Rotator Cuff Impingement: Pathogenesis, MR Imaging

Shoulder impingement can be diagnosed based on clinical findings, but
clinical staging of its severity, which may range from reversible edema and
hemorrhage to a complete rotator cuff tear, is often not possible. MR imaging
allows direct examination of the thickness, signal characteristics, and conti¬
nuity of the rotator cuff. Furthermore, MR imaging can be used not only
to determine the status of the cuff, but also to detect pathology involving
surrounding structures and to identify anatomic variations that may contrib¬
ute to the impingement. Rapid MR imaging during active unguided shoul¬
der motion will likely further increase our understanding of the biome
chanics of the shoulder at different stages of impingement, and may shed
further light on the pathogenesis of this disorder.
MR Imaging of Impingement and Rotator Cuff Disorders:
A Surgical Perspective 721

The role of MR imaging in the diagnosis of impingement and rotator cuff
disorders is expanding greatly. This article discusses the clinically pertinent
history, physical examination, and surgical treatment of rotator cuff prob¬
lems. A rationale for consideration of open, mini open, and arthroscopic
surgical intervention is proposed. The utilization of MR imaging by the
orthopedic surgeon for nonoperative, preoperative, and postoperative
decision making are presented. A close working relationship between the
radiologist and the orthopedist is emphasized.

MR imaging provides clinically useful information in detecting and charac¬
terizing pathology of the rotator cuff and other shoulder disorders in a
noninvasive fashion. Complete and partial tears of the rotator cuff as well
as factors contributing to impingement can be detected and characterized
with MR imaging. The size and location of complete tears of the rotator
cuff can be accurately determined with MR imaging.
MR Imaging of the Biceps Muscle Tendon Complex 755

The anatomy of the biceps brachii muscle tendon complex is reviewed.
Particular attention is given to the tendon of the long head. Pathologic
conditions affecting the biceps are discussed with respect to clinical features
and current ideas regarding pathogenesis, which are correlated with the
appearance at MR imaging.

instability by conventional MR imaging. In this article, the pathophysiology
of glenohumeral instability and the various MR imaging findings are dis¬
cussed and illustrated.
MR Arthrography of the Shoulder 811

Although conventional MR imaging allows direct visualization of major
anatomic structures, smaller intraarticular structures, including the glenoid
labrum, glenohumeral ligaments, and articular surface of the rotator cuff
tendon can be difficult to evaluate in the absence of a joint effusion. The
authors review the techniques employed in performing MR arthrography,
imaging characteristics of MR arthrography, and diagnostic utilization of
the technique. Suggested indications are provided.
MR Imaging of the Postoperative Shoulder 841

The field of MR imaging of the postoperative shoulder continues to evolve.
As the indications expand for obtaining postoperative shoulder studies,
there arises a need for delineation and definition of criteria associated with
these imaging studies. Further investigations are necessary to analyze these
data in greater detail, and to correlate these data to clinical situations.
MR Imaging of Arthritides Affecting the Shoulder 861

MR imaging has had a limited role in the evaluation of arthritis involving
the shoulder, despite studies that have shown this technique to be more
sensitive than radiography in the evaluation of osseous erosions and carti¬
lage loss. Factors responsible for limiting the use of MR imaging are its
relatively high cost, as well as its insensitivity to diminished osseous mineral¬
ization and subtle areas of calcification or hyperostosis. MR imaging findings
of some arthropathies, however—such as synovial osteochondromatosis,
PVNS, and amyloid arthropath—are highly characteristic, and help in deter¬
mining both diagnosis and treatment. Physicians also should be aware
that MR imaging is highly effective at diagnosing numerous secondary
conditions common in patients with shoulder arthropathies, including rota¬
tor cuff rupture, synovial cyst formation, and osteonecrosis.

During the course of routine MR imaging of the shoulder, a wide variety
of abnormalities may be encountered owing to the technique s broad soft
tissue contrast resolution and multiplanar tomographic capability. In this
article, the authors provide an overview of the lesions that might be encoun¬
tered in this setting.

MR imaging is the modality of choice for the evaluation of the brachial
plexus. Regardless of the cross sectional imaging modality used, a knowl¬
edge of the anatomy in the lower neck and thorax is necessary in the
interpretation of these studies. Before imaging an attempt should be made
to determine the possible cause and location of the lesion.
