A Potential Target for Risk Factor Management 1

During the past 15 years, new concepts have arisen regarding regulation of
vasomotor control. It is now clear that the vascular endothelium modulates
vascular tone via the release of a variety of substances. Of these, nitric
oxide is predominant in modulating vasodilation. Nitric oxide also has a
number of other roles that are clearly antiatherogenic. Importantly, a
number of disease processes alter the release of nitric oxide and in fact
may serve as risk factors in part via this mechanism. In this review, the
mechanisms through which hypercholesterolemia alters vasomotion are
discussed, and interventions that may improve endothelial function are
considered.

Atherosclerotic coronary artery disease is a major cause of death in much
of the industrialized world. Acute coronary syndromes of unstable angina,
acute myocardial infarction, and ischemic sudden death, the most lethal
clinical manifestations of coronary atherosclerosis, result from rupture
or disruption of the atherosclerotic plaque with superimposed thrombus
formation. In this article, the potential mechanisms contributing to the
rupture of the atherosclerotic plaque with consequent thrombosis are
reviewed. The concept of plaque stabilization is discussed also.

This article describes the background and methodology of the published
studies assessing the effects of cholesterol lowering therapy and of treat¬
ment of multiple risk factors on the progression of coronary atherosclero¬
sis. The results are then presented, followed by a discussion of the lessons
that can be learned from this large series of similar trials.

Cigarette Smoking and Coronary Heart Disease: Risks and

It is estimated that 29% of all deaths from coronary heart disease are
attributable to smoking. The extent of harm caused by tobacco makes the
treatment of smoking, and not just smoking related disease, a high priority
for physicians. Evidence indicates that such treatment is not occurring.
This article reviews the effects of tobacco use and smoking cessation on
coronary heart disease and outlines an approach for physicians to use
when caring for smokers in inpatient and outpatient practice.

The role of diet in the development or prevention of cardiovascular disease
has been explored for the past half century. Overwhelming evidence
indicates that our western diet plays a major role in atherogenesis. We are
only now beginning to tease out the precise components of the diet that
are harmful or beneficial. This article reviews three dietary factors—fat,
alcohol, and antioxidants—that may explain some of the cross cultural
differences and secular changes in atherosclerotic disease rates.

Exercise is an essential component of the rehabilitative process for patients
with cardiovascular disease. As such, exercise is an important component
of antiatherosclerotic therapy. Physical inactivity is one of the four major
modifiable coronary risk factors. This article addresses these issues and
details the substantial supportive data and the elements in a proper exer¬
cise prescription.
Psychosocial Factors: Role in Cardiac Risk and Treatment Strategies 97

Recent epidemiologic research has identified several psychosocial factors
that impact unfavorably on prognosis among patients with established
coronary heart disease (CHD). Several biologic and behavioral characteris¬
tics have been identified in patients with these psychosocial risk factors
that are biologically plausible mediators of their adverse impact on prog¬
nosis. Several small scale clinical trials offer encouraging evidence that
both behavioral and pharmacologic interventions have the potential to
ameliorate the health damaging effects of psychosocial risk factors in pa¬
tients with CHD. In this article we review the evidence on these points
and offer recommendations as to how incorporation of this new knowl¬
edge into clinical care of the patient with CHD can lead to reduced
mortality and morbidity rates in this population.

Estrogens, Menopause, and Coronary Artery Disease 105

The incidence of cardiovascular disease increases after menopause as
women age. Estrogen replacement therapy has been found to reduce car¬
diovascular risk by about 50% in over 30 observational studies. Part of the
beneficial effect appears to be due to a reduction of LDL cholesterol and
an increase in HDL cholesterol. Another important mechanism is related
to the effect of estrogen on endothelial function. In making a decision to
use estrogen replacement therapy, the benefits must be weighed against
the risks of endometrial and breast cancer.
Lipid Lowering: The Case for Identifying and Treating the
High Risk Patient 117

Heightened interest in the identification and treatment of lipid disorders
in the high risk patient is derived from new findings in several areas of
research. Vascular biologic mechanisms have been elucidated through
which low density lipoproteins may cause both acute and chronic manifes¬
tations of coronary disease. Powerful new lipid lowering agents with
fewer side effects have become available. Both new and older lipid low¬
ering drugs have been examined in clinical trials, demonstrating reduc¬
tions in anatomic, clinical, and economic endpoints. This article reviews
the evidence supporting the aggressive management of lipid disorders in
the high risk patient.
Measurement of Short Term, Intermediate, and Long Term Outcomes
of Treating Hypertension 131

Hypertension is widely treated by primary care physicians as well as by
cardiologists and other specialists. Clinical progress is often monitored by
blood pressure measurements and other routine evaluations, but more
sophisticated approaches may be required to learn whether treatment is
effectively protecting the heart, kidneys, and other susceptible areas. Out¬
come measures therefore involve quantification of short term, interme¬
diate, and long term clinical observations; and, in addition, assessment
of overall effectiveness must take into account economic factors and the
levels of satisfaction perceived by patients, physicians, and health
plans.
Section III. Challenges in the Delivery of Secondary Prevention
MULTIFIT: A New Approach to Risk Factor Modification 143

MULTIFIT is a physician directed, nurse managed, home based program
for coronary risk factor modification that has been expanded to include
multiple chronic conditions associated with coronary risk factors (hyper¬
tension, diabetes, heart failure). MULTIFIT fosters cost effective care for
patients not only by favorably altering the natural history of chronic
conditions such as ischemic heart disease but also by reducing the transac
tional costs of care; it substitutes protocol driven care provided by phone
by specially trained nurses for some routine visits to the physician s office,
and it obviates some hospitalizations. The number of individuals with
established heart disease who can receive preventive services under
MULTIFIT is potentially in the millions.

Multiple barriers exist to the delivery of needed patient risk factor
changes. Some of these barriers are at the level of the patient, but there
also are many that inhibit the physician in delivering appropriate counsel¬
ing and treatment. Organizational barriers also exist. This article describes
these barriers and suggests integrated approaches that improve the
physician s ability to assist patients in making lifestyle changes.
