Historically, pain has been associated with many different
meanings and views about its causes and appropriate treat¬
ment. Nurses have contributed to pain knowledge through re¬
search on psychosocial variables affecting the pain experience,
assessment and intervention practices, understanding patient
and family meanings of cancer pain, and sytematic studies of
pain intervention programs. Nurses also have been leaders in
promoting public policy on effective pain management and in
implementing and evaluating innovative programs on pain
management in a variety of healthcare institutions and agen¬
cies.
The Importance of Nursing Research Design and Methods in
Cancer Pain Management: Enhancing Care 597

Cancer is diagnosed currently in more than 1 million Ameri¬
cans every year and cancer pain is experienced by patients in
all stages of the disease. Even though research indicates that
optimal pharmacologic management alone can provide ade¬
quate relief for 70% to 90% of these patients, and additional re¬
lief can be obtained from nonpharmacologic interventions, the
problem of pain continues to exist. This article focuses on the

contributions of nurse scientists to the study of cancer pain
during the last 5 years. Selected contributions of nursing re¬
search designs and methods to the understanding of pain
caused by cancer and cancer treatment modalities are re¬
viewed. Limitations of present methodologic approaches to the
study of cancer pain and gaps in nursing knowledge are exam¬
ined. Recommendations for future nursing research designs
and methods used to study nursing management of cancer
pain and the implications of projected future treatment modali¬
ties also are discussed.
The Impact of Pain on Quality of Life: A Decade of Research 609

Pain and the quality of life are closely related. This article sum¬
marizes a series of studies conducted over the last decade con¬
tributing to the understanding of the pain and quality of life rela¬
tionship. Details about the meaning of pain, the experience of
pain in children, and the impact of pain on the family are pre¬
sented. Information obtained from research studies led to the de¬
velopment of pain education programs that strengthen direct
nursing care of patients and families experiencing cancer pain.
Family Members Perceptions of Cancer Pain:
Comparisons with Patient Sensory Report and by Patient
Psychologic Status 625

This article describes a pilot study comparing patient and fam¬
ily member perceptions of sensory pain and the relationship
between these perceptions and psychologic factors in patients
with lung cancer and pain. The authors findings indicate that
many family members knew where the patient had pain, but
family members rarely knew the intensity, quality, or pattern of
the patient s pain. Findings also indicate that family members
tended to overestimate strategies used by patients to cope with
pain, especially in patients with low levels of anxiety and in
patients with an internal locus of control. Similarities to other
investigators findings and implications for nursing practice are
discussed.
Patterns of Pain Experiences and Use of Analgesics Among
Hospitalized Cancer Patients in Korea 647

Concerns exist about practices of cancer pain management in
large teaching hospitals and in community hospitals. This article
presents information about pain severity and patterns of anal¬
gesic medication use for 450 postoperative patients with pain re¬
lated to cancer diagnoses in Korea. Recommendations to improve
cancer pain management also are included.

Impact of Surgery on Head and Neck Cancer Patients and
Their Caregivers 659

Little information has been gathered about the experiences of
head and neck cancer patients at the time of their initial surgery
and about the ramifications of the surgery for patients and their
caregivers. The authors discuss the pain experience of patients,
the relationship of pain to their fatigue and negative mood, and
the concerns of both patients and caregivers. Family caregivers
are expected to take responsibility for the patients, and it is im¬
portant for nurses to recognize the concerns of both patients and
caregivers at the time of discharge into the home setting.
Unscheduled Readmissions for Uncontrolled Symptoms:
A Health Care Challenge for Nurses 673

The purpose of this study was to measure the impact of nursing
strategies to improve cancer pain management on hospital read
mission for uncontrolled pain. Strategies include implementing a
pain resource nurse program (PRN), making pain management a
focus in the continuous quality improvement process of the insti¬
tution and creating a supportive care service. Admissions were
compared before and after implementation of the strategies. Re¬
sults for 1989 to 1990 revealed 5772 total admission with 4.4%
(255) admissions for uncontrolled pain; results for 1992 to 1993
revealed 4066 total admissions with 3.0% (121) admissions for
uncontrolled pain. Findings indicate that strategies were effective
in reducing the number of readmissions for uncontrolled pain.
Outcomes That Provide Evidence of Change in Cancer
Pain Management 683

Making improvements in the management of cancer pain is a
complex process involving patient and professional factors. This
article reviews the types of outcome measures that have been
used to document improvement in cancer pain management. Ex¬
amples from several studies are provided to illustrate the diffi¬
culty and benefit of documenting evidence of change in profes¬
sional practice related to cancer pain management.
Relaxation and the Relief of Cancer Pain 697

Progressive muscle relaxation combined with guided imagery
may promote cancer pain relief, but further scientific evidence is
needed. This article describes a randomized clinical trial of two
methods of instructing patients with cancer pain to use progres¬
sive muscle relaxation combined with guided imagery. Findings

indicate that both methods reduce sensory pain quality, pain in¬
tensity, and pain severity when compared with patients not using
the techniques. Implications for clinical nursing practice are dis¬
cussed.
An Algorithmic Approach to Cancer Pain Management 711

Pain and symptom management algorithms have helped a rural
hospice develop a standardized plan of care for terminally ill pa¬
tients and offer flexibility related to the individual differences
found in this patient population. The algorithm process has in¬
creased interdisciplinary collaboration and communication,
much to the benefit of the patient.
Sublingual and Oral Morphine Administration: Review and
New Findings 725

Clinical reports rave about the efficacy of sublingual morphine,
but most research data suggest that sublingual morphine lacks
the necessary physical characteristics to be absorbed through
sublingual tissues. This article clarifies these assertions by re¬
viewing the clinical literature that supports sublingual adminis¬
tration, the theories relevant to sublingual morphine administra¬
tion, and the pharmacokinetic research that supports or negates
the benefit of this route. Recommendations for clinical nursing
practice are provided to guide decision making in care of pa¬
tients with cancer pain.
Pharmacologic Management of Cancer Pain 745

This article describes three types of cancer pain—somatic, vis¬
ceral, and neuropathic. Descriptions of the location, intensity,
quality, and temporal pattern of the cancer patient s pain can be
used to determine the appropriate pharmacologic management
of the pain. Improved cancer pain management is possible when
etiology of the pain and the mechanisms of drug action are con¬
sidered in development of a therapy plan.
