ucts, and even the products that are used to fight fires. Measurements of exposure

cyanide, formaldehyde, acetaldehyde, benzene, and other chemicals.

The authors cover the care of burn injuries from start to finish, beginning with a
discussion of immediate intervention and concluding with a look at psychosocial
aspects of burns. Topics in the middle include early management, evaluation of
the patient and classification of the burn s severity, burn resuscitation, the patho
physiology of smoke inhalation, dressing of burn wounds, escharotomies and fas
ciotomies, surgical management, and rehabilitation.

and present their recommendations for treatment.

Although it is universally accepted that firefighting ranks among the most stress¬
ful occupations in the U.S., the quality of information on the matter has often been
limited and subject to extreme variation. To help address the situation, the authors
provide a thorough review of the literature on psychic trauma resulting from crit¬
ical occupational events, draw specific applications to fire and rescue enterprises,
discuss models of addressing stress disorders, and argue for a reasoned, ethical,
and effective approach to the problem.

A substantial body of literature now exists on the carcinogenic hazards of fire¬
fighting. The authors discuss in detail the data on the carcinogens benzene, as¬
bestos, PAHs, formaldehyde, and diesel exhaust, and they go on to examine the
prevalent cancers in firefighters, including leukemia, nonHodgkin s lymphoma,
multiple myeloma, and cancer of the brain and bladder.

The author reviews the literature of the past 20 years on heart disease among fire¬
fighters, covering the specific aspects of firefighting that may be related to poten¬
tial cardiovascular disease. The author then outlines steps that can be taken to re¬
duce the risks of developing cardiovascular disease.

The authors summarize the available data on three populations at potential risk
from reproductive toxins: men, women, and developing fetuses. Among the areas
discussed are the mechanisms of reproductive toxicity, industrial hygiene in the

Since the NIH received a request to investigate the high degree of hearing loss in
a fire department in 1980, hearing loss among firefighters has become an area of
increased investigation. The author identifies the sources of occupational noise in
firefighting, looks at audiometric testing and recent research in firefighting noise,
and presents guidelines for implementing hearing conservation programs.
Respiratory Health Hazards and
Lung Function in Wildland Firefighters 857
Robert Harrison, Barbara L. Materna, and Nathaniel Rothman
The authors discuss the multitude of contaminants to which wildland firefighters
are exposed, including carbon monoxide, sulfur dioxide, paniculate and silica,
polyaromatic hydrocarbons, aldehydes, and benzene. They examine the respira¬
tory effects of these contaminants and then present their recommendations for an
occupational health program for wildland firefighters.
Firefighters: Fitness for Duty 871
Dick Gerkin
One of the functions of the fire department physician is to give candidates and fire¬
fighters clearance to work based on medical and physical performance criteria. In
this chapter the author discusses the practical application of the medical standards
of NFPA 1582 and physical standards as measured by agility tests and the pro¬
posed NFPA 1583, presenting guidelines for hiring, return to work, and adminis¬
tration of periodic exams.
