Polyps of the gastrointestinal tract are common and often hard to
diagnose by endoscopic or gross examination. Biopsy or polypec
tomy enables diagnosis and prognostication. This article is by no
means encyclopedic, but attempts to discuss the pathology of the
more common intestinal polyps.
The Adenoma Carcinoma Sequence in Colorectal Neoplasia 513

In this article the epidemiologic, pathologic, molecular, and clinical
evidence for the adenoma carcinoma sequence is reviewed. The
authors describe the morphologic evolution of the precursor ade¬
nomas, from the earliest discernible abnormality of the colon to
adenomas with advanced pathologic features, the factors that de¬
termine the risk of progression of adenomas to adenocarcinoma,
the molecular events that parallel or underlie the morphologic
changes, the outcome of clinical interventions that have tested this
hypothesis, and the time frame within which this progression is
likely to occur.

Associations between polyps and cancer of the gastrointestinal
tract have been fully documented for adenomatous polyps and

cancer of the large bowel. The geographic, gender, and age distri¬
bution of colorectal adenomas parallels that of colorectal cancer
incidence, and individuals with a history of adenomas are at in¬
creased risk for metachronous adenomas and cancer. Observa¬
tional studies of risk factors for incident adenomas have found el¬
evated risk associated with dietary variables including: high fat,
low fiber, and low vegetable intake; family history; obesity; smok¬
ing; and other exposures, some of which also are associated with
cancer. Dietary intervention trials with adenoma recurrence as an
endpoint, however, have had little success. Screening with poly
pectomy has proved effective in reducing cancer incidence and
may change the epidemiology of colorectal cancer.

Existing guidelines for colorectal cancer screening in standard risk
patients are reviewed, as well as the data on which they are based.
A family history of colorectal cancer or significant adenoma in¬
creases the risk above that of standard risk individuals. Risk as¬

Many gastrointestinal polyposis syndromes have well defined clin¬
ical and histopathologic features. Recent advances in our ability to
analyze these syndromes at the genetic level have allowed for fur¬
ther characterization of these entities and their pathophysiology.
Elucidation of the underlying molecular genetic alterations that re¬
sult in the various forms of adenomatous polyposis syndromes has
been reported recently and is discussed in this article. Defining
these syndromes at the genetic level has important biologic and
practical implications.

Polyps and polypoid lesion of the small intestine present a major
challenge to the surgeon. In contrast to similar lesions in the large
intestine, small bowel polyps present late in their course and are
difficult to diagnose. Small bowel tumors produce vague symp¬
toms and screening tests are poor. Most of these polyps are found
at autopsy or during exploration for another cause. In this article,
the authors outline the problem as well as discuss the diagnosis
and treatment options.

The development of fiberoptic technology, specifically flexible sig
moidoscopy and colonoscopy, has lowered the mortality from co¬
lorectal cancer. Because colonoscopic screening of the entire pop¬
ulation is not practical, this procedure is reserved for populations
at high risk for developing colorectal cancer. This article discusses
the significance of finding benign colorectal polyps, reviews cur¬
rent screening and surveillance techniques, gives specific recom¬
mendations for colonoscopic surveillance for adenomatous polyps,
and comments on whether surveillance impacts on the incidence
and mortality from colorectal cancer.
Endoscopic Management of Malignant Colorectal Polyps 633

Malignant polyps of the colorectum are increasingly identified en
doscopically. The management of these polyps depends on tech¬
nical, histologic, and patient factors. Prognostic criteria, including
margin of resection, depth of invasion, degree of differentiation,
and lymphovascular invasion, are evaluated by way of a review of
the existing literature. Technical issues are addressed. Treatment
options and the rationale for selecting options are discussed.

Colonoscopic polypectomy has revolutionized the treatment of be¬
nign colonic neoplasia. It is performed widely by gastroenterolo
gists and surgeons and is a safe procedure. The two major com¬
plications of colonoscopic polypectomy include hemorrhage which

has a reported incidence of 0.2% to 3% and perforation which has
a reported incidence of 0.5% to 3%. Factors that increase the risk
of these complications and management strategies are discussed in
this review.

Ampullary tumors include a diverse group of benign and malig¬
nant neoplasms. A surgical approach to their management was
initiated in 1899 by Halsted and continues to evolve. Surgical op¬
tions include a spectrum of operative procedures designed to re¬
lieve symptoms and to remove malignant or potentially malignant
tumors. This article discusses the issues relevant to ampullary tu¬
mor management with emphasis on the currently accepted and
controversial aspects of the surgical approach.

with Resection in the Management of Complex Polyps

Surgical options available to patients with endoscopically unre
sectable complex polyps of the colon include laparoscopic colec¬
tomy, laparotomy with segmental resection, and a combination of
techniques using the laparoscope and the colonoscope. The deci¬
sion of which method to use depends on the nature of the lesion
as well as the choice of both the patient and the surgeon. The sur¬
geon must be knowledgeable in all modes of therapy.
Surgical Excision of Large Rectal Villous Adenomas 723

A significant percentage of villous tumors occur in the rectum.
These tumors have malignant potential, and complete excision
while maintaining anal sphincter function is the goal of treatment.
The surgical management of villous tumors of the rectum may be

accomplished by one of several transanal techniques, posterior ap¬
proaches, or intra abdominal operations depending on the location
and size of the tumor.
