Understanding the anatomy of the auricle is particularly important for the
reconstructive surgeon. The unique three dimensional framework of the ear
presents the surgeon with many challenges. Highlighted are the topographic
landmarks and the neurovascular contributions that must be considered
during surgery. The embryology of the ear is remarkable and without paral¬
lel in other organs of the human body.
Tissue Engineering for Auricular Reconstruction 311

Tissue engineering is a new area of biomedical science that has proven
useful for the creation of structural tissues. It involves the principle of
isolating cells, expanding them in culture, and seeding them onto biodegrad¬
able polymer meshes to create new structures. Under the right conditions,
chondrocytes treated in this fashion will generate sound neocartilage. An
approach to auricular reconstruction using this technology is described.

The identification of a patient with microtia necessitates a thorough evalua¬
tion of the patient s healthy status. When atresia is concomitantly identified,
the patient should be managed under the joint care of an otologist and
plastic surgeon. Close cooperation between the two disciplines is necessary
to ensure that the otologic and cosmetic results are maximized. Knowledge
of the anatomy and embryology associated with the atretic ear is vital. In
addition, understanding of the various classification schemes as they apply
to this problem will guide surgeons in the proper identification of surgical
candidates and the minimization of complications.

reconstruction. The article approaches auricular reconstruction by dividing
the ear into subsets and highlighting the currently available techniques of

Auricular Reconstruction With Osseointegrated Implants 395

An alternative to autogenous reconstruction of the malformed or trauma¬
tized auricle is the use of an external auricular prosthesis. The application
of osseointegrated techniques to the temporal bone for the placement of se¬
cure fixtures allowing implant bone prostheses has dramatically improved
the acceptability of prosthetic ears. The surgical technique for implant place¬
ment is safe and implant success rates in the temporal bone are excellent.
The aesthetic outcome and patient satisfaction with implant bone auricular
prostheses can be very high. A thorough understanding of the appropriate
application and limitations of this technology can help surgeons guide their
patients in choosing between options for auricular reconstruction.
Auricular Hematoma: Treatment of Wrestler s Ear 401
Charles S. Giffin
The auricular hematoma is the most common injury sustained by wrestlers.
It has been conclusively shown by Ohlsen and Skoog in 1975 that the main
problem in auricular hematoma is the regeneration of perichondrium and
the formation of new cartilage. This is stimulated by the presence of blood
beneath the perichondrium. This author has published two articles on this
topic and has advocated the complete excision of the perichondrial layer
containing the neocartilage; otherwise, a thickened ear will result. Simple
needling or incising the hematoma will only relieve the swelling temporarily,
but the neocartilaginous layer persists. This is a cauliflower ear.
