Today s critical care nurses are faced with numerous challenges related to the complexity of critical
illness, the rapidity of changing technology, and the comprehension and application of scientific
advances related to the treatment of critical illness. The past decade has provided the bedside nurse
with an overwhelming body of literature regarding wound healing and the inflammatory/immune
system response during critical illness. A myriad of risk factors, such as advanced age, oxygen
and perfusion imbalances, diabetes mellitus, obesity, pharmacologic agents, malnutrition, infection,
radiation, and immunocompetence may negatively affect wound healing in the critically ill patient.
This article discusses the physiology of wound healing and risk factors that may impede wound
healing in the critically ill patient.
Wound Assessment and Decision making Options 125

Critically ill patients are at a particularly high risk for impaired skin integrity and for developing
complex, chronic wounds. Nurses who are experts in critical care may be unfamiliar with risk
assessment, evaluation, and management of both acute and chronic wounds. Factors that alter the
blood supply to the skin and underlying tissues are described in this article along with nursing
interventions to promote tissue perfusion and to avoid hospital acquired skin injury. An understanding
of the normal healing process and advanced wound care can assist the caregiver in promoting a
healing environment. The old rules no longer apply for wound care. Continuing research in wound
biology mandates a level of expertise that cares for the whole patient and not just the wound.

There are four lw.sk principles of wound care: (I) debride necrotic tissue and cleanse the wound to
remove debris. (2) provide a moist wound healing environment through the use of proper dressings,
(.4) protect the wound from further injury, and (i) provide nutritional substances essential to the
healing process. A plethora of wound care products are available on the market and are used to
support wound healing. It is essential that the critical care nurse thoughtfully selects a wound care
product that will promote a healing environment. A discussion of wound care products and the
scientific rationale for their use are presented.

The purpose of nutritional support in the critically ill is to blunt the effects of the stress response, to
preserve musculature and organ function, and to promote hepatic protein synthesis, tissue regenera¬
tion, and wound healing. Adequate nutritional support is vital to the patient with acute wounds. This
article addresses the body s response to physiologic stress and reviews means of supporting nutritional
status and the substrates used to maximize nutritional therapy. The critical care nurse will have a
better appreciation for how and when to implement nutritional therapy, appropriate nutrients, and
methods for monitoring nutritional status.

This article addresses methods of reconstruction for major acquired and traumatic soft tissue defects
in adults. The reconstructive ladder is used as a basis for discussing the various options for surgical
closure. Conditions such as facial trauma, head and neck cancer, median sternotomy, sternal osteomy¬
elitis, bronchopleural fistula, necrotizing fascitis, pressure ulcers, and degloving injuries are used as
examples of reconstruction. Nursing aspects, including postoperative flap monitoring, are reviewed.
Outcomes of reconstruction, including flap failure, are addressed.
Psychological Considerations for Patients with Acute Wounds 183

One s physical appearance is an integral part of one s sense of self. Any sudden alteration in one s
body image can initiate a grief response, anxiety, depression, and a host of unconscious defense
mechanisms. Nurses need to be well grounded in their understanding of both body image and
adjustment reactions to be effective in assisting patients to cope with the sequelae of wounding. This
article provides a brief overview of the psychological reactions of patients with acute wounds.

Diabetes mellitus is a complex disease that magnifies the problems associated with critical illness.
This disease seems to have a particular impact during the wound healing process, causing impaired
healing and an increased risk of infection. The complications of diabetes, such as macrovascular,
microvascular, and neuropathic diseases, further influence wound healing. It is essential for the critical
care nurse to have a clear understanding of the effects that diabetes mellitus has on seriously ill
patients to maximize positive outcomes for these patients.

Advances in the treatment and care of the burn wound have been largely responsible for the dramatic
increase in the survival and recovery rates among burn patients. Concomitant advances in topical
microbial control, surgical intervention, physiologic support, and associated research all have contrib¬
uted to the development of contemporary burn wound management. Critical care nurses who incorpo¬
rate these advances in their professional practice bring heightened knowledge and skill to the care
of the burn wound and assist in the recovery of the burn injured patient.

Because patients today are discharged from the acute care setting to the home care setting at a much
earlier time in their recovery period, wound care will be delivered by patients, family members, and
home health care providers, rather than by the hospital nurse in a traditional in patient setting. This
transition from an acute care setting to rapid discharge with home care for convalescence necessitates
the critical care nurse s active involvement in patient discharge planning and teaching regarding
wound care management.
