Cough is a common clinical complaint. Patients may not recog¬
nize its significance. Usually a careful history and a complete
physical examination aid the clinician in making a diagnosis of
the causes of cough. The most common causes of chronic cough
are asthma, postnasal drip sinusitis, and chronic bronchitis.
Differential Diagnosis of Adult Asthma 19

This article first outlines the clinical hallmarks that characterize
asthma. It also addresses various asthma masqueraders, focus¬
ing primarily on clinical presentation and diagnostic approaches.
Finally, conditions that commonly complicate and aggravate mod¬
erate to severe asthma are addressed.
Chronic Sinusitis 35

Sinusitis is a common but frequently underdiagnosed and under
treated condition. Because of its subtle presentation, the diagnos¬
tic index of suspicion for chronic sinusitis must be high. The use
of CT scans has helped considerably in diagnosis. Functional
endoscopic sinus surgery is the treatment of choice in medically

resistant cases. The association of sinusitis and asthma empha¬
sizes the relationship between the upper and lower airways.
Nonallergic Rhinitis 49

Not all that sneezes can be attributed to allergic rhinitis. Per¬
haps half of all rhinitis can be a result from a heterogenous group
of nonallergic nasal disorders. Distinguishing these disorders
from allergic rhinitis based on history alone can be difficult. In
this article we review nonallergic rhinitis, the differential diagno¬
sis, epidemiology, pathophysiology, and treatment.
Dyspnea Originating from the Larynx 69

The larynx is a critical anatomic region through which all incom¬
ing and outgoing air must pass. The fine, complex movements of
this fascinating area of our respiratory tract are carefully coordi¬
nated using an orchestra of neurologic and musculoskeletal
movements combined with proprioceptive, auditory, and cogni¬
tive inputs. Numerous infections and noninfectious conditions
may structurally interfere with airflow, and many require inter¬
vention for resolution to take place. Other conditions involve
abnormal movements of laryngeal structures, and some, but not
all, are amenable to treatment. The foundation for accurate diag¬
nosis of these laryngeal abnormalities is direct visualization of
the larynx. It is imperative that physicians who treat patients
with shortness of breath are familiar with the normal anatomy
and function of the larynx to accurately diagnose these conditions
and orchestrate their effective treatment.
Vasculitides with Prominent Respiratory Manifestations 77

The spectrum of vasculitis is extremely diverse and frequently
confusing. Several of the vasculitis variants can present with
respiratory symptoms that may masquerade as common allergic
disorders. Among the more prominent possibilities are the granu
lomatous vasculitides known for their prominent respiratory tract
manifestations. These include Wegener s granulomatosis, Churg
Strauss syndrome, and necrotizing sarcoid granulomatosis, which
are the subject of this review. Familiarity with these conditions is
crucial because early diagnosis and treatment may be critical in
achieving a good result.
Sjogren s Syndrome 91

Sjogren s syndrome is a chronic inflammatory disorder with mul¬
tiple systemic manifestations. Because this syndrome has promi

nent ocular and respiratory manifestations, patients frequently
present to the clinical allergist with suspected allergic disease.
Because many of the manifestations of this disorder can result in
significant medical morbidity, it is important for the clinician
to be aware of the prominent features and potential treatment
modalities of this condition. The protean clinical and laboratory
manifestations of this disorder are presented and effective treat¬
ment options are reviewed.

Many patients present with symptoms suspected to be allergic in
origin that ultimately turn out to be illnesses unrelated to allergic
disease. As there are a number of upper and lower respiratory
manifestations of endocrinopathies that may cause confusion with
allergic diseases, a review of these illnesses will be helpful for the
clinician. This review focuses on autoimmune thyroid diseases, as
Hashimoto s thyroiditis and Graves disease are the most frequent
masqueraders. Thus, the endocrinopathies should be carefully
considered in the differential diagnosis of many allergic diseases.
A careful history and physical examination, followed by appro¬
priate laboratory testing, will usually confirm the diagnostic clini¬
cal suspicions.
Allergic and Nonallergic Eczematous Dermatitis 119

Correctly diagnosing common diseases that present as eczema¬
tous dermatitis can sometimes be difficult. Although clinical fea¬
tures may suggest an allergic dermatitis, many are not mediated
by allergic response. Recognition of these common skin problems
allows appropriate evaluation and therapy. The allergic eczema¬
tous dermatitides discussed in this article are atopic dermatitis
and allergic contact dermatitis. These are compared with several
nonallergic dermatitides, including irritant contact dermatitis, sta¬
sis dermatitis, nummular dermatitis, dyshidrotic dermatitis, and
neurodermatitis.

There are thousands of agents that are added to the food we
consume and the pharmaceuticals we ingest. Fortunately, only a
small number have been associated with hypersensitivity reac¬
tions. This article examines these agents. Background information
concerning these chemicals and the mechanisms by which they
may provoke reactions are discussed. The association between
these agents and urticarial and asthmatic reactions is discussed.

Most adults presenting with food intolerance have other gastroin¬
testinal disorders, including irritable bowel syndrome, lactase
deficiency, or gastroesophageal reflux disease. Despite popular
perception, food intolerance is not a symptom of gallstone disease
or peptic ulcer disease. Most patients can be easily diagnosed
based on careful history and respond favorably to dietary manip¬
ulation and symptomatic medical therapy.
Is It Milk Allergy or Lactose Intolerance? 187

Milk allergy and lactose intolerance are two completely different
disorders that are often interchangeably misdiagnosed because
both are caused by milk and their symptoms may be similar.
They differ markedly in epidemiology, pathogenesis, morbidity,
treatment, and prognosis. Therefore, a definitive diagnosis is es¬
sential.

Some psychiatric disorders produce respiratory symptoms that
can mimic allergic airways disease. Panic disorder, hyperventila
tion, somatization disorder, psychogenic upper airway obstruc¬
tion and psychogenic cough are included in this group. This
article reviews the diagnosis and treatment of these conditions
and other social, familial, psychologic, and psychiatric aspects of
asthma. An approach to patients with symptoms and health care
utilization out of proportion to objective findings is included.
