Solid organ transplantation is a common undertaking in the management of patients with end
stage renal, pancreatic, hepatic, cardiac, and pulmonary disease. Refinements in surgery, anes¬
thesia, immunology, critical care medicine, and infectious disease have all contributed to this
successful practice. This discussion focuses on developments in the perioperative care of
patients undergoing heart, lung, and liver transplantation. It addresses indications for pursuing
transplantation, patient selection criteria and evaluation, preoperative support, intraoperative
management, and postoperative care, with a particular focus on immunosuppression, rejection,
and prevention of infection.

Perioperative acute renal failure is a devastating complication in critically ill patients. The
mortality remains high despite dialysis. The most important consideration in acute renal fail¬
ure is prevention, primarily by providing adequate fluid resuscitation. Of pharmacologic treat¬
ments, atrial natriuretic peptide warrants continued investigation as treatment for early estab¬
lished acute tubular necrosis.

This chapter addresses the common indications for vasoactive therapy in the critically ill. It
reviews the various populations of critically ill patients and their unique response to inotropic,
chronotropic, vasodilating, and vasoconstricting drugs. Classic vasoactive drugs and new
agents are discussed. Synergistic and antagonistic effects of drug combinations are presented,
along with end points to guide appropriate care and complications associated with use of these
drugs.
Are New Therapies Effective in Acute Respiratory Distress Syndrome? 356

Acute respiratory distress syndrome (ARDS) continues to be a common, life threatening
process affecting the critically ill and challenging intensivists. Despite enhanced understand¬
ing of the pathophysiology of acute lung injury and ARDS, combined with newer techniques
to ventilate and support these patients, there are limited data demonstrating improvement in
survival. This review emphasizes prevention of ARDS, explores the association of mechani¬
cal ventilation, inflammation, and lung injury, discusses gas support therapies, and touches on
the current use of extracorporeal gas exchange in ARDS.

How Should We Feed Critically 111 Patients? 382

There is little evidence that nutritional support improves the outcome of patients in an inten¬
sive care unit (ICU). However, all clinicians agree that ICU patients who cannot ingest ade¬
quate nutrients must be fed at some point in time. However, it is controversial as to when one
should feed such patients, what route one should use to supply nutrients, the number of calo¬
ries that should be fed, and the composition of the diet. Information is presented suggesting
that patients should have caloric needs met by ICU days 5 to 10 (sooner for severely mal¬
nourished patients) using the enteral route. The goal is to supply kilocalories at 90% to 120%
of the calculated or measured energy expenditure, with 1.0 to 1.5 kcal/kg per day of protein
and the remainder of the calories as 70% glucose and 30% fat.

Antimicrobial resistance is an increasing problem for patients with community acquired and
nosocomial infections. This chapter touches on the mechanisms of the development of antimi¬
crobial resistance, discusses recognition and treatment of specific problem pathogens, and pre¬
sents strategies to prevent and control the emergence and spread of antimicrobial resistant
organisms. Infection control protocols for use by critical care practitioners are presented.

Sedatives, analgesics, and neuromuscular blockers are some of the most commonly adminis¬
tered drugs in the intensive care unit. This chapter reviews recent data identifying the need for
more objective investigation into the indications, proper application, efficacy, and risks and
benefits of sedative and neuromuscular blocking drugs in the critically ill. New sedatives and
paralytic agents, controversies regarding their proper application, current guidelines for cost
effective and safe use, and techniques to monitor the effect of these potent agents are dis¬
cussed.

Pain is common among the critically ill. It is frequently a challenge to quantify, and it is com¬
monly underestimated or considered relatively unimportant by critical care practitioners.
However, pain is associated with many deleterious systemic effects and should be limited or
eliminated whenever possible. This discussion highlights difficulties related to the implemen¬
tation of analgesic strategies in the intensive care unit, outlines available options, and suggests
some useful approaches in creating a logical plan for pain management.

Trauma and surgery alter the volumes and composition of the intracellular and extracellular
spaces. Therapeutic infusion of fluids further alters compartmental volumes and composition.
Appropriate management of fluids may limit surgical morbidity and mortality.
