This article presents the sequences followed in order to maximize
the yield of laboratory assistive diagnosis of parasitic infections
in the physiologic passage and/or collection of specimens for
those parasites producing pathologic disease in the gastrointesti¬
nal tract and liver. The advantages and disadvantages of the
many methods available to the clinician and laboratorian in the
examination of those specimens are presented in a work flow
mode. The controversy relative to the current practice of control¬
ling unwarranted specimen submission and the reasons for their
rejection of limited examination and interpretation are discussed.
Quality assurance and safety practices are also addressed.

bic dysentery and amebic liver abscess, and ranks third world

wide among parasitic causes of death. The application of molecu¬
lar techniques to the study of this organism have led to major
advances in understanding the pathophysiology of amebic infec¬
tion. This article reviews what is currently known about the
pathogenesis, clinical manifestations, diagnosis, and treatment of
amebiasis.
Giardiasis 493

Despite rapid progress in understanding the biology of Giardia,
several questions remain unanswered. First, there is no adequate
explanation for the diverse clinical spectrum of giardiasis. Second,
the mechanisms by which Giardia produces diarrhea and malab
sorption are poorly understood, although some progress has been
made. Finally, despite extensive studies in animal models and
human infections, the key immunologic determinants for clear¬
ance of acute infection and development of protective immunity
remain ill defined. This article discusses the epidemiology, pa¬
thology, diagnosis, treatment, and prevention of giardiasis.
American Trypanosomiasis (Chagas Disease) 517

Gastrointestinal dysfunction is a major problem for many patients
with chronic Chagas disease, as are cardiac dysrhythmias and
cardiomyopathy. The underlying anatomic abnormality in these
patients is a denervation of the gastrointestinal tract. This process
of nerve destruction usually develops insidiously over many
years, and it is highly variable in terms of its extent in individual
patients as well as in the segments of the gastrointestinal tract
that are most affected. Megaesophagus is the most common mani¬
festation of gastrointestinal Chagas disease, and mechanical dila¬
tion of the esophageal sphincter or surgery in advanced cases
usually give satisfactory relief of symptoms. Megacolon, particu¬
larly of the sigmoid segment, is also common in patients with
chronic T. cruzi infections, and its presence can be complicated
by fecal impaction or sigmoid volvulus. Patients with advanced
megacolon who have resections of the sigmoid colon and most
of the rectum generally do well postoperatively.
Visceral Leishmaniasis 535

Visceral leishmaniasis results from spread of the Leishmania sp.
protozoa including L. donovani, L. chagasi, and L. infantum
throughout the organs of the reticuloendothelial system. The dis¬
ease is associated with splenic, hepatic, gastrointestinal, and he
matologic manifestations. A new form of visceralizing leishmani¬
asis, termed viscerotropic leishmaniasis, was first recognized in
Persian Gulf veterans who were infected with L. tropica. Resolu

tion of visceral leishmaniasis requires both appropriate chemo¬
therapy and an adequate cellular immune response. Thus, there
has been a recent increase in visceral leishmaniasis due to coinfec
tion with HIV and L. infantum in southern European countries.
Ascariasis 553

Ascariasis is a helminthic infection in humans caused by the
nematode Ascaris lumbricoides. The infection is acquired by the
ingestion of the embryonated eggs and the larvae pass through
pulmonary migration phase for maturation. It has been estimated
that over 1.4 billion people throughout the world are infected
with A. lumbricoides. Pulmonary disease caused by Ascaris is
caused by larvae during their pulmonary migration and matura¬
tion. Ascan s induced intestinal obstruction is a frequent complica¬
tion in children with heavy worm loads in endemic areas. Pro¬
longed obstruction may be complicated by intussusception,
volvulus, hemorrhagic infarction of the bowel, and perforation.
Hepatobiliary and pancreatic ascariasis are two of the most com¬
mon and well described entities caused by Ascaris.
Enterobius, Trichuris, Capillaria, and Hookworm

Infections by the nematodes Enterobius, Trichuris, Capillaria, and
hookworm are some of the most widespread gastrointestinal in¬
fections worldwide. This article is designed not only to provide
an overview of the different parasites in terms of epidemiology,
clinical manifestation, and disease but also to highlight particular
areas of intensive research activity that are providing new in¬
sights into both the biology of the host parasite relationship and
possible new approaches to infection management for the future.
Schistosomiasis: Pathophysiology, Diagnosis, and Treatment 599

Schistosomiasis afflicts more than 200 million people worldwide.
Because international travel is commonplace, physicians around
the world need to consider, recognize, and treat this prevalent
parasitic infection. This article reviews the life cycle, epidemiol¬
ogy, pathophysiology, diagnosis, and treatment of schistosome
parasite infections, focusing on intestinal and hepatosplenic schis¬
tosomiasis.
Liver and Intestinal Flukes 627

Trematode infections of the liver and gut exclusive of schistoso¬
miasis include opisthorchiasis, fascioliasis, fasciolopsiasis, and

several other less common helminthic infections. Chronic Opisth
orchis liver fluke infections are endemic in Southeast Asia and are
therefore prevalent among immigrants to North America from
this region. Humans become infected with these trematode para¬
sites by eating fluke infested fish or water plants. The diagnosis
can be made by stool examination for eggs or specific serology.
Praziquantel is an effective treatment for all fluke infections ex¬
cept fascioliasis.
Tapeworms: Cestodiasis 637

Nearly all of the cestodes, or tapeworms (class Cestoda in the
phylum Platyhelminthes, the flatworms), are parasitic as adults
in the intestinal tract of vertebrates. This article discusses the
epidemiology, diagnosis, and treatment of taeniasis, diphyllo
bothriasis, hymenolepiasis, dipylidiasis, and other tapeworm in¬
fections.
Cestodes: Echinococcus 655

This article deals with cystic echinococcosis (CE) and alveolar
echinococcosis in humans caused by the cestode parasites Echino¬
coccus granulosus and Echinococcus multilocularis. The life cycles of
these parasites and their epidemiologic aspects are briefly dis¬
cussed, and a detailed review is presented on clinical aspects
of the diseases, including diagnosis and therapy. Considerable
progress has been made in chemotherapy but the results are not
yet satisfactory. A new alternative for treatment of CE is puncture
of cysts, aspiration of fluid, injection of ethanol, and reaspiration
of fluid (PAIR).
Parasitic Diseases in Immunocompromised Hosts:
Cryptosporidiosis, Isosporiasis, and Strongyloidiasis 691

Cryptosporidiosis and isosporiasis are intestinal infections caused
by the protozoan parasites Cryptosporidium parvum and Isospora
belli, respectively. HIV infection and other immunodeficiency dis¬
eases predispose human subjects to severe and prolonged cryp¬
tosporidiosis. There is also evidence that HIV infection predis¬
poses to chronic isosporiasis. Strongyloidiasis is caused by a
nematode worm, Strongyloides stercoralis. Administration of corti
costeroids to patients with chronic low grade S. stercoralis infec¬
tion can trigger a fulminant, life threatening form of strongyloidi¬
asis.
