Magnetic resonance imaging of the beating heart is made possible by using
an external signal, the patient s own electrocardiogram, to time image acqui¬
sition. ECG gated cardiac MR examination may be performed using either
spin echo or gradient reversal techniques. Spin echo techniques provide the
highest contrast between rapidly moving blood and the cardiac chambers
and arteries and veins containing the blood, but are of relatively low tempo¬
ral resolution, restricting their utility in functional evaluation of the heart.
Gradient reversal acquisition, on the other hand, provides higher temporal
resolution, which is ideal for evaluating changes in myocardial thickening,
ventricular wall motion abnormalities, and changes in chamber volume
through the cardiac cycle; this acquisition technique, however, has signifi¬
cantly lower contrast resolution.

The evaluation and diagnosis of thoracic aortic disease using magnetic
resonance imaging, including magnetic resonance angiography, is assuming
an increasing role in clinical practice. Comparison with other modalities,
including computed tomography and echocardiography, is discussed.
Newer magnetic resonance imaging techniques, including 3 D breath hold
time of flight sequences are highlighted, as well as imaging strategies using
standard magnetic resonance imaging techniques. Thoracic aortic disease
entities covered include aneurysm, dissection, penetrating ulcers and intra¬
mural hematoma, aortitis and infection, tumors, and congenital aortic
disease.

The pericardium and cardiac malignancies are well visualized on MR im¬
aging because of the superb contrast resolution and multiplanar capability
of the technique. This article highlights some of the clinical uses of MR
imaging in evaluating the pericardium and cardiac lesions and provides
comparison with echocardiography and CT scan. Specific diseases that are
described include pericardia! effusion, constrictive pericarditis, neoplastic
pericardial disease, congenital abnormalities of the pericardium, benign,
malignant and metastatic cardiac tumors, and paracardiac masses. This
article discusses the impact of newer MR techniques such as cine MR im¬
aging on the assessment of pericardial disease and cardiac lesions.

Cardiac MR imaging is a noninvasive means of investigating the physio¬
logic state of the ventricular myocardium, as well as the effect on the
myocardium resulting from coronary insufficiency and valvular dysfunc¬
tion. The high contrast resolution of MR techniques and its exquisite
sensitivity to changes in local water concentration provides a means of
investigating local changes in the state of hydration of ventricular myocar¬
dium. Augmented by the administration of intravenous paramagnetic con¬
trast material, spin echo MR techniques allow demonstration of local myo
cardial ischemic changes. Furthermore, spin echo and gradient reversal
magnetic resonance imaging techniques allow evaluation of the morpho¬
logic changes brought on by acute and chronic ischemic atherosclerotic
heart disease.

The findings of MR imaging in hypertrophic, dilated, and restrictive cardio¬
myopathy are discussed. Axial spin echo views provide information con¬
cerning the distribution and severity of ventricular myocardial thickening.
Left ventricular outflow obstruction, systolic anterior mitral leaflet motion,
mitral regurgitation, and asynchronous left ventricular contraction are all
demonstrated in patients with asymmetric septal hypertrophy using cine
MR imaging. Cine MR imaging in short axis views allows quantitation of
left ventricular myocardial mass and chamber volume as well as evaluation
of abnormality in regional myocardial thickening. MR imaging demon¬
strates not only the anatomical abnormalities, but also functional derange¬
ment in patients with cardiomyopathy.

MR imaging cardiac quantitation is accurate, highly reproducible, and feasi¬
ble using equipment available in most hospitals. MR imaging is the reference
method for the determination of myocardial mass, evaluation of RV function,
and quantitative measurements of flow in the great vessels and periph¬
eral vasculature.
MR Imaging of Pulmonary Hypertension and Right Ventricular

Although right ventricular dysfunction may be the result of primary
disease of the right ventricular myocardium, it is more often caused by
chronic left ventricular and mitral valvular, pulmonary parenchymal, and
pulmonary vascular disease. Evaluation of the morphologic sequelae of
these diseases is difficult because of the complex shape of the right
ventricle as well as complicating abnormalities which prevent or seriously
limit conventional diagnostic imaging techniques. MR imaging techniques
are very well suited to evaluating the right ventricle. The large field of
view, high contrast and temporal resolution, and obviation of geometric
assumptions needed for quantitation of ventricular chamber volume and
myocardial mass allow accurate and reliable qualitative and quantitative
evaluation of the chamber in most patients.
MR Imaging of Congenital Heart Disease 327

Adult and pediatric patients with congenital heart disease may benefit from
noninvasive diagnostic cardiac MR imaging examination. Spin echo ECG
gated cardiac MRI provides wide field of view high contrast and spatial
resolution images of the heart in standard tomographic cardiac section.
Anatomic and morphologic information obtained from these images allows
characterization of the morphology of the cardiac chambers and great arter¬
ies, and accurate description cardiac situs as well as atrioventricular and
ventriculoarterial connections. Cardiac examination using gradient reversal
techniques provides quantitative functional information concerning cham¬
ber volume, wall thickness, and blood flow across cardiac valves and cardiac
shunts. MR examination may be used to directly demonstrate specific car¬
diac lesions, or provide additional inferential data concerning changes in
cardiac morphology allowing pre or postoperative evaluation of patients
with congenital heart disease.
Coronary MR Angiography 361

The principles underlying coronary MR angiographic pulse sequences are
reviewed, including basic coronary anatomy, motion, and flow physiology.
Coronary MR angiographic techniques can be subdivided into breath hold
(single or repeated) and nonbreath hold techniques. The recent introduction
of navigator pulses for real time respiratory gating or triggering allows
nonbreath hold or repeated breath hold 3 D coronary MR angiography, and
will allow a more widespread use of this technique.

With very fast MR imaging techniques, myocardial perfusion can be assessed
by monitoring the changes of myocardial signal intensity over time follow¬
ing bolus injection of a contrast medium. Very fast MR imaging techniques
also allow for freezing of cardiac motion that is important for coronary MR
angiography and their high temporal resolution allow for coronary flow
reserve measurements. In the current overview, basic principles and strate¬
gies of very fast cardiac MR imaging are presented. In the second part,
applications of these fast sequences in the fields of myocardial perfusion,
coronary MR angiography, and low measurements are presented.
