This article reviews the role of platelet activating factor in repro¬
ductive biology, emphasizing its involvement in several processes
and the complications thereof. The authors believe that other
mediators and cytokines also are involved. Some investigators
have suggested a central role for one mediator that serves as the
trigger in each of these events. The authors believe that these
systems are regulated most probably by several affecters, among
which platelet activating factor is important.

Controlled ovarian hyperstimulation using injectable gonadotro
pins offers the benefit of recruiting multiple follicles for the pur¬
pose of in vitro fertilization or superovulation with intrauterine
inseminations. This article discusses the recent concepts regarding
the association of elevated estradiol levels in controlled ovarian
hyperstimulation and the potential risk for thrombosis as it affects
the fibrinolytic and coagulation systems.

Hemostasis During Hormone Replacement Therapy 35

Exogenous estrogen and progesterone supplementation in the
form of hormone replacement therapy in postmenopausal women
seems to protect against risks for cardiovascular disease. Hor¬
mone replacement therapy alters lipid profiles in a favorable way
and activates the fibrinolytic system without adversely affecting
the clotting system. There is no evidence that hormone replace¬
ment therapy increases the risk for arterial or venous thromboem
bolic events.
Antiphospholipid Antibodies: Immunology and Clinical
Consequences in Pregnancy 49

Procoagulant antiphospholipid antibodies (APA) react to proteins
complexed with phospholipids. In the case of lupus anticoagu¬
lant, the protein is either prothrombin complexed with hexagonal
phase phospholipids or B2GPI complexed with anionic bilayer
phospholipids. The latter antibodies also account for anticardio
lipin antibody reactivity. Additional phospholipid binding pro¬
teins that may be targets for APA include annexin V, protein C,
protein S, and yet to be characterized endothelial and trophoblast
antigens. Although APA may be caused by conditions likely to
be associated with poor pregnancy outcomes, such as systemic
lupus erythematosus, these antibodies directly interfere with tro¬
phoblast function and seem to promote placental thrombosis and
vasculopathy. Thrombosis seems to result from APA interference
with various phospholipid associated anticoagulant proteins (e.g.,
B2GPI, antithrombin III, thrombomodulin, protein C, and protein
S). Because of the common side effects of the various treatments,
meticulous diagnostic criteria are required to diagnose lupus
anticoagulant activity and anticardiolipin antibody reactivity.
Immunotherapy for Unexplained Recurrent Spontaneous

Repeated spontaneous abortion is believed to be a manifestation
of failure in protective mechanisms which can be induced by
alloimmunication either passively using pooled intravenous im
munoglobulin or actively by paternal leukocyte infusion. The
effectiveness of intravenous immunoglobulin in unexplained re¬
current spontaneous abortion seems to be enhanced by commenc¬
ing treatment before conception and maintaining it until the end
of the second trimester. Paternal leukocyte immunization is effec¬
tive in women who have unexplained primary recurrent abortion
and who have no antipaternal cytotoxic antibody and no autoim¬
mune abnormalities.

The hematologic and cardiovascular changes of pregnancy sig¬
nificantly influence the physiologic response to bleeding. General
principles of care can be applied regardless of the cause of bleed¬
ing. Specific causes of obstetric bleeding can be grouped ac¬
cording to the relationship to delivery. Peripartum hemorrhage
is typically caused by abnormal placentation, trauma, or a failure
of the natural mechanisms of postpartum hemostasis.

Intraoperative hemorrhage, which may result in anemia, hypovo
lemia, and even death, is one of the most stressful situations
faced by any surgeon. Extensive diagnostic and clinical expertise
is needed in the evaluation and treatment of a bleeding patient.
A precise diagnosis is essential because of the availability of
general and specific measures. This article discusses an approach
to the management of patients experiencing intraoperative bleed¬
ing.

Since the increased fear of infectious complications of transfusion
therapy, the indications for using blood components has been
scrutinized. Consensus recommendations from several panels and
committee are presented on the proper use of allogeneic and
autologous blood components. A periodic review of currently
accepted guidelines and criteria for the use of these products
behooves the clinician who may be faced with making such
decisions.

Anemia is a common finding and often indicates significant un¬
derlying pathology. As obstetrician gynecologists become more
involved in the primary care of women, it is important for them
to have a basic understanding of the problem. This article pre¬
sents a brief overview of the general approach and evaluation of
anemia and highlights some common types seen in women.
