bronchoscopy is a necessity for the practicing general thoracic
surgeon.

Fiberoptic bronchoscopy is an excellent tool for the evaluation
of the airways and lung parenchyma. The history of flexible

Thoracic endoscopy encompasses bronchoscopy, esophagoscopy,
laryngoscopy, mediastinoscopy, and thorascopy (pleuroscopy). In
this article, the role of documentation as it applies to bronchos¬
copy is discussed. However, the discussion is generally applicable
to other thoracic endoscopy procedures.

Bronchoscopy with biopsies, bronchoalveolar lavage, and other
sampling techniques are frequently needed to establish definitive
diagnoses of pulmonary disorders. Combinations of specimens
provide superior results to single specimens alone for lung can¬
cer, including those which are endoscopically visible and periph¬
eral in location. Transbronchial biopsy is useful to establish tissue
diagnoses in certain diffuse parenchymal lung diseases with spe¬
cific recognizable histologic patterns such as sarcoidosis or eosin
ophilic granuloma, but it is less useful for disorders such as
interstitial pulmonary fibrosis. Patients with tuberculosis can be
diagnosed by performing bronchoscopy, but other sampling tech¬
niques are equally good and safer for the bronchoscopist and
other health care workers. Bronchoalveolar lavage is especially
valuable for confirming infectious complications in immunocom
promised hosts, and it also has great potential to elucidate basic
mechanisms of pulmonary diseases in research applications.
The Staging of Lung Cancer by Bronchoscopic Transbronchial
Needle Aspiration 223

The development of flexible needles has made the staging of
patients with lung cancer possible with flexible fiberoptic bron¬
choscopy. Combining the specificity of transbronchial needle as¬
piration (TBNA) with the sensitivity of thoracic CT scanning has
greatly enhanced the accuracy of this procedure. A bronchoscopi
cally oriented mediastinal and hilar lymph node staging system
is described which utilizes the complementary advantages of
these two techniques. TBNA is a safe, accurate, and effective
method to noninvasively stage bronchogenic carcinoma.

Endoscopic techniques for evaluation and management of airway
and pulmonary problems in pediatric patients have matured
greatly in the past decade, especially with the introduction of
flexible instruments suitable for use in the smallest infants. Care¬
ful selection of the most appropriate instrument and technique
by well trained medical or surgical endoscopists will result in
safe and effective diagnosis and treatment of a wide variety of
conditions affecting the upper and lower respiratory tract.

Despite many advances in instrumentation and anesthetic tech¬
niques, foreign bodies of the aerodigestive tract continue to
challenge even the most skilled and experienced endoscopists.
Complications, at times catastrophic, still occur. This excessive

morbidity and sometimes mortality often results from delayed
diagnosis, poor choice of instrumentation, improper technique of
extraction as a result of inexperience or unpracticed skills, and
poor communication between the anesthesiologist and surgeon.
In this article, we review current management recommendations
for airway foreign bodies in children and adults. Indications for
applications of new techniques in instrumentation are empha¬
sized and discussed along with the time tested methods of the
past.

Laser bronchoscopy allows reestablishment of patent airways in
patients with exophytic intraluminal tracheobronchial obstruction
from malignancy. Patients with benign strictures can also be
treated, thereby avoiding open surgical repair in selected in¬
stances. Because of laser tissue interactions, and because patients
often have terminal illness or severe underlying medical prob¬
lems, these are potentially high risk procedures. Proper resection
techniques, experience, and repetitive performance are necessary
to ensure satisfactory patient safety and outcome.

Intraluminal radiation for the local control of bronchogenic carci¬
noma has recently undergone rapid technological progress. Re¬
mote afterloading of high intensity radiation sources into endo
scopically placed multiple small catheters with computerized
optimization of dosimetry is the state of the art. Fractionated high
dose rate (HDR) outpatient treatment has been shown to reduce
airway obstruction and improve performance status and quality
of life.

The management of tracheal and bronchial stenoses remains a
problem for which few options of treatment are available. The
development of expandable stents for vascular occlusive disease
has provided a resource that can be applied to occlusive airway
disease. A number of stents are available, each with certain ad¬
vantages and disadvantages. They can be inserted more easily
than rigid prostheses, and can provide excellent palliation in
selected patients.
The Use of Fiberoptic Endoscopy in Anesthesia 329

The fiberoptic bronchoscope has contributed greatly to the prac¬
tice of anesthesiology over the past 30 years. It has become an

well as the placement and positioning of double lumen tubes for
thoracic surgery. The equipment, preparation, and methods for
using the endoscope in anesthesia, are presented here.

With the prevalence of bronchopulmonary disease in the inten¬
sive care unit, bronchoscopy has become an essential tool for the
management of patients. This article describes the variety of
situations in which bronchoscopy can be of assistance in establish¬
ing diagnosis, managing the difficult airway, and supporting
the patient s suffering of trauma, hemoptysis, atelectasis, and
pneumonia.

Two new potentially useful endoscopic diagnostic procedures are
described. Fluorescence bronchoscopy exploiting differences in
tissue autofluorescence between normal and abnormal tissues can
be used to localize areas of dysplasia or carcinoma in situ that
are barely visible or invisible under conventional white light
bronchoscopy. The depth of tumor infiltration or peribronchial
invasion to major blood vessels or mediastinal lymph nodes can
be assessed by endoscopic ultrasound and biopsied under sono
graphic guidance when necessary.

Management of disorders of the tracheobronchial tree have ad¬
vanced significantly since the first use of the rigid bronchoscope
by Gustav Killian. The addition of the flexible bronchofiberscope
by Shigeto Ikeda offered a second generation of new therapeutic
opportunities. The future use of thoracic endoscopy will depend
upon the application and improvement of technologies that cur¬
rently exist in addition to those not yet developed. There are
three current technologies which are being improved and may be
applicable in the future management of diseases of the tracheo¬
bronchial tree and these include cryotherapy, electrosurgery, and
photodynamic therapy.
