Necrotizing fasciitis is one of the most serious infections in the clinical
arena. A delay in diagnosis leads to very high morbidity and mortality
rates. The emergence of toxic shock strains of Streptococcus makes it neces¬
sary to make a diagnosis and institute antibiotics and surgical interven¬
tion rapidly.

Sexually transmitted diseases (STDs) are common problems with world¬
wide distribution and immense social and economic costs. Changing epi¬
demiologies and drug susceptibilities make management of these infec¬
tions difficult. This article addresses the diagnosis and treatment of the
STDs that are most pertinent to dermatologic practice, including anogeni
tal warts, anogenital herpes, syphilis, chancroid, lymphogranuloma vene
reum, granuloma inguinale, molluscum contagiosum, and scabies.

The childhood exanthems include a spectrum of common and uncommon
disorders caused by a variety of pathogens. In this article, timely issues
relating to immunization for measles and varicella are discussed. Recently
reported exanthematous illnesses, including papular purpuric gloves and
socks syndrome, unilateral laterothoracic exanthem, and eruptive pseudo
angiomatosis, are described. The current research and debate about bacte¬
rial toxins as the cause of Kawasaki syndrome are presented.

Travel associated dermatoses are among the six most frequent medical
problems encountered by international travelers. Skin disorders caused by
parasites previously confined to the tropics are seen with increasing fre¬
quency by physicians in nonendemic areas. Cutaneous manifestations may
provide important clues to the underlying infection. This article describes
the epidemiology, assessment, and management of important parasitoses
with cutaneous involvement.

Individuals with immune defects, such as cancer patients, organ transplant
recipients, and HIV infected patients, are all at increased risk for the
development of opportunistic fungal infections. Superficial mycoses, in¬
cluding candidiasis, dermatophytosis, and pityrosporum, all may contrib¬
ute to morbidity. Aspergillosis and mucormycosis are particularly lethal
infections which require early diagnosis and therapy. Endemic mycoses,
such as histoplasmosis and coccidioidomycosis, also are life threatening in
the immunocompromised host. Penicilliosis is an emerging problem in
southeast Asia. A high level of suspicion and prompt diagnosis are crucial.
Spider Bites in the United States 307

The bite of the brown recluse spider rapidly causes an infarction in skin
that is painful and can be slow to heal. This is the only North American
spider to cause such severe necrosis. The bite sometimes can produce a
sudden, severe hemolysis. Conservative care without drugs is the safest
proven treatment.
Rickettsialpox 313

Rickettsialpox is a member of the spotted fever group of the rickettsioses
and results from an infection with Rickettsia akari. This microbe is transmit
ted by the bite of the house mouse mite Liponyssoides sanguineus. Patients
experience fevers, sweats, headaches, and a vesicular eruption over the
trunk and extremities. The palms and soles are spared. An eschar results
at the spot of the mite bite. Tetracycline is the treatment of choice.
Antiretroviral Agents: Current Usage 319

Recent advances in the understanding of HIV pathogenesis, the develop¬
ment of a new potent class of antiretroviral agents, and new data on the
effectiveness of older, less potent agents when used in combination are
resulting in a new era of more effective therapies for HIV. Three classes
of antiretroviral agents are approved for clinical use: nucleoside and non
nucleoside reverse transcriptase inhibitors and protease inhibitors. Prob¬
lems with the potency of therapy and resistance can be overcome to a
significant extent by using three agents in combination. Short term results
obtained with combination regimens are impressive and represent a con¬
siderable advance over older approaches to the treatment of HIV infection.
Data on the long term clinical and virologic effectiveness of new combina¬
tion regimens are needed, however, before the true impact of these treat¬
ments on the prognosis for HIV infected patients can be assessed.
Therapeutic Approaches to Papillomavirus Infections 331

Human papillomaviruses (HPVs) cause benign tumors (i.e., warts) and
are occasionally responsible for malignant tumors such as squamous
cell carcinomas. Therapy for most warts is commonly via surgical or
cytodestructive methods. Presently, only one antiviral/immunomodula
tory drug is available for wart therapy; this agent, interferon alpha (IFNct),
is approved only for genital warts (condylomata acuminata) and is expen¬
sive, relatively difficult to use, associated with systemic side effects, and
somewhat slow acting. Two new antiviral/immunomodulatory drugs,
imiquimod and cidofovir, have been proved to be effective and able to
overcome many of the shortcomings of IFNa. While these two agents are
pending approval, other treatments are being evaluated, such as antisense
oligonucleotides and therapeutic HPV vaccines. In contrast to surgical and
cytodestructive therapies, the goal of these new antiviral/immunomodula¬
tory agents is not just to remove the tumor but also to reduce sufficiently
the amount of latent and subclinical HPV so as to reduce the rate of
recurrence.
Current Aspects of Bacterial Infections of the Skin 341

The practicing dermatologist is faced with an ever changing epidemiologic
spectrum of cutaneous bacterial diseases. Studies have stated that bacterial
skin infections may account for up to 17% of clinical visits. It is hoped
that the information presented in this article will enable the practicing
dermatologist to provide improved patient care in the diagnosis and
management of bacterial infections of the skin.
