Spinal deformities are a major orthopedic problem in patients with osteogenesis im¬
perfecta. Spinal surgery in these patients has been discredited because of the high in¬
cidence of short and long term complications. The authors suggest that surgical
approach could have its validity in these patients, providing that interventions are per¬
formed at a small angle or after an appropriate preparation by halo traction to correct
the major curves, and complications avoided through a multitude of precautions.

Idiopathic scoliosis may encompass more than a single disorder and may include
well defined neurologic abnormalities. The authors observed that a large percent of
left sided thoracic curves were signs of syndromic, neuromuscular, or congenital
diseases, and thus warn that a true major left thoracic scoliosis should prompt an
investigation of a possible nonidiopathic etiology and an awareness of a higher
progressive potential of those curves.
