Anatomy of the Asian Eyelid 1

The eyelids are complex structures with multiple functional layers. Varia¬
tions in these anatomic relationships lead to features that often are typified
as white or Asian. The white person s upper eyelid is thin with a lid
crease from 8 mm to 10 mm, whereas the Asian upper eyelid may have
an epicanthal fold, full lid, and lower or absent crease. This fuller upper
eyelid is caused by a prominent submuscular fibroadipose layer (preseptal
fat pad) derived from the brow fat and a weak inferior orbital septum
attachment that permits downward extension of the preaponeurotic fat
pad. The Asian lower eyelid also tends to be fuller. This is probably
because of a more prominent suborbicularis oculi and preretractor fat pads
with anterior superior extension.
Upper Lid Blepharoplasty: The Double Eyelid Operation External

Surgical modification of the upper eyelid has become an increasingly
popular operation in Asia since World War II. Currently, it is the most
commonly performed operation in the Orient. Although blepharoplasty as
performed for Asian and white patients is a similar procedure, there are
important anatomic and technical differences, and the facial plastic sur¬
geon must be aware of them to achieve optimal aesthetic results.

The most confusing and misunderstood aspect of blepharoplasty for the
Oriental eye is management of the epicanthus. This article describes a
range of procedures to allow the facial plastic surgeon to select an opera

tion that enables a modified epicanthus to blend harmoniously with the

Surgical intervention in the eyelid region must be precise and deliberate
to preserve the beauty and expressiveness of the eyes. Blepharoplasty
performed on Asian patients should concentrate on surgical modification
of the upper lid. This article presents anatomic considerations and the
selection of a technique for lower lid blepharoplasty.

Upper lid blepharoplasty is the most popular aesthetic procedure in Orien¬
tal patients and is related most commonly to the creation of a double
eyelid or westernization of the upper eyelid. To avoid various unsatisfac¬
tory results, a thorough understanding of the dynamic relationships of the
six cardinal structures of the upper eyelids is very important. Seven major
complications, including asymmetry of the eye fold, blepharoptosis, extra
skin fold, sunken upper orbit, surprised eyes, fading double fold, and
concealed double lids are discussed and explained with case examples.
Silicone Implants for Augmentation Rhinoplasty 55

Silicone implants as used for augmentation rhinoplasty in Asia are consid¬
ered to be the implants of choice. Avoidance and management of the
complications of this implant are discussed in this article. Emphasis is
given to the accurate shaping of the implant and a precise intraoperative
technique. Guidelines for the safe use of silicone implants are provided.

Asian nose. In the past, a variety of augmentation materials have been
used to treat the findings. The authors present a rhinoplasty technique

The goal of aesthetic rhinoplasty in the Oriental nose is similar to that of
occidental rhinoplasty—creation of a strong, smooth dorsum exhibiting a
prominent origin at the nasion but not competing with the tip as the
leading point of the nasal profile. An approach to nasal augmentation
should involve two separate considerations: (1) the surgical technique, and
(2) the design and construction of the implant.
Premaxillary Augmentation in Asian Rhinoplasty 93

One of the unfortunate potential consequences of all types of nasal surgery
is the failure to achieve proper tip projection. In this article, the author
presents his experience from 1984 to 1994 with 300 cases of premaxillary
augmentation, including an original Silastic implant, bone and cartilage
grafts, other allografts, and surgery of the anterior nasal spine and de¬
pressor septi nasi muscle (DSN). The sublabial approach under the DSN,
as applied to Asian rhinoplasty patients, is stressed.

The various nasal subunits pertinent to Asian rhinoplasty are described in
this article. These include the nasal tip, the nasal lobule, the columella,
and the boundaries of the nostril rim. With the increasing sophistication
of surgical techniques, new and bold procedures have been added to the
armamentarium of the plastic surgeon. Three techniques for improvement
of the nasal tip, correction of ala ptosis, and reduction of the size and flare
of the nostril commonly seen in the Oriental nose are described in detail.

The need to create or to restore harmony and balance to the facial features
with the use of augmentation mentoplasty remains well recognized by
facial plastic surgeons, yet few reports have been published in the litera¬
ture of the last decade. As the Asian population grows in the West, the
surgeon will be called on more frequently to meet the requests of first
and second generation patients. In this article, the author points out some
of the subtle differences between augmentation mentoplasty in Asian and
in white patients.

In most cosmetic surgery procedures, the patient s major concern is facial
contour changes that do not require the same considerations of function
but are more attuned to changes of form. This type of surgery has been
designated as interface surgery, aesthetic surgery of the facial skeleton, or
aesthetic contouring of the facial bone. Facial contouring surgery can be
divided into two different methods for obtaining a shape or balance of the
face: augmentation and reduction. This article deals with reduction skeletal
aesthetic surgery in the Orient, particularly with mandibular angle con¬
touring, malar reduction, and bimaxillary setback osteotomy on the basis
of the authors experience.

The goals of facial rejuvenation are identical in both Oriental and white
patients. Differences between the white and the Oriental face are mani¬
fested in skin thickness and texture, patterns of fat accumulation, and
perhaps most importantly, skeletal structure. These differences need to be
taken into consideration to formulate a proper surgical approach.

In some societies dimples and clefts are a sign of beauty, cuteness, and
strength. In other societies, they may indicate faithfulness and the ability
to produce many children. Simple methods of creating clefts in chins and
dimples in the face are described in this article. The procedures provided
have few complications and are not difficult to perform.

Trichloroacetic acid peel, when properly performed, is ideal for Asian
patients. A new patient classification that is based on the patient s race
rather than on his or her skin color is presented in this article. The action/
protection or one ladder/one base peel preparation program is dis¬
cussed. The fully standardized strip peel technique is explained. Post peel
care and possible complications are reviewed.

In recent years, the art of cosmetic tattooing flourished as a popular and
accepted method of cosmetic enhancement. Just as make up application to
an Oriental face differs from that to the white face, so does the art of
micropigmentation. An Oriental patient s unique facial features, skin tex¬
ture, and color can mislead the technician who is not familiar with Oriental
characteristics. This article focuses on what features to be aware of and
how to accentuate them through a variety of techniques and guidelines.
