Intravascular ultrasound (IVUS) provides morphologic and morphometric
information about the coronary anatomy and its response to interventional
strategies. IVUS aids in the diagnosis of coronary artery disease with
respect to plaque composition and dimensions as well as in the guidance
of coronary interventions with respect to anatomic orientation, adequate
device selection, and documentation of the result. The description of basic
image interpretation and its limitations in this article reflects the current
status of development of this rapidly changing imaging format.

Using intravascular ultrasound (IVUS), stenosis formation and restenosis
(or late lumen loss following coronary angioplasty procedures) can be
subdivided into two distinct underlying components: tissue accumulation
and arterial remodeling. Arterial remodeling is defined as a change in
total arterial cross sectional area over time; it can be adaptive (an increase
in arterial cross sectional area as a compensatory response to plaque accu¬
mulation) or pathologic (a decrease in arterial cross sectional area or
chronic arterial shrinkage). Adaptive arterial remodeling can delay the
development of coronary artery stenoses and prevent restenosis; patho¬
logic remodeling can contribute to de novo lesion formation and has been
shown to be the dominant mechanism of restenosis following coronary
intervention. Serial IVUS studies have also been used to study the natural
history of the restenosis process; adaptive remodeling occurs early (within
1 month) and pathologic remodeling occurs late (between 1 and 6 months)

after intervention. The residual plaque burden postintervention acts as an
amplifier in this process.

Intracoronary ultrasound provides unique information during percutane¬
ous transluminal coronary angioplasty (PTCA), including more accurate
measurement of vessel size and plaque burden as well as plaque character¬
istics such as composition and distribution. As a research tool, it has been
useful in determining the mechanisms of PTCA, which primarily involves
vessel stretch, plaque fracture/dissection, and plaque redistribution. It
may be clinically useful in assessing lesion severity in patients with inde¬
terminate clinical and angiographic findings. Plaque characteristics as de¬
termined by intracoronary ultrasound may also be helpful in developing
an individualized interventional approach for each lesion. Finally, certain
intracoronary ultrasound findings after PTCA, such as large dissections
and large residual stenosis, are associated with increased risk of short
term and long term adverse outcomes.

The first section of this article reviews how intracoronary ultrasound
(ICUS) has been used to identify the mechanisms of action of directional
coronary atherectomy (DCA) and examines the influence of plaque compo¬
sition and morphology on DCA outcomes. The process of restenosis is
then described. Results from trials using ICUS guided DCA are evaluated,
demonstrating how the information obtained from ICUS is being used to
improve the angiographic and clinical outcomes of directional atherec¬
tomy. Finally, data are incorporated to provide practical applications for
the use of DCA.

Intravascular ultrasound (IVUS) imaging has dramatically increased un¬
derstanding of the process of coronary stent placement. Preintervention or
diagnostic IVUS has been shown to be of value before stent placement to
assess lesion severity and length as well as the degree and location of
calcification. Before stent placement, ultrasound dimensions may also be
used to select the appropriate type and size of device. To date, IVUS
imaging has greatly contributed to advancements in coronary stent place¬
ment techniques. The future of IVUS guided coronary stent placement
depends on the results of several ongoing clinical trials.

techniques of three dimensional reconstruction using intravascular im¬
aging data. The article also describes new intravascular ultrasound im¬
aging devices that have the ability to interrogate the arterial wall ahead of
the imaging catheter.

Coronary angiography incompletely delineates the physiologic conse¬
quences of many epicardial stenoses. Intracoronary flow velocity measure¬
ments using the Doppler flow wire provide data discriminating the physi¬
ologic significance of coronary lesions. Translesional and distal coronary
flow velocity dynamics during procedures can assess the physiologic ade¬
quacy of intervention. Impaired postintervention distal coronary flow ve¬
locity and vasodilator reserve predict subsequent clinical events, and phys¬
iologically inadequate results of angioplasty may be identified and
improved by additional intervention. Evidence in support of Doppler flow
velocity analysis as a clinically relevant technique for both diagnostic and
therapeutic aspects of cardiovascular medicine is discussed.
Clinical Application of Coronary Flow Reserve Using an
Intracoronary Doppler Guide Wire 101

Coronary flow reserve (CFR) is a critical measurement in the assessment
of the coronary circulation. The development of this physiologic variable
in animal and human studies is reviewed. Human studies documenting
the limitations of coronary angiography, especially in the setting of severe
diffuse coronary artery disease, are analyzed. Furthermore, the important
variables that must be accounted for when CFR is measured are examined.
With this background, the applications of CFR in a variety of clinical
settings and the development and use of the Doppler FloWire for its
measurement are discussed.
Coronary Angioscopy: Clinical Applications 131

Percutaneous coronary angioscopy is an alternative imaging modality that
can now be used routinely in the interventional cardiac catheterization
laboratory. The value of angioscopy lies in its ability to determine safely,
quickly, and accurately intraluminal morphologies before and after coro¬
nary interventions. The results from angioscopy can then be used to target
more specific treatment strategies. Percutaneous coronary angioscopy re¬
quires meticulous attention for the proper preparation and use of the
angioscope to minimize the potential risks associated with the imaging
procedure.
