Oral cancer and precancerous lesions represent a diverse clinicopathologic
set of conditions in which accurate and early recognition is paramount.
Without an adequate understanding of the clinical manifestations and the
pathogenesis of these lesions, the clinician is handicapped severely. The
intent of this article is to familiarize clinicians with the pertinent clinico¬
pathologic features of a variety of the most common oral, premalignant
and malignant conditions. Particular emphasis is placed on the prognosis
of these conditions and how many of them are interrelated.

Human herpesviruses and human papillomaviruses are ubiquitous and
may cause an array of oral disease. Herpesviruses are unique because
recurrent localized disease occurs after the primary infection. Human
papillomaviruses are known for their ability to produce benign epithelial
proliferations and increasingly are implicated in the development of oral
cancer. This article reviews the pathogenesis of common oral lesions in¬
duced by these viruses.

Recurrent aphthous stomatitis (RAS) is the most common oral mucosal
disease in North America. In some instances, RAS represents the central
feature of the multisystem disease complex Behcet s syndrome. This article
reviews the clinical features, contributing etiologic factors, and etiopatho
genesis of RAS and Behcet s syndrome and describes therapeutic consider¬
ations and strategies essential to management of patients suffering from
recurrent mouth ulcers.

and the deglutition of food. This muscular organ is affected by many
conditions and diseases, including a variety of developmental, traumatic,
inflammatory, infectious, and neoplastic lesions.

Candidal infections are common oral opportunistic microbial disease en¬
countered in the practice of stomatology and oral medicine. With changes
in population demographics, there has been an overall increase in the
incidence in opportunistic mycotic infections. The increased involvement
of several candidal species in oral mucositis and perioral dermatitis is a
significant problem in medical dental practice. Recent advances in de¬
termining mechanisms involved and the clinical significance of these my¬
cotic organisms have confirmed the wide distribution, chronicity, and
other unique characteristics of Candida in localized mucosal and dissemin¬
ated disease conditions. Correspondingly, the development of alternative
antifungal regimens has enhanced the ability of clinicians to manage
candidal mucositis effectively.
Diagnosis and Management of Oral Lichen Planus 281

Oral lichen planus (OLP) is a relatively common dermatologic problem
with a wide range of clinical presentations. Although some types require
no treatment, the atrophic and erosive forms require confirmational biopsy
and careful management. Unlike skin lesions of lichen planus, OLP lesions
are chronic, sometimes lasting for 20 years or more. Management strategies
include careful explanation of the condition to the patient and the adminis¬
tration of topical or systemic corticosteroids.
Vesiculobullous Disease of the Oral Cavity 291

This article reviews the clinical features of vesiculobullous diseases with
oral manifestations. Oral vesiculobullous diseases may be viral, immuno
logically mediated, drug induced, hereditary, or traumatic in origin. A
general approach to the diagnosis is outlined, including irnmunofluores
cent testing of suspected vesiculobullous disease. Recommendations for
treatment of oral vesiculobullous diseases are included.

Clinicians recognize an increasing number of sexually transmitted diseases
(STDs). It is prudent for health care personnel to acknowledge the presence
of many STDs in the oropharynx and consider them in the differential
diagnosis of many well established conditions. The purpose of this article
is to review the clinical manifestations, diagnosis, and treatment of selected
diseases and conditions of sexual origin that may affect the oropharynx.

Oral lesions are common in HTV infection and may be the first sign of
AIDS. This article reviews the oral fungal and viral infections commonly
detected in HlV infected patients, particularly candidiasis, deep fungal
infections, herpes simplex virus infections, cytomegalovirus infections, and
oral hairy leukoplakia. The neoplasms associated with AIDS such as oral
Kaposi s sarcoma and lymphoma are related periodontal diseases. Each
disorder is discussed by clinical appearance, diagnosis, and management.
Recent advances in therapy are stressed.

Many medications can induce lesion of the oral tissues. Although many
of these lesions have a similar appearance to cutaneous counterparts, some
are distinctive. The objective of this article is to present the more common
oral lesions induced by medication. The clinical presentation of each entity
is described. Additionally, the etiology, treatment, and prognosis for each
lesion are presented.
Burning Mouth Syndrome 339

Burning mouth syndrome is a common condition particularly affecting
elderly women. Numerous precipitating factors are recognized that lead
to a burning sensation in clinically normal mucosa. By taking each precipi¬
tating factor into account, a favorable treatment outcome usually can be
achieved. This article highlights the significance of precipitating factors in
burning mouth syndrome and suggests a treatment protocol based on
current scientific evidence.
Swellings of the Oral Cavity 355

Many lesions that appear in the oral cavity manifest as a nodule or
swelling. A particular lesion may have the clinical characteristics of several
entities, and a differential diagnosis should be generated, with a final
diagnosis reached by microscopic examination. This article discusses and
shows the clinical features of some of the more common oral lesions or
conditions that present as localized or generalized swellings, and it in¬
cludes diagnoses and suggestions for management.
Melkersson Rosenthal Syndrome and Orofacial Granulomatosis 371
Roy S. Rogers III
Melkersson Rosenthal syndrome is a triad of recurrent orofacial swelling,
relapsing facial paralysis, and fissured tongue. The classic triad is not
always present at the same time, and the disease may be seen in incom¬
plete forms such as cheilitis granulomatosa. Exacerbations and remissions
are common. The presence of noncaseating sarcoidal granulomas on bi¬
opsy specimens supports the diagnosis, but these findings invariably are
not present. The Melkersson Rosenthal syndrome is a clinical disease with
elements of orofacial granulomatosis.

Epidermolysis bullosa is a group of systemic disorders whose manage¬
ment requires familiarity with its many extracutaneous complications.
These include gastrointestinal, ophthalmologic, laryngeal, dental, and he
matologic problems. This article reviews wound care and management of
systemic complications seen in patients with epidermolysis bullosa.
