Endorectal ultrasound is a valuable diagnostic adjunct for anorectal tumors and
benign conditions such as incontinence and complex fistulas. The examination is
well tolerated by patients, and the images provide a clear and concise picture of
the regional anatomy.

Increased technologic capabilities have allowed for expanded use of ultrasound
beyond simple differentiation of a lesions solid versus cystic nature, to classifi¬
cation into various categories based on a number of descriptive features. Breast
ultrasound may be the preferred modality in the patient with questionable phys¬
ical findings and negative or nonspecific mammography, in younger women
before mammographic age, in pregnant patients, in evaluation of the postopera¬
tive field, in evaluation of possible breast abscess, and as a guide for a variety of
interventional procedures involving the breast including aspirations, core biop¬
sies, and localizations.

The enhanced sonographic images associated with cervical ultrasound provides
particularly useful information on pathology and function. Ultrasound of the
neck may be used to detect thyroid, parathyroid, and cervical lymph node pathol¬
ogy; even detecting lesions as small as 2 to 3 mm under appropriate conditions.
Ultrasound of the thyroid has increased to the extent that is has generally
replaced scintigraphy as the first line study of choice in many centers. It is sen¬
sitive in detecting cervical lymph node pathology and as an adjunct to guide
biopsy of vaguely palpable or nonpalpable cervical pathology.

Whereas before 1970, the diagnosis of arterial and venous disease was primarily
based on examination and arteriography or venography, the recent availability of
duplex ultrasound scan has revolutionized noninvasive testing and, in many cases, has
replaced invasive techniques, hi many cases, the Duplex Color Flow Scan has
replaced venogram in the diagnosis of deep venous thrombosis and arteriogram in the
evaluation of carotid disease. In addition to its use in the preoperative evaluation of
arterial disease, vascular ultrasound increasingly is used intraoperatively to evaluate
anastomotic patency. Duplex ultrasound may be used as a preoperative map of the
venous anatomy in patients undergoing bypass of access procedures.

Ultrasonography is rapidly becoming part of the diagnostic armamentarium of sur¬
geons in North America. Because it is noninvasive, portable, rapid, and repeatable, sur¬
geons can use the results of the ultrasound examination to prioritize resources for
resuscitation and evaluation of injured patients. Using basic ultrasound principles, this
examination can also be extended into the critical care setting for the detection of
pleural effusion as well as an unexplained decrease in hematocrit.

Although interventional ultrasound is used most commonly to guide biopsies of
masses involving the liver, breast, and thyroid, other indications include drainage of
fluid collections and/or abscesses, biopsy of retroperitoneal masses, as a guide for vas¬
cular access, as a guide for biopsy of cervical masses other than those in the thyroid,
and others. The availability of high frequency and multifrequency transducers with
increasingly specialized applications and associated equipment have facilitated an
ever expanded role for ultrasound as a guide for surgical intervention.

Since the introduction of high resolution B mode ultrasound in the late 1970s,
intraoperative ultrasound has been gaining acceptance in various general surgi¬
cal fields, including operations of the liver, biliary tract, pancreas, gastrointesti¬
nal tract, and endocrine system. This chapter reviews instrumentation, tech¬
niques, indications, clinical results, benefits, and advantages and disadvantages
of intraoperative ultrasound. Its appropriate use will improve surgical manage¬
ment during operations.

or hollow organ using an ultrasound endoscope or an ultrasound catheter. This
chapter reviews the instrumentation, indications, and controversies of

illustrate the findings and benefits derived from this procedure, especially from
the surgeon s perspective.

Appropriate education and credentialing in the clinical use of ultrasound is nec¬
essary for surgeons to become facile with this technology and to be able to use it
in their everyday clinical practice. This chapter provides specific examples of
educational curricula in ultrasound. It also reviews credentialing processes to be
applied by surgeons in the credentialing of other surgeons.
