This article provides a preventive or preemptive approach to endo¬
scopic complications based on the premise that the informed and
prepared physician can better ensure that an endoscopic procedure
will be performed under optimal conditions that result in maximal
patient safety. A directed patient history is one of the keys to this
preventive approach. This information can then provide the basis
for other preventive measures such as the administration of pro¬
phylactic antibiotics or the discontinuation of an anticoagulant.

discussed in this article, including intravenous access, medication
related complications, topical oropharyngeal medications, para¬
doxical reactions, and cardiopulmonary complications. Preventa

dures. The mortality rate is estimated to be between 0.5 and 3 per
10,000 cases. Cardiopulmonary events comprise 50% of all major
complications, and most of these events result from the medications
used for conscious sedation. Diagnosis, treatment, and prevention
of common complications are discussed; rare complications are
mentioned.

Endoscopic therapy has had a significant impact on the manage¬
ment of bleeding peptic ulcers and varices. Therapy is effective at
stopping immediate bleeding and has been shown to improve key
patient outcomes. Ulcer therapy has few significant complications,
whereas variceal therapy is fraught with serious complications,
which are reviewed.

Dilation techniques add to the complication rates of diagnostic
endoscopic procedures. Endoscopy assisted dilation methods have
the advantages of direct inspection of the strictured region, biopsy
capabilities at the same sitting, and greater success with difficult
strictures, but complication rates are not perceptibly different from
other dilation methods. Perforation is the most common, severe,
and important of the complications.

Complications can occur during both diagnostic and therapeutic
endoscopy. Most statistics concerning complications are based on
reports generated when colonoscopy was in its infancy. This article
examines the diagnosis and treatment of complications that arise
during the course of both colonoscopy and polypectomy. Surgery
may not be required in all cases. The criteria for operative or
nonoperative treatment are presented.

Percutaneous endoscopic gastrostomy has become the procedure
of choice for the establishment of enteral feedings in most clinical
settings. Minor modifications in the technique and tools of PEG
may have had some effect on the type of complications seen with
this procedure. The major and minor complications of PEG are
reviewed with a focus on those manipulations that may assist in
reducing the incidence of common complications of this procedure.

major complications are being encountered. This article discusses
complications related to operative laparoscopy, including those

surgery, and herniorrhaphy, are also discussed in detail.

Pediatric endoscopic procedures provide essential information for
diagnosis and the opportunity to undertake therapeutic interven¬
tion. Complications occur infrequently but represent an inherent
risk of invasive procedures. With careful planning, meticulous
observation, and proper experience, endoscopists may anticipate,
prevent, and recognize complications of upper gastrointestinal en¬
doscopy, colonoscopy, and suction rectal biopsy. Effective treat¬
ment of complications requires knowledge of the mechanism of
injury, assessment of the extent of injury, and design of appropriate
treatment, often in consultation with experienced colleagues in
gastrointestinal medicine and surgery.

Endoscopic retrograde cholangiopancreatographv (EKCP), to¬
gether with its substantial therapeutic capabilities, carries a higher
potential for complications compared with other endoscopic proce¬
dures. Complications of ERCP vary in severity from trivial to lethal,
and their presentation has a variable timing, the incidence of ERCP

complications correlates with physician experience and clinical en¬
vironments and may be underestimated if routine follow up is
not provided. Physicians should be fully trained in therapeutic
endoscopy, and their volume of procedure should be sufficient to
maintain therapeutic skills.
