Multiple techniques are currently available for MR imaging of the liver.
Optimization of MR pulse sequences is a critical issue in MR imaging of
the liver because image quality and diagnostic value greatly depend on
imaging protocol. Imaging protocols vary among institutions because of
differences in equipment and because of the absence of consensus regarding
the most appropriate sequence. This article presents an overview of the
basic MR sequences, highlights the most recent advances, and presents the
more promising future trends in MR imaging of the liver.
Contrast Agents 223

Contrast agents are needed for improvement of lesion detection and lesion
characterization on hepatic MR imaging. According to their biodistribution,
contrast agents are classified into extracellular, hepatocyte targeted, macro
phage monocytic phagocytic system(MPPS) targeted, and blood pool con¬
trast agents. Dynamic MR imaging of the liver enhanced with extracellular
gadolinium chelates improves lesion characterization. Hepatocyte targeted
and MPPS targeted contrast agents improve lesion detection. Hepatocyte
targeted contrast agents that have an initial extracellular distribution phase
and blood pool contrast agents offer a potential for improvement of both
detection and characterization of focal hepatic lesions.
Hepatic Hemangiomas 241

MR imaging is a very accurate technique for the detection and characteriza¬
tion of hepatic hemangiomas. The combination of T2 weighted imaging
and serial postgadolinium spoiled gradient echo imaging results in high
observer confidence.
Benign Liver Tumors 255

This article focuses on the main clinicopathologic and MR findings in the
more frequent benign liver lesions (excluding hemangiomas) such as cysts,
focal nodular hyperplasia, hepatocellular adenoma, and fatty tumors. These
entities raise several questions concerning their pathogenesis, differential
diagnosis from various malignant tumors or pseudotumoral hepatic lesions,
and management that remain frequently controversial.
Primary Malignant Neoplasms in the Adult 289

Primary malignant liver neoplasms of the adult are discussed with emphasis
on hepatocellular carcinoma, the most common neoplasm. The clinical and
pathologic aspects of the following neoplasms are correlated with imaging
features: hepatocellular carcinoma, fibrolamellar carcinoma, intrahepatic
cholangiocarcinoma, angiosarcoma, epithelial hemangioendothelioma,
other sarcomas, and lymphoma. The complementary role of ultrasound,
CT, and MR imaging in characterizing these lesions and determining resect
ability is highlighted.

Metastatic disease is the most common cause of malignant liver lesion in
the United States. This article focuses on the MR techniques utilized for
evaluation of the liver for metastatic disease, the MR appearance of hepatic
metastases, and several contrast agents being developed to further improve
detection of focal hepatic lesions by MR imaging.
Cirrhosis and Premalignant Nodules 331

In this article, the MR imaging appearance of various aspects of liver cirrho¬
sis (macroscopic morphologic changes and focal manifestations) is reviewed
with particular reference to the premalignant nodular lesions that should
be differentiated from hepatocellular carcinoma. MR imaging findings re¬
flecting portal hypertension are also presented.
MR Imaging of Diffuse Liver Disease: Hepatic Fat and Iron 347

This article reviews the various disorders that result in abnormal iron and
fat within the liver. MR techniques that detect and characterize fat and iron
are discussed. Chemical shift images are useful in detecting intracellular
lipid and can characterize diffuse hepatic steatosis as well as focal areas of
fatty sparing and fatty infiltration. T2* weighted gradient echo sequences
are useful in detecting hepatic iron. Typical imaging features of genetic
hemochromatosis and hepatic iron from blood transfusions are described.
MR Imaging of Inflammatory Disease of the Liver 367

Although MR imaging has achieved a clear role in the assessment of focal
benign and malignant hepatic lesions, its role in the evaluation of inflamma¬
tory processes in the liver is less well defined and studied. Many inflamma¬
tory diseases in the liver have no characteristic or specific imaging findings
with MR imaging. Familiarization with these entities and their respective
imaging findings is important, however, to aid in their identification and
distinction from the more commonly studied neoplastic disease.
Vascular Diseases and MR Angiography of the Liver 377

Delineation of the portal vein and its major attributes is essential for assess¬
ing portal hypertension and its complications. MR imaging appears to be
well suited for such an evaluation. This article provides an overview of the
MR imaging techniques used to assess the portal system as well as highlights
of the more common etiologies associated with altered portal venous flow.
Imaging of Pediatric Liver Tumors 397

Primary pediatric liver tumors are a unique group of neoplasms. The age at
presentation, clinical sigma symptoms, and imaging characteristics provide
helpful clues in making a prospective diagnosis and in tailoring a differential
diagnosis. The role of imaging is fourfold: characterization, localization,
determination of resectability, and follow up use. MR is usually complemen¬
tary to ultrasound and CT.
MR Imaging of the Liver—A Practical Approach 415

Clinical information must be combined with radiologic features to evaluate
focal or diffuse liver disease effectively, and MR examinations need to be
tailored appropriately. If there are characteristic radiologic findings, in many
cases a correct diagnosis is possible; however, radiologic methods may
provide an exact description of disease extent, vascular supply, infiltration,
metastases, and tumor changes like necrosis or hemorrhage, without provid¬
ing a definitive diagnosis. In these situations interventional radiologic tech¬
niques (such as percutaneous biopsy) have a potential role in diagnosis.
