Iatrogenic Illness: An Overview with Particular Reference
to Neurologic Complications 1

Neurologic illness relating to health care delivery has been increas¬
ingly described since the 1960's, primarily in the general medical
literature. Iatrogenic neurologic complications have either been de¬
fined generically in terms of consequences of particular therapeutic
or diagnostic approaches, or have been delineated with reference
to more specific and serious complications such as intracerebral
hemorrhage. In these reports, little attempt has been made to sit¬
uate iatrogenic neurologic complications, either historically or con¬
ceptually, within the larger framework of medically related harms.
Moreover, in this literature, few suggestions have been provided
with regard to strategies to reduce such complications. The objec¬
tive of the present review is to place the problem of iatrogenic
neurologic complications within a larger historical and conceptual
framework, as well as to provide suggestions for limiting such in¬
juries.

Technological advances in the management of cardiovascular dis¬
orders have resulted in an expansion of eligibility criteria for treat¬
ment, as well as an increased demand for improved outcomes.
Neurologic complications after coronary artery bypass surgery,
particularly stroke and cognitive dysfunction, substantially in¬
crease mortality, strain health care resources, and reduce the clini¬
cal effectiveness of the procedure. Carotid endarterectomy can be
both the optimum stroke preventative strategy as well as a cause
of stroke. The trend toward minimally invasive endovascular pro¬
cedures, which has provided non surgical options for both coro

from epileptic patients who are undergoing inpatient evaluation
for epilepsy surgery is also discussed.

This article provides an overview of the clinically relevant compli¬
cations of the main immunosuppressants currently used in the
treatment of neurologic disorders with suspected or established
immune etiology. The most serious complications are discussed
in detail, including pathophysiology and preventative measures.
An intimate knowledge of these complications proves helpful in
the day to day practice of the neurologist supervising immuno
therapy.

Care of the pregnant patient is challenging because of the multiple
physiologic changes associated with pregnancy and the need to
consider the impact of any intervention on the fetus. This article
addresses management issues that arise while caring for patients
with epilepsy, eclampsia, stroke, multiple sclerosis, and headache.
An emphasis is placed on considerations involving medication use
and approaches to patient care are suggested.

There is often a fine line between medical malpractice and maloc
currance. Physicians need to develop techniques and skills that en¬
hance the patient physician relationship based on honesty and in¬
formed consent. Documentation and proper maintenance of
medical records is demanded by the current legal and health care
systems. Failure to follow programs which maintain the integrity
of patients' records often has negative consequences. The cases dis¬
cussed illustrate areas in which the neurologist has come into con¬
flict with the legal system.
