Sonography is the most accurate method of evaluating patients
with suspected early pregnancy failure. This article reviews normal
sonographic findings and their embryologic basis and presents
current diagnostic criteria for embryonic demise. Sonographic pre¬
dictors of impending demise in patients with live embryos are
discussed also.

This article deals with the presence of chromosome anomalies in
single and recurrent spontaneous abortion. Unbalanced karyotypes
can repeatedly result from parental translocations and inversions.

Uterine anomalies are associated with an increase in the incidence
of miscarriages and recurrent pregnancy losses; however, early
abortions related to other causes may be more common in these
patients. The late abortions and immature deliveries mostly may
be caused by an associated cervical incompetence.
Endocrine Factors in Recurrent Miscarriage 721

Endocrine abnormalities undoubtedly underlie a significant pro¬
portion of recurrent miscarriages. Over the past decade, the focus
of research has switched from examining endocrine parameters in
the early weeks of pregnancy to looking at those factors that control
the quality of ovulation and the subsequent luteal phase. Hyperse
cretion of luteinizing hormone has been identified as a risk factor
for both infertility and miscarriage; however, suppression of high
luteinizing hormone levels before conception in women with high
luteinizing hormone levels, polycystic ovaries, and a history of
recurrent miscarriages does not improve the subsequent live birth
rate. It seems that high luteinizing hormone levels are a down¬
stream marker of another endocrinopathy that needs to be eluci¬
dated by further research studies.
Thrombophilic Defects and Pregnancy Loss 745

Individual components of the coagulation and fibrinolytic path¬
ways are important in embryonic implantation, trophoblast inva¬
sion, and placentation. Women with a history of recurrent miscar¬
riage are in a procoagulant state and have a defective hemostatic
response to pregnancy. The authors develop the theory that this
defective response is important in the pathogenesis of fetal loss in
these women.
Endometriosis and Spontaneous Abortion 759

Despite the lack of good quality evidence in the literature, there
seems to be an association between endometriosis and spontaneous
abortion. The association is consistent from study to study, and
treatment may reduce the risk for spontaneous abortion. Mild dis¬
ease seems to have a higher risk for spontaneous abortion than do
more severe categories. The mechanisms of action are likely to
include corpus luteum deficiency, immunologic factors in the peri¬
toneal fluid, and poor oocyte and embryo quality.

Most investigators agree that certain autoimmune diseases, such
as systemic lupus erythematosus or antiphospholipid syndrome,
are associated with an increased risk for pregnancy loss and that
some women with a chief complaint of fetal death or recurrent
pregnancy loss are found to have antiphospholipid syndrome. The
relationship between other antiphospholipid antibodies and auto
antibodies and recurrent pregnancy loss remains poorly defined.
In the future, investigators in this field will be obligated to design
and complete studies according to specific guidelines.

Infection frequently is cited as a potential cause of pregnancy loss.
Because few studies exist and results are inconsistent, the exact
role of infection in the first half of pregnancy as the cause of
pregnancy loss remains controversial. Several microorganisms,
including bacteria, mycoplasmas, viruses, and parasites, have
been found to be involved in sporadic occurrence of spontaneous
abortion. These infective agents may cause miscarriage during the
primary stage of infection, but after the initial phase, an immuno
competent mother produces specific antibodies that protect subse¬
quent pregnancies.
The Psychological Aspects of Recurrent Miscarriage 807

The key findings in both the anecdotal descriptive and research
literature concerning the psychological aspects of recurrent miscar¬
riage are reviewed. This critical analysis focuses on psychological
contributions to the understanding of the cause, impact, and treat¬
ment of recurrent miscarriage with the purpose of providing physi¬
cians with guidelines to facilitate emotional care of these patients.
Although the search for a relationship between personality factors
and cause has yielded little practical value, there are strong indica¬
tions for the routine use of emotional support and some indications
for the use of psychotherapeutically related interventions as an
adjunct to medical care for women with recurrent miscarriage.
Specific practice guidelines are suggested, and the role of enhanced
interdisciplinary research to clarify the psychological principles
and treatment of recurrent miscarriage is discussed.
Basic Workup of the Couple with Recurrent Pregnancy Loss 825

The field of recurrent pregnancy loss has seen a blossoming of
investigations in the evaluation of this condition over the past few

years. Although this is a welcome development, there remains
much controversy, which ranges from discrepancies in the defini¬
tion of recurrent pregnancy loss to elements in the workup and
the existence of certain conditions for which aggressive therapeutic
modalities are proposed. This article reviews the various defini¬
tions proposed and the standard elements in the diagnosis of recog¬
nized or suspected causative factors.
