This article reviews the recent trend of increasing adolescent
substance use, along with other trends of early onset of substance
abuse, interrelationships of substance abuse and mental disor¬
ders, multiple interrelated risk factors, and the recent emphasis
on prevention. A developmental biopsychosocial disease model
that integrates research, prevention, and treatment is presented.
Predisposing risk factors, substance use initiation and progres¬
sion, and enabling systems are discussed. Interventions are
described along the continuum of treatment and prevention, in¬
cluding an approach of building the prevention pyramid for
coordinating primary prevention efforts.

Sexual abuse is a complex behavioral phenomenon with many
divergent repercussions. The purpose of this article is to explore
the relationship between sexual abuse and adolescent substance
abuse. Theoretic formulations as well as empiric research help

shed light on some of the phenomenologic aspects suggested by
the association between adolescent substance abuse and sexual
victimization. Strategies for screening and treating these dual
disorders are offered.
Adolescent Substance Abuse and Internalizing Disorders 45

Clinical observation suggests that anxiety and affective disorders
are important in the development and course of psychoactive
substance abuse in some adolescents. These internalizing disor¬
ders sometimes precede and may contribute to the development
of psychoactive substance use disorders. Screening for internaliz¬
ing disorders, suicide risk, and trauma history is important in the
clinical evaluation of all adolescents with psychoactive substance
use disorders. Depression and anxiety symptoms may be caused,
however, by chronic alcohol or drug consumption or withdrawal.
Therefore, a period of abstinence and reevaluation should pre¬
cede specific treatments. Specific treatment for anxiety disorders
or depression may be appropriate in some substance abusing
adolescents. Focused psychotherapeutic techniques or psycho
pharmacologic treatments can sometimes be useful adjuncts to
psychoactive substance abuse programs.

This article reviews and updates reported clinical studies on the
relationship between substance use and substance use disorders
and suicidal behavior among adolescents. Issues regarding pre¬
vention of suicidal behavior are critically discussed, and sugges¬

There has been increasing interest in the overlap between atten¬
tion deficit hyperactivity disorder (ADHD) and psychoactive sub¬
stance use disorder (PSUD). In this article the relationship be¬
tween PSUD and ADHD and associated concurrent disorders
such as conduct disorder is addressed. A review of the literature
suggests a robust association between ADHD and PSUD; how¬
ever, the precise nature of this association remains unclear. Diag¬
nostic and treatment strategies for adolescents and adults with
ADHD plus PSUD are discussed.

manifest aggressive and violent behavior. The relationship be

tween adolescent substance use and aggression is complex, influ¬
encing the course of substance use disorders and the risk for
these disorders among adolescents. The presence of aggressive
behavior in adolescents with substance use disorders points to
the need for comprehensive treatments targeting aggression, sub¬
stance use, and other problems affecting the psychosocial func¬
tioning of adolescents.

is interactive and often synergistic, giving rise to increased patient

Strategizing treatment interventions with chemically dependent
teenagers and their families is challenging and frustrating. Profes¬
sionals are encouraged to examine the following frames of refer¬
ence using a biopsychosocial perspective: diagnosis, individual
development, knowledge about substance abuse and addiction,
stages of recovery and relapse prevention, family dynamics, sys¬
tems of support in the community, financial limitations, and

Empiric models demonstrate that adolescent substance abuse is
multiply determined. Multisystemic therapy is a social ecologic
model of treatment that addresses the multisystemic factors in¬

Preventing relapse first means establishing a state of recovery.
This involves completing the first step of a 12 step program,
whereby a teenager accepts that alcohol and drugs have been so
destructive in his or her life that he or she needs to be abstinent.
Moderate use is not possible if the teenager is at the stage of
severe abuse or chemical dependency. Being around alcohol and
drug abusing friends and the presence of psychiatric disorders
are the most common causes of relapse.

To understand the clinical ramifications of managed care and
managed competition, one must first understand the history of
the traditional fee for service system and its steady transforma¬
tion in the procompetition movement. This article describes the
history and operations model of managed care as well as delineat¬
ing its impact on adolescent substance abuse treatment planning
and care delivery. The role of assessment, outcome, treatment
planning, and treatment matching documentation is reviewed.
Finally, the ethical issues in managed care are discussed.

The Challenge Model: Working With Strengths in Children

This article presents the Challenge Model, a new clinical para¬
digm for evaluating the children of alcoholics and other sub¬
stance abusing parents. Unlike traditional risk paradigms, the
Challenge Model incorporates both the resilience and the vulnera¬
bilities that can result when children struggle against hardship
early in life. A developmental vocabulary of strengths is provided
for child and adolescent psychiatrists to use in research, clinical
thinking, and treatment and prevention efforts with children of
alcoholics.
