Antibiotic Selection in Obstetric Patients 1

Penicillin and ampicillin are of primary value in treating infec¬
tions caused by aerobic gram positive organisms such as group
B streptococci and enterococci. The extended spectrum cephalo
sporins and penicillins provide excellent coverage against the
broad range of pelvic pathogens. Clindamycin and metronidazole
target anaerobic organisms, whereas aminoglycosides and aztreo
nam are specific for aerobic gram negative bacilli. Erythromycin
and azithromycin are useful for treating obstetric patients with
infections caused by mycoplasmas and Chlamydia trachomatis.
Urinary Tract Infections Complicating Pregnancy 13

Urinary tract infections (asymptomatic bacteriuria, cystitis, and
pyelonephritis) are frequently encountered medical complications
of pregnancy. The majority of infections in pregnancy are asymp¬
tomatic; however, even covert bacteriuria places the mother at
risk for low birth weight and preterm birth. Pyelonephritis can
result in significant maternal and fetal morbidity and mortality.
Therefore, all pregnant women should be screened for asymptom¬
atic bacteriuria, and urinary tract infections should be promptly
treated to prevent adverse pregnancy outcome. This article re¬
views the diagnosis, etiology, treatment, and complications asso¬
ciated with urinary tract infections in pregnancy.

Perinatal Herpesvirus Infections: Herpes Simplex,
Varicella, and Cytomegalovirus 27

The herpesvirus infections (herpes simplex, varicella, and cyto¬
megalovirus) create many dilemmas when encountered during
pregnancy. This article reviews the epidemiologic diagnosis and
management of perinatal herpesvirus infections. A review of pos¬
sible future trends is also included.
Toxoplasmosis and Parvovirus B19 55

Infection of the fetus with Toxoplasma gondii may result in chorio
retinitis, hydrocephalus, or, rarely, death. Maternal ante natal
screening programs designed to diagnose and treat fetal infection
are currently impractical because of the low rate of seroconver
sion during pregnancy, the low positive predictive value of sero
logic testing, and the necessity for invasive testing to determine
appropriate antibiotic regimens. Similarly, maternal parvovirus
B19 infection may cause fetal infection, hydrops, and death, but
screening programs cannot be recommended. In the presence of
hydrops, fetal transfusion may be life saving, but hydrops may
resolve without therapy. There are no definitive guidelines for
determining which fetuses require transfusion.
Hepatitis in the Obstetric Patient 77

The six agents identified thus far that cause viral hepatitis are
reviewed, and their impact upon pregnancy is described. Al¬
though it is the most common cause of jaundice during preg¬
nancy, viral hepatitis does not generally increase the risk of
pregnancy complications, nor is it teratogenic. Vertical transmis¬
sion of some types of viral hepatitis does occur, however.
Lyme Disease During Pregnancy 93

There has been great concern in the past regarding the possible
fetal infection and teratogenicity from Lyme disease contracted
during pregnancy because of the similarities of disease caused by
Borrelia burgdorferi to syphilis. Although the initial retrospective
case reports were alarming, more recent prospective data have
been reassuring. This article reviews the evidence that currently
supports the assurance of the benign nature of this infection with
respect to the fetus.

This article reviews the parasitology of malaria, epidemiology in
pregnancy, pathogenesis of increased susceptibility to infection

tions of malaria, the effect of malaria on perinatal outcomes,
the safety of antimalarial agents during pregnancy, the role of
chemoprophylaxis for inhabitants and travelers to endemic areas,
and treatment of clinical malarial infections during pregnancy.
Obstetric Factors and Mother to Infant Transmission

Perinatal HIV transmission may be the result of a combination of
factors that cannot be isolated entirely. Among the obstetric fac¬
tors reported to be more strikingly associated with mother to
infant transmission are preterm delivery, low birth weight, birth
order in twin pregnancies, and the duration of hours of mem¬
brane rupture. The protective effect of the cesarean section is
still controversial. The use of potent antiviral drugs (protease
inhibitors) may prove to be more efficient in preventing such
transmission than a cesarean birth.

This article discusses the bacteriology, virology, and clinical pre¬
sentation of community acquired pneumonia in pregnant women.
In addition to treatment options, the effect of pneumonia on
pregnancy outcome is presented. The epidemiology, clinical pre¬
sentation, screening, and treatment for tuberculosis complicating
pregnancy is also addressed.

This article reviews the definition, frequency, microbiology, path¬
ogenesis, and clinical significance of intrauterine infections oc¬
curring in the context of preterm labor with intact membranes
and preterm premature rupture of membranes and then focuses
on the role of the products of arachidonic acid metabolism and
proinflammatory cytokines in the mechanisms of preterm parturi¬
tion associated with intrauterine infection.

Preterm delivery is the major cause of perinatal morbidity and
mortality in the United States. Corticosteroid therapy is one of

the most effective ante natal interventions in reducing infant
morbidity and should be administered if feasible. Acute tocolytic
therapy offers short term benefit to enable corticosteroid ad¬
ministration but is not without risks to the mother and fetus.
Adjunctive antibiotic administration after preterm premature rup¬
ture of membranes remote from term offers a reduction of chorio¬
amnionitis, prolongation of latency, and a reduction of neonatal
infections and gestational age dependent morbidity.

The purpose of this article is to review the incidence, microbiol¬
ogy, pathophysiology, diagnosis, and treatment of pregnancy
related infection, especially chorioamnionitis and endometritis.
The initial choice of therapy is generally empiric and the diagno¬

Perinatally acquired bacterial infections remain an important and
frequent cause of morbidity and mortality in newborn infants,
especially those delivered prematurely. Early onset neonatal sep¬
sis, defined as cases that occur in infants less than 7 days of age,
occurs in approximately 1 to 10 per 1000 live births. This article
reviews the pathogenesis, epidemiology, and bacteriology of
early onset neonatal sepsis. The management of the newborn at
risk for infection and possible strategies to prevent early onset
sepsis are also discussed.
