Principles of Microsurgery 987

The surgical principles and techniques used in ophthalmic micro¬
surgery differ considerably from those used in general surgery.
Successful ophthalmic microsurgery requires that the surgeon
understand not only the design and complexities of the operating
microscope, but how tissues are affected by minute manipulations
with microsurgical instruments. Furthermore, ophthalmic micro¬
surgery requires a detailed understanding of how microsurgical
techniques need to be adjusted to accommodate the unique fea¬
tures of ocular tissues such as conjunctiva, cornea, and lens. A
diligent effort to master the principles of ophthalmic microsur¬
gery is probably the single most important prerequisite to becom¬
ing an accomplished ophthalmic surgeon.

Review of the functional anatomy of ocular adnexal tissues is
followed by presentation of surgical procedures aimed at correct¬
ing conditions of the eyelids, conjunctiva, and third eyelids of
small animals. Procedures used effectively by the primary author
are described in detail including instances where combination
procedures may be indicated. Some newer, recently described
techniques are also briefly discussed. Illustrations of applied anat¬
omy and multiple surgical techniques are provided by illustrator
and coauthor Dr. Gheorghe Constantinescu. References are given
to encourage readers to further explore alternative techniques
focusing on the surgical correction of adnexal diseases of dogs
and cats.
Surgery of the Cornea 1067

Corneal surgery is a common and essential part of veterinary
ophthalmology and ranges from simple linear keratotomy for
indolent ulcers to penetrating keratoplasty for restoration of opti¬
cal clarity. Success in corneal surgery relies on an understanding
of corneal anatomy, physiology and wound healing, meticulous
attention to detail, microsurgical equipment and techniques, and
use of appropriate preoperative, intraoperative, and postopera¬
tive medications. Surgical management of corneal disease is indi¬
cated for corneal ulceration, excision of a mass lesion, recon¬
structive procedures, therapeutic indications, optical restoration,
and cosmetic purposes.
Surgery for Glaucoma 1109

Most cases of glaucoma in small animals ultimately require surgi¬
cal treatment for long term control of intraocular pressure. Surgi¬
cal procedures that have the potential to preserve vision in acute
cases are categorized into those that reduce aqueous production
(cyclodestructive techniques). Salvage procedures for irreversibly
blind eyes include enucleation, implantation of an intraocular
prosthesis, and pharmacologic ciliary body ablation. The indica¬
tions, surgical technique, and complications of these procedures
are discussed in this article.
Phacoemulsification: Technology and Fundamentals 1131

The number one rule of phacoemulsification and aspiration cata¬
ract surgery is to know your machine. This chapter is designed
to help the surgeon who is currently using phacoemulsification,
or those who wish to understand more about technique, learn the

basics and technology of the various types of phacoemulsification
machines. Fluidics, pump design, handpiece mechanics, phaco¬
emulsification needles, and fundamentals of phacoemulsification
of cataracts will be reviewed.
Surgery for Cataracts 1143

The advent of phacoemulsification has substantially improved
the success rate of cataract surgery in dogs, whereas the develop¬
ment of artificial lens implantation has equally improved postop¬
erative visual acuity. In this chapter information pertaining to
the etiology, diagnosis, and management of canine cataracts is
provided for the general practitioner. More detailed information
on phacoemulsification and artificial lens implantation is pro¬
vided for residents in training or practicing ophthalmologists that
may be converting from extracapsular extraction.
Surgery for Lens Instability 1175

Lens luxation is a common hereditary ocular disease in terrier
breeds. If not detected early, lens luxation invariably leads to
vision threatening ocular diseases such as retinal detachment and
glaucoma. Successful management of lens instability requires that
the lens be removed as early as possible, and preferably before
luxation has occurred. With attention to exacting detail, the dis¬
placed lens cannot only be removed, but it can be replaced with
a prosthetic intraocular lens that is stabilized within the eye
with sutures of 10 0 prolene. With early detection and aggressive
surgical intervention, the prognosis for vision in dogs with lens
instability should be good.
Surgery for Retinal Detachment 1193
Thomas C. Sullivan
The treatment of retinal detachment in veterinary patients has
progressed within recent years and promises to improve with the
application of newer modalities. This article will review the basic
principles of retinal adhesion and the causes of retinal separation,
and thereby define a rational approach to determining what con
si titutes a surgical retinal detachment. Specific techniques will be
reviewed that have potential veterinary applications.
Surgery of the Orbit 1215

Orbital surgery is challenging to perform, specifically when a
sighted eye is compromised by orbital disease. Whenever orbital
surgery is indicated, complex orbital anatomy is invariably al¬
tered by disease processes. Orbital disease may be primary or

may be a manifestation of a systemic disease process. Some
orbital diseases are more effectively treated medically while oth¬
ers require surgical treatment. A thorough preoperative diagnos¬
tic evaluation is imperative to determine if orbital surgery is
indicated. Detailed familiarity of orbital anatomy and perception
of the probable orbital pathology are prerequisites before at¬
tempting orbital surgery. Preoperative characterization and local¬
ization of orbital disease permits the surgeon to select the most
appropriate surgical approach, plan for adjuvant postoperative
treatment, and provide the owner a realistic expectation for surgi¬
cal outcome.
