This article is a review of allergic disease in the elderly. As a
means of introduction, the cause and demographics of aging is
reviewed. Physiologic changes of aging that may impact allergic
disease are detailed. Since the immune response is central to
most disease processes, the immunology of aging is highlighted.
General recommendations in the care of the aging patient with
allergic disease are presented.

This article discusses the diagnoses, differentiation, and treatment
of allergic contact and irritant contact dermatitis and alerts the
clinician to things that can mimic contact such as neurodermatitis,
scabies, time, and drug reactions. Further, other common skin
problems of the elderly including pruritis, vulvar atrophy, and
actinic purpura are described and the clinical features of skin
cancers including basal cell squamous cell carcinomas, melano¬
mas, and skin metatases are outlined and illustrated.

Rhinitis and sinusitis are common bothersome conditions in the
elderly. Despite the importance of these diseases to the clinician,

little information exists in the medical literature. A review of four
recently published geriatric medicine textbooks reveals that not
one included rhinitis or sinusitis in the index.
Differential Diagnosis of Bronchial Asthma in the Elderly 557

Asthma is very common in the elderly, with a prevalence of
about 7% to 9%. Since many patients with asthma either currently
smoke cigarettes or have in the past, they frequently haved mixed
disease with features of both asthma (reversible airflow obstruc¬
tion) and COPD (fixed airflow obstruction). Since patients with
COPD often have a reversible component to their condition,
asthma medications may relieve some symptoms and improve
the patient s quality of life. The differential diagnosis of asthma
in the elderly needs to take into account not just COPD, but also
other pulmonary conditions such as upper airway obstruction
(UAO), interstitial lung disease (ILD), bronchiectasis, pulmonary
embolism (PE), and bronchogenic carcinoma. Asthma also needs
to be distinguished from nonpulmonary diseases, particularly
cardiovascular and gastrointestinal diseases. Finally, it is essential
to differentiate the normal, psychologic, and psychosocial
changes that accompany aging processes from abnormal changes
that accompany age associated diseases such as asthma.

Asthma is a common, chronic respiratory disease affecting pa¬
tients of all ages. Eosinophilic airway inflammation, often driven
by type I hypersensitivity to environmental allergens, is a critical
pathologic component of asthma. This article focuses on new
onset asthma in elderly patients, examining its typical clinical
presentation, epidemiology, and pathogenesis.

It is unusual for elderly patients to be free of hypertension,
coronary artery, or other types of cardiovascular and metabolic
disease. They are frequently a variety of therapies for these disor¬
ders that have to be considered before formulating a strategy for
the treatment of asthma. Thus, a clinical understanding of dis¬
ease disease, disease drug, and drug drug interactions between
asthma and other associated illnesses is imperative. This article
focuses on the major diseases, drugs, and drug interactions that
challenge us when treating asthma in the older patient. Although
clearly an integral part of a continuum of disease, asthma in the
elderly patient may be quite different.

Obstructive pulmonary disease, including both chronic obstruc¬
tive pulmonary disease (COPD) and asthma, is a significant cause
of functional disability in older adults that is often characterized
clinically by progressive breathlessness and increasing physical
limitations. This article examines how asthma, or reactive airflow
obstruction, alters physical capacity and human performance in
the elderly, and the role for exercise training in the management
of reactive airways disease among older patients.

The diagnosis and treatment of reactive airways disease in elderly
patients with cardiovascular disease are complicated both by
similarities in presenting symptoms and by the potential for one
disorder s pharmacologic therapy to exacerbate the other. Because
both heart disease and asthma are common in elderly adults,
their coexistence and interaction in an individual patient are
frequently encountered and are often a vexing clinical problem.
This article is written from the point of view of consulting cardiol¬
ogists, who are often asked to comment on cardiac causes of
dyspnea and the safety of certain medications in patients with
heart disease.

The prevalence of gastroesophageal reflux (GER) in the elderly
population may be as high as 20%. There is little information,
however, about elderly patients with GER associated asthma. The
exact relationship between GER and asthma is unclear, although
the association is well recognized. This article reviews the data
that link GER to clinical presentations, diagnostic tools, and treat¬
ment options.

People over 65 years of age constitute an ever increasing propor¬
tion of patients undergoing operations and are more likely to
suffer pulmonary complications postoperatively. Elderly patients
with pulmonary diseases such as asthma are at particularly high
risk owing to inability to clear secretions and take a deep breath.
Acute asthma attacks and infection should be treated before pa

tients undergo operations. When the patient is clinically stable,
pulmonary function tests and blood gases are important to assess
baseline function and guide preoperative therapy with bronchodi
lators and other adjunctive measures designed to decrease post¬
operative pulmonary complications. This article discusses the ef¬
fects of aging and asthma on the lung and how it relates to
postoperative pulmonary complications.
Lower Respiratory Tract Infections in Elderly Patients with
Asthma 671

Asthma continues to be a significant disorder for the geriatric
population. Although aging and asthma itself may predispose
individuals to more severe and protracted infections, the organ¬
isms and course of infections in elderly individuals are more
dependent upon associated comorbidities and overall physiologic
health, rather than any age of asthma associated immune defi¬
ciencies. Except for the recent association with Chlamydia pneu
moniae, and the known association with ABPA, the causes of
pneumonia in the elderly asthmatic are similar to those occurring
in nonasthmatic elderly. This article reviews the effects of aging,
nutrition, and asthma on the host immune system, and the role
of infection in the pathogenesis and exacerbations of asthma.
Additionally, the authors review admission decision analysis in
the elderly, concluding with preventive and therapeutic strategies
for the elderly patient with pneumonia.

Asthma is a chronic inflammatory disease of the airways, recog¬
nized to affect millions of children and young adults in the
United States. However, it is also a disease that is often underdi
agnosed and underemphasized in the elderly population, in
which many unique considerations become significant. The prev¬
alence of asthma in persons over 65 has been reported to range
from 6.5% to 40%.

Pulmonary involvement by the rheumatic diseases in the elderly
can have significant consequences. It can help in differential diag¬
nosis, since some rheumatic diseases frequently, and some never,
cause lung disease. The specific nature of the lung involvement
may be of less assistance, since many of the rheumatic diseases
share similar lung manifestations, and indeed many of the anti
rheumatic drugs can occasionally cause similar lung problems.

Pulmonary involvement may have major effects on the morbidity
and mortality of the underlying rheumatic illness, particularly
since the elderly more often have comorbid conditions that can
potentiate the process.
