Perioperative cardiac morbidity remains the leading cause of
death following anesthesia and surgery. This article focuses on
the preoperative assessment of adult coronary artery and valve
disease patients. The cornerstones in the evaluation of the risk
of morbidity and mortality are based on history and physical
examination and simple laboratory tests. More sophisticated exer¬
cise, stress, and radionuclide tests may be needed for selected
cases. The optimization of patients physical status and preopera¬
tive treatment is discussed also.

The choice of appropriate monitoring for patients with cardiac
disease presenting for noncardiac surgery is a difficult one. The
risks and benefits must be considered carefully, and many aspects
of the patient s underlying disease and the planned surgery will
influence the anesthesiologist s decision. This article summarizes
the advantages and disadvantages of a variety of noninvasive,

Regional Versus General Anesthesia 37

This article focuses on the results of randomized clinical trials
evaluating epidural or spinal anesthesia used either alone or as
an adjunct to general anesthesia in groups of patients at high risk
for cardiovascular morbidity. Several mechanisms for protection
of the heart by regional anesthesia have been proposed, most
notably, protection from the stress response to surgery. This
includes both protection from elevated catecholamines in re¬
sponse to surgical stress and protection from accelerated throm¬
bosis and maintenance of normal clot lysis. The randomized
clinical trials have had mixed results, however, and overall, have
not proven that adverse cardiac morbidity or death are reduced
by the use of regional anesthesia.

This article reviews preoperative evaluation and intraoperative
management from an evidence based approach. The preoperative
evaluation focuses on the recent American Heart Association/
American College of Cardiology guidelines, which incorporate
the clinical presentation of disease, exercise tolerance, and extent
of the surgical procedure. Studies evaluating the influence of
choice of anesthesia and monitoring are discussed from the per¬
spective of their influence on perioperative cardiac death and
myocardial infarction.

form of congenital heart disease (CHD). Anesthesiologists will

This article primarily addresses common problems and questions
that cause residents and general anesthesiologists to seek consul¬
tation from pediatric or cardiac anesthesiologists. Three general

An unfortunate reality in today s society is the rise in trauma,
especially violent trauma. A thorough understanding of the
mechanism(s) of injury and the primary and associated injuries

Those that present with cardiac injuries require aggressive and
timely treatment. A high index of suspicion is also needed, espe¬
cially for the blunt trauma victim who presents with numerous
distracting injuries.

neurologic and cardiovascular monitoring, and issues in postop¬
erative care and pain management.

This article discusses the management of patients with coronary
artery and valvular heart disease who present for abdominal
aortic reconstruction. It is a clinically oriented article with several
main foci. These include a section on preoperative evaluation,
which concentrates on clinically meaningful evaluation, risk strat¬
ification, and preparation of these patients; an intraoperative sec¬
tion, which pays careful attention to the hemodynamic stresses
associated with aortic cross clamping and unclamping; and a
section concerning the role of regional anesthesia and analgesia
in the perioperative management of these patients. Included as
well are sections concerning the effect of coexisting illnesses and
anesthesia for emergent aortic reconstruction.

Since the introduction of cyclosporine, cardiac transplantation has
become an acceptable therapeutic option for treatment of end
stage heart disease. With improved survival, the population of
living cardiac recipients is increasing; hence, there is a greater
chance that anesthesiologists will encounter these patients in their
daily practice. This article discusses the physiology of cardiac
denervation, the pharmacodynamics of cardioactive and vaso
active drugs, immunosuppressive therapy to include adverse ef¬
fects, preoperative, intraoperative, and postoperative anesthetic
concerns and management.
