In this article the authors review factors determining risk for the
development of colorectal cancer (CC) and their modification.
Emphasis is placed on understanding the difference between
average risk and high risk individuals. Risk factors including
genetics, diet, environment, and coexistent diseases are discussed.
The data regarding modification of risk via dietary, pharmaceuti¬
cal, and prophylactic endoscopic and surgical interventions are
reviewed.
Screening and Surveillance for Colorectal Carcinoma 579

Screening and surveillance examinations are effective in lowering
colorectal cancer risk. Screening tests have been demonstrated
to reduce colorectal cancer mortality. Colonoscopic removal of
adenomatous polyps has been determined to reduce colorectal
cancer incidence. High risk individuals and their family members
should be identified and offered more aggressive recommenda¬
tions for appropriate screening and surveillance guidelines. Colo¬
rectal cancer screening strategies are in an acceptable range of
cost effectiveness.

Colorectal cancer is the second leading cause of cancer deaths in
the United States. Because of this, the molecular pathogenesis of

medical community. An understanding of the genetic blueprint
for a malignant colonocyte will lead to avenues of intervention
to decrease the impact of the disease.

Management and survival in colorectal cancer are dictated by the
extent of the disease at the initial diagnosis. Technological ad¬
vances over the past 25 years have improved the ability to accu¬
rately preoperatively stage these lesions and detect recurrence.
This article reviews the focus on the utility of computerized
tomography, magnetic resonance, endoscopic ultrasound, and
newer imaging methods including PET scan and monoclonal
antibodies in the management of colorectal carcinoma.

Colorectal surgery remains the cornerstone of curative therapy
for colorectal carcinoma. The development of new instruments
permitting technical advances, however, as well as the advent of
effective adjuvant therapies and the progress in staging and early
detection, have changed some of the indications for surgery as
well as surgical methods. Even so, emphasis has always been
placed on thorough preoperative evaluation and staging. This
article explores the current state of standard surgical care of the
colorectal cancer patient with special attention given to preopera¬
tive evaluation, standard and controversial surgical therapies,
and postoperative surveillance.

Colorectal cancer is the third leading cause of cancer mortality
for men and women in the United States. For the past 40 years
fluorouracil has been the only agent with significant activity in
this disease. More recently, advances in our understanding of the
molecular biology of cancer have permitted the development of
effective new agents for this disease. This article examines the
current and future status of these new agents.

The majority of patients diagnosed with colorectal carcinoma die
of metastatic disease. The liver, peritoneum, and lung are the
most common sites of metastasis. Surgical resection improves
survival and offers the only chance for cure. The indications,
techniques, and results of resection and newer treatment methods
including cryotherapy, hepatic artery chemotherapy, and intra
peritoneal hyperthermic chemotherapy are reviewed.
Chemoprevention for Colorectal Carcinoma 779

The goal of chemoprevention is to modify risk factors that con¬
tribute to the development of cancer before the neoplastic process
can be completed. Chemoprevention for colorectal carcinoma has
focused on several groups of drugs that act via different mecha¬
nisms. Aspirin and non steroidal anti inflammatory drugs may
prevent colorectal carcinoma by inhibiting prostaglandin synthe¬
sis, specifically by inhibiting the enzyme cyclooxygenase 2. Fiber
may reduce the risk of colorectal carcinoma by increasing stool
bulk, decreasing bowel transport time of potential carcinogens,
or by binding to bile acids or other carcinogens. Calcium may
be chemoprotective by affecting cellular differentiation and/or
proliferation. The literature evaluating chemoprevention for
colorectal carcinoma is reviewed here.

Advances in tumor technology and gene transfer technology have
provided several new methods for treating colorectal cancer.

These new approaches are based on a more thorough understand¬
ing of the molecular aspects of the disease. New tumor vaccines
that are engineered to activate specific immune mechanisms are
now being tested in clinical trials. Gene transfer technologies
allow for the expression of a variety of genes within tumors that
may be of therapeutic potential. These issues are discussed in the
context of prior and existing methods of treating colorectal can¬
cer.
