¬
tions in mesial temporal sclerosis as well as in diffusion and functional im¬
aging.

New techniques are dramatically expanding current clinical applications of
neuroimaging in neuropsychiatry. Neuroimaging research that examines
brain structure and function relationships in neuropsychiatric disorders is
also enhancing our understanding of the pathophysiology of neuropsychiat¬
ric illnesses such as dementia, focal CNS insult, schizophrenia, affective
disorders, and neurodevelopmental disorders.

Postcontrast MR angiography complements the standard spinal MR imaging
study by improving detection and display of normal and abnormal intra
dural vessels, primarily veins. Detection of abnormal veins facilitates diag¬
nosis of spinal vascular malformations and vascular tumors. The most useful
application has been in screening for spinal dural arteriovenous fistula, in
which MR angiography demonstrates the medullary vein into which the
fistula drains, thus allowing noninvasive identification of the spinal level
of the fistula.
MR Neurography: MR Imaging of Peripheral Nerves 179

Recent advances in MR imaging coupled with specially designed phased
array surface coils are revolutionizing imaging of the peripheral nervous
system. Direct visualization of normal sized major peripheral nerves within
the body is now possible. This article describes the appearance of normal
peripheral nerves together with imaging characteristics of various types of
nerve pathology including traumatic injury, compressive syndromes, and

neural tumors. Imaging of the brachial plexus, lumbosacral plexus, carpal I
tunnel, cubital tunnel, and cervical nerves is illustrated and discussed. MR ]
neurography techniques permit imaging detection of peripheral nervous
system pathology that in some cases allow earlier and more accurate diagno¬
sis. It is believed that this will ultimately lead to improved understanding
of peripheral nerve pathophysiology that will, in turn, lead to improved
treatment.
High Resolution MR Imaging of the Auditory Pathway 195

MR imaging is a valuable tool in the evaluation of the auditory pathway.
The current techniques in high resolution MR imaging of the temporal bone
are presented followed by a review of normal anatomy. Several diseases
involving the middle ear, inner ear, internal auditory canal, and cerebello
pontine angle are then presented. A radiologic pathologic approach is used
to illustrate the nature of these diseases and their appearance on MR im¬
aging.
