between patient and physician. Careful assessment of history and physical
findings will help prevent surgery day surprises and reduce risk of compli¬
cations. This article outlines essential components of the preoperative
patient evaluation.

Wound care after cutaneous surgery can play an integral role in wound
healing. This article reviews wound healing physiology and the roles of
topical agents, such as antiseptics and topical ointments, and the numerous
occlusive dressings presently available in postsurgical wound care. The
use of prophylactic antibiotics in preventing wound infections and endo¬
carditis is discussed in the setting of cutaneous surgery. General wound
care guidelines are also provided.

ing by second intention. In general, an elliptical excision exhibits faster
healing, better cosmesis, and yields a specimen for histologic exam and
review of margins, and with proper attention to the details of cutaneous
anatomy, familiarity with instrumentation, local anesthetics, suture materi¬
als, and observance of meticulous surgical technique, is performed with
relative ease in the dermatology outpatient office setting.

This article reviews nasal reconstruction. It will summarize the anatomy,
anesthesia, and the anatomical subunits of this complex structure. It then
reviews reconstruction of various subunits of the nose by secondary inten¬
tion, primary closures, full thickness skin grafts, and local flaps. These are
repairs commonly used by the dermatologic surgeon.

The ear is a common site for the development of cutaneous malignancies
such as basal cell carcinomas, squamous cell carcinomas, and melanomas
as well as benign processes including nevi, epidermal inclusion cysts and
chondrodermatitis nodularis chronic helices among others. This article
focuses on the treatment of these entities. Repair options for the ear
are discussed including complex linear closure, flaps, grafts, and wedge
excisions along with the anatomy of the ear, anesthesia, wound care, and
possible complications.

quire excision either for biopsy or treatment. Several surgical techniques
used for excision and repair of the lip in the treatment of these conditions
are reviewed. Topics covered include the importance of the lip, lip anat¬
omy as it pertains to cutaneous surgery, techniques for obtaining lip
anesthesia, partial thickness and full thickness lip excision and repair,
surgical vermilionectomy, and carbon dioxide laser vermilionectomy. Em¬
phasis will be placed on those surgical techniques that might commonly
be used in a general dermatology practice.

The permanent eradication of ectatic veins while minimizing the incidence
of adverse effects, minimizing patient discomfort, and treating the maxi¬
mal number of veins during each session are goals of injection sclerother¬
apy. Accomplishing these goals requires more than skill at cannulation
and injection into a vein. An understanding of the mechanism of action of
sclerotherapy, and knowledge of lower extremity venous anatomy and
hemodynamics are crucial. Insuring a satisfactory outcome also requires
that the patient is thoroughly informed about the procedure and its possi¬
ble risks. In addition to addressing these issues, this author presents the
salient aspects of his pretreatment consultation, posttreatment recommen¬
dations, and appropriate documentation.
