Frequency and Consequences of Ocular Trauma: A Population Perspective 559

Epidemiologic studies of ocular injuries have established the magnitude
and severity of the problem in a variety of settings in North America and
in other selected countries in Europe and Asia. There are dramatic differ¬
ences in risk of ocular trauma with age, including important peaks in young
adulthood and again among those 70 years of age or older. Men have been
shown to have a significantly higher risk than women, especially in the
younger age groups. Causes of severe ocular injuries range from motor
vehicle crashes to assault to contact lens induced corneal ulceration. The
limited information available on the social and economic consequences of
ocular trauma suggests that significant financial and social resources are
spent on medical care rehabilitation.
A Mechanistic Approach to Ocular Trauma 569

Our understanding of the mechanisms of injury resulting from ocular
trauma has increased substantially in the past century. Paramount to this
understanding has been the careful review of case series, clinicopathologic
correlation, and the development of animal models of ocular injury. This
article reviews in detail the many mechanisms of injury that are set into
motion by blunt and penetrating forces. With better understanding of the
mechanisms of injury, improvements in treatment and prevention should
follow.
Modern Diagnostic Techniques in the Evaluation of Ocular Trauma 589

This article examines penetrating and blunt injuries to the globe, highlight¬
ing the diagnosis and management of these injuries through the use of plain
film radiography, computed tomography (CT) scans, magnetic resonance
(MR) imaging, ultrasonography, and electrophysiologic testing. Often, no
diagnostic test is required because the clinical examination provides the

necessary information. The major exception is the eye in which a foreign
body is either confirmed on clinical examination or suspected but not seen
clinically. For documentation, a minimum of plain radiogram, CT scan, or
MR image is recommended in conjunction with an ultrasonogram, depend¬
ing on the suspected material.
Corneal and Anterior Segment Trauma and Reconstruction 609

The cornea and anterior segment are commonly involved in ocular trauma.
The ophthalmologist equipped with the general principles of management
of ocular trauma will be able to approach his or her patient in a logical
manner and apply the principles outlined to various clinical settings involv¬
ing the anterior segment.
Management of Trauma Induced Cataracts 633

Trauma induced cataracts are a major cause of visual loss in the patient
with eye trauma. The clinical methods and ancillary studies used for evaluat¬
ing these patients are reviewed. Anterior and posterior surgical approaches
to trauma induced cataracts are discussed. Advances in surgical equipment,
techniques, and increasing options for visual rehabilitation have improved
the visual prognosis for these patient.
Nonpenetrating Posterior Segment Trauma 647

Nonpenetrating posterior segment ocular trauma consists of blunt trauma
directly applied to the eye in the setting of an intact globe and trauma to
the other parts of the body that may indirectly affect the eye. Direct injuries
include commotio retinae, choroidal rupture, sclopetaria, traumatic macular
hole, and conditions associated with traumatic retinal detachment such as
vitreous base avulsion, retinal dialysis, retinal tears, and giant retinal tears.
Remote trauma with indirect ocular injuries include Purtscher s retinopathy,
Terson s syndrome, Valsalva retinopathy, and shaken baby syndrome. The
characteristic setting, presenting symptoms, ophthalmoscopic findings, flu
orescein angiographic features, current treatment options, clinical course,
prognosis, histopathologic findings, and pathogenesis of these conditions
are reviewed.
Trauma Related Ocular Infections 667

Trauma is a major predisposing factor in ocular infections. Maintaining a
high index of clinical suspicion based on the circumstances of injury is
critical to allow prompt recognition, immediate diagnostic investigation, and
initiation of broad spectrum antimicrobial therapy without delay. Optimal
management strategies for traumatic endophthalmitis, keratitis, and perioc
ular infections are discussed with particular emphasis on incidence, clinical
features, diagnostic techniques, causative organisms, and specific therapy.

Recent diagnostic and therapeutic advances have drastically altered the
management of orbital and periorbital trauma. This article explores trauma
primarily affecting the orbital and periorbital structures and reviews modern
approaches in the management of these serious injuries. Specifically, the
management of orbital hematoma, traumatic optic neuropathy, bony orbital
injury, lacrimal system injury, and orbital foreign bodies are addressed.

There is no agreement about the proper treatment for traumatic optic neu¬
ropathy. This article briefly reviews the different recommended treatments
and then presents the author s personal perspective based on the cooperation
of patients in the care of Massachusetts Eye and Ear Infirmary, performing
39 optic nerve decompressions for traumatic optic neuropathy and organiz¬
ing two international meetings to discuss traumatic optic neuropathy and
optic nerve decompression. The international prospective study that has
begun is presented.
The Prevention of Sports and Work Related Injury 709

Sports related injuries are predictable and, for the most part, preventable
if we make an eye safety prescription part of our routine. We can decrease
eye injuries by prescribing appropriate eyewear, which takes no more than
10 seconds, once the office is set up to promote eye safety. This article is
designed to be a practical guide to the busy practicing ophthalmologist as
an aid in writing a protective eyewear prescription and discusses the many
different types of available eye protectors, radiation and optical radiation
protection, protective filter materials, and welding filters.
The Traumatized Patient: Legal Considerations 723

In the management of ocular injuries, an ophthalmologist often comes in
contact with the legal system. Such injuries commonly occur at work or
as a result of assault, and patients may seek criminal and civil suits. An
understanding of legal considerations in these circumstances will help the
ophthalmologist better care for their traumatized patients and reduce the
risk of becoming the target of litigation. This article reviews pertinent issues
in medical malpractice and explores ways of minimizing the risk of litigation.
In addition, it highlights physician responsibilities of informed consent and
in the reporting of abuse cases.
