The treatment of meniscal injuries has changed drastically over
the past 50 years. This evolution has been fueled by research on
the natural history, basic science, and biomechanics of meniscal
injury and by the advent of arthroscopy. This article presents a
brief overview of these developments.
Meniscal Repair Using the Inside to Outside Technique 455

The frequent occurrence of degenerative joint disease following
complete or partial meniscectomy is well recognized. Meniscal
repair has been shown to lead to a lower prevalence of degenera¬
tive changes in the knee. Arthroscopically assisted inside to out
side meniscal repair is a safe, reproducible technique for salvag¬
ing the torn meniscus.
Meniscal Repair Using the Outside to Inside Technique 469

The outside to inside technique of meniscal repair was developed
to decrease the risk of injury to the posterior neurovascular struc¬
tures. This technique is especially useful for tears in the anterior

Meniscal repair has become a commonplace procedure for the
treatment of meniscal injuries. Although clinical studies have
shown excellent results following repair of tears within the pe¬
ripheral vascular zone of the meniscus, various techniques have
been developed to optimize repair possibilities in the marginally
vascularized areas of the meniscus. This article examines the basic
science rationale for these repair enhancement techniques.

As arthroscopic techniques and equipment improved over time,
arthroscopic meniscus repair techniques were developed. Reports
in the literature suggest that single longitudinal tears in the pe¬
ripheral one third of the meniscus can be repaired arthroscopi
cally with predictable results. Complex tear patterns, such as
double longitudinal, radial, or flap tears, once were thought to be
unsuitable for repair. This article presents the authors guidelines
and technical considerations to be followed when attempting
arthroscopic repair of complex meniscus tears.

Recent interest in and awareness of laser technology applications
have developed in the field of orthopedic surgery. This article
presents a review of the literature relevant to the use of lasers for
meniscal repair and discusses techniques such as vaporization
and removal of tissue using laser heat transfer to tissue as well
as laser tissue welding.

Allograft Meniscus Transplantation 537

Allograft meniscus transplantation recently has become a poten¬
tial method of treating the meniscus deficient knee. The history,
immunology, processing and sterilization techniques, animal
studies, clinical trials, indications, surgical techniques, postopera¬
tive rehabilitation, and complications of this procedure are re¬
viewed. At this time, meniscal allograft transplantation remains
an investigational procedure with limited indications.
Meniscus Replacement 557

Meniscus replacement remains a difficult prospect. Attempts to
use artificial materials have led to synovitis and articular cartilage
wear. Autografts generally have failed to protect the tibial surface
and have led to scar tissue formation. Allografts of meniscus
cartilage from cadavers generally have shrunk in animal and
human tests to date, have failed in arthritic knees, and may be
complicated by immunologic reaction. Pain relief has been re¬
ported in pristine knees, however. Collagen scaffolds for menis¬
cus regeneration are showing promise in current human clinical
trials.
The Management of Meniscal Tears in
the ACL Deficient Knee 573

This article discusses the cause, recognition, and management of
meniscal tears in the anterior cruciate ligament (ACL) deficient
knee. An algorithm summarizing the authors preferred manage¬
ment of such tears is presented. Because meniscal repairs are at
a higher risk for retear in the ACL deficient knee, it is recom¬
mended that ACL reconstruction be performed in the otherwise
appropriate candidate. Nevertheless, repair of a meniscal tear
considered reparable is not contraindicated in the ACL deficient
knee that does not undergo ACL reconstruction.
Rehabilitation After Meniscal Repair 595

The literature indicates that most existing rehabilitation protocols
for meniscal repair are restrictive. At present there is no consen¬
sus regarding the ideal rehabilitation program following meniscal
repair. This article presents the clinical results of isolated meniscal
repair performed using the authors technique, followed by an
accelerated rehabilitation program consisting of no immobiliza¬
tion, immediate full weight bearing as tolerated, and early return

to sports. A prospective, randomized study of isolated meniscal
repair comparing various rehabilitation protocols is needed to
resolve the controversial issue of postrepair rehabilitation.
Complications of Arthroscopic Meniscal Repair 613

Arthroscopic meniscal repair is a well documented treatment op¬
tion for the injured knee. The potential for complications is also
well known. Precautionary techniques should be used to mini¬
mize the likelihood of complications. Appropriate preventive
steps combined with educating patients regarding the possible
risks will prove beneficial.
Meniscal Repair: Where Do We Go from Here? 621

This article discusses possibilities for the future of meniscal re¬
pair. As growth factors and tissue cloning become readily avail¬
able in future years, the methods of meniscal repair will change
from mechanical to biomechanical. The application of these new
techniques is a prelude to an exciting future in meniscal salvage
surgery.
