also must deal with an unhappy patient who has already had one operation

Nasal tip ptosis after primary rhinoplasty surgery often occurs because of
an iatrogenic loss of nasal tip support. To correct this particular deformity
fully, the surgeon must reestablish nasal tip support in the destabilized
nose. This article highlights the use of the open rhinoplasty approach for
correcting the postrhinoplasty ptotic nasal tip. The concept of structural
augmentation using autologous cartilage grafting and precise suture stabili¬
zation is discussed.

Disproportion of the alar columellar relationship is a common anatomic
variant in a patient presenting for rhinoplasty. Alar retraction and hanging
columellar deformity can result from inexact surgery. Surgical procedures

agement are presented in this article.

Reconstruction of the postrhinoplasty or post traumatic crooked nose is a
difficult surgical task because the deformity is multivariable. A thorough
preoperative analysis of the deformity and the nasal airway must be made.
A baseline understanding of both lateral osteotomy and cartilage grafting
techniques is required because a combination of both is often necessary
for treatment.

Revision rhinoplasty and septal perforation repair present a unique set of
challenges to most surgeons. Both can be attempted simultaneously by the
surgeon using the external or open approach. Cases in which the perforation
is small and in which a dorsal hump can be reduced are the most ideal for
synchronous correction.

Revision cases often present with a lack of autogenous nasal cartilage. The
restoration of structure and function to the previously operated nose,
therefore, may require the use of grafts and synthetic implants. The priori¬
ties, applications, advantages, and limitations of the various grafting and
implant materials are discussed in this article.
