What is known today is only a fraction of the real potential that exists for
the field of endosseous dental implants. Dramatic increases in the number
of reports on the results from clinical studies in this area have occurred in
recent years; however, some fail to meet the standards associated with
scientific clinical studies, thereby introducing flawed data into the clinical
knowledge base. Dentists should be familiar with the basic features on an
ideal clinical study design to enable them to assess the degree of validity
of published and oral reports. This article briefly outlines such features
and suggests points that should be included in clinical reports.

As implant prosthodontics continue to become a more popular option for
the restoration of eduntulism, the population who will most benefit will
be in the older age group. Many of these individuals have a high incidence
of chronic systemic illnesses. This article will review the possible systemic
risk factors for endosseous dental implant failure and present research
data on this issue.

level of patient satisfaction. Failures that occur may be related to patient

plants. Appropriate attention to the regional anatomy and the application

Good communication among members of the implant team comprised of
the oral surgeon, restorative dentist, laboratory technician, and the patient
is the first and most important component for successful and satisfying
implant treatment outcomes. Excellent surgical technique is also important
for success, but can still involve compromise of results without communi¬
cation and team interplay toward prosthetic goals. This article describes
important perioperative factors and surgical misadventures that contribute
to compromised implant placement.

nents specifically designed for compromised situations.

The Influence of Tobacco Use on Endosseous Implant Failures 255

As part of the Dental Implant Clinical Research Group s comprehensive
experimental design, data was recorded for confounding variable tobacco
use. Comparisons of implant failure through 36 months after uncovering
were made for current smokers, previous smokers, and never smokers in
relation to use of preoperative antibiotics, hydroxyapatite coating, bone
density, patient ASA status, prosthetic application, patient gender, peri
odontal indices, and Periotest values. The results continue to implicate
tobacco use as a significant risk factor for implant failure.

The design and manufacture of dental implants can have a profound effect
on implant survival. All implants must be manufactured from biocompati
ble material, but it is not known if one material is better than another. The

influence of implant design and composition on implant success or failure
is presented. The controversy concerning the effect of hydroxyapatite
coating on long term implant success is reviewed in detail. Clinicians must
use valid research and their clinical experience to select the appropriate
implant for their patients.

The condition of soft tissue may effect peri implant health in positive and
negative ways. Historically, the transgingival implant component which
perforates through the soft tissue, has been blamed for many of the
adverse sequela that develop after implants have been restored. This
article will discuss soft tissue gingival conditions and implant surface
characteristics that influence implant failure.

Our understanding of the soft tissue inflammatory lesion known as peri
implantitis is increasing as further studies are conducted. This article
reviews what is known about the peri implant soft tissues and how they
compare to natural dentogingival tissues, then examines the bacterial
etiology of peri implantitis, and concludes with a discussion of the current
treatment methods for detoxification of an infected implant surface.

The success of osseointegrated implants for the treatment of the partially
edentulous patient has been high, but not as high as originally anticipated
based on the success of the mandibular fixed edentulous bone anchored
bridge. Today we have a much better appreciation for the differences
between the edentulous mandible and the partially edentulous patient.
The treatment planning guidelines for the edentulous patient cannot sim¬
ply be correlated to the partially edentulous patient. This article illustrates
inherent biomechanical differences of the partially edentulous patient, and
provides treatment planning recommendations to eliminate the complica¬
tions and failures seen in the partially edentulous group.
Treating the Failing Implant 323

Prevention, interception, and recovery are the watchwords of restoring the
failing implant. First stage implant failure can be prevented, but not
treated. Early second stage failure may represent a biologic failure or
injudicious technique. Late, second stage failure is usually the result of
lack of care, mechanical loading, or poorly understood inflammatory con¬
ditions. Treatment of mechanical overload is directed to the fixtures, the
prosthesis, or both. Peri implantitis requires antimicrobial, implant surface
cleansing, and surgical intervention.
