An understanding of the means by which paratuberculosis is
transmitted is essential to controlling the spread of this disease.
Although fecal oral transmission from infected adults to calves is
the most important method of transmission, other methods such
as milk, colostrum, and in utero transmission are discussed. Most
paratuberculosis control or eradication programs must include
measures to prevent transmission at these critical points.

Disease caused by Mycobacterium paratuberculosis involves a com¬
plex interaction of lymphoid and phagocytic cells of the periph¬
eral and mucosal immune responses. For resistance to develop,
animals must generate an effective cellular immune response to
primary infections as well as multifocal exogenous and endoge¬
nous reinfections. If an effective immune response does not de¬
velop, infected animals transgress through a complex immuno
logic spectrum in which the immunologic reactions themselves
are responsible for the disease manifestations.

The term Johne s disease immediately brings to mind an image
of an emaciated, debilitated ruminant with bottlejaw and fluid,

pipestream diarrhea. The clinical case, however, is merely the tip
of the iceberg in terms of the total number of infected animals on
the farm. If the clinically affected animal was born on the farm,
a minimum of 25 other animals are probably infected and fewer
than 30% of those will be detectable by currently available tests.
Infected animals in the early stages of the disease progress slowly
over a period of months or several years to stages where they
may be diagnosed by antigen detection tests and antibody based
diagnostic tests. The duration of each stage of infection seems
dependent upon age at the time of exposure and the dose of
organisms ingested.

The currently available and accurate diagnostic tests for paratu¬
berculosis are reviewed. Technical issues concerning test perfor¬
mance are briefly mentioned and the advantages, disadvantages,
and comparative accuracy are described. The article concludes
with recommendations on which tests to use in eight different
situations requiring laboratory diagnostics for paratuberculosis.

The economic impact of paratuberculosis has been estimated in
dairy cattle for several areas of the United States and in some
other dairy intensive areas of the world. Losses are primarily due
to decreased milk production and unrealized income related to
premature culling. Milk production losses in the range of 15%
have been reported in paratuberculosis infected cows. Additional
economic costs include diagnostic testing, control measures, and
accelerated cull and replacement rates.

Uncertainties in the understanding of Johne s disease make the
pursuit of disease control confusing for producers and veterinari¬
ans. Generic control recommendations have limited adoption or
success because they fail to account for the unique circumstances
of individual farms. A systematic and pragmatic approach for
developing farm control plans is presented. It targets the develop¬
ment of plans that are adapted to individual farm circumstances
and are thus more likely to be implemented and effective.

Paratuberculosis is a chronic, fatal condition that is usually clini¬
cally undetectable until the onset of copious diarrhea. Pararuber

culosis is caused by Mycobacterium paratuberculosis. Successful
eradication of paratuberculosis depends on the early detection of
infected animals, allowing removal of carrier animals from the
herd. Treatment for paratuberculosis is therefore rarely indicated
or undertaken. Treatment may, however, be considered in rumi¬
nants of exceptional genetic value or in companion animals.

paratuberculosis as an etiologic agent in human Crohn s disease is
reviewed. Despite almost 15 years of investigation, the question
