The symptoms and signs of lumbar radiculopathies are caused
primarily by injury/irritation of a lumbar nerve root, although
other pathophysiologic processes may also contribute. Nerve
roots are injured by a combination of mechanical and inflamma¬
tory processes. Because inflammation plays a role, epidural corti¬
costeroids should be effective in the treatment of lumbar radicu¬
lopathies; research supports their efficacy. Local anesthetics are
often mixed with corticosteroids in epidural injections. They may
well play a beneficial role, although this has not been conclusively
established.

Epidural injections in the cervical, thoracic, and lumbosacral
spine have become important tools in the management of painful

spine conditions. These injections should be carefully integrated
into a comprehensive rehabilitation treatment plan. Well con¬
trolled anesthetic and corticosteroid injections given with fluoro
scopic guidance can help distinguish active pain generators from
nonpainful structural abnormalities. This article presents various
approaches and injection techniques that are used to selectively
treat the tissues most often causing pain within and adjacent to
the epidural space.
Zygapophysial (Facet) Joint Injections: Intra articular and
Medial Branch Block Techniques 715

Fluoroscopically guided analgesic zygapophysial joint (z joint)
injections are a safe and scientific method of documenting z joint
pain. Lumbar z joint injections provide significant diagnostic data
and may facilitate a more focused treatment program. Because of
the possibility of placebo response to a single block, however,
diagnosis of definitive z joint mediated pain requires a physio¬
logic response to two separate injections with different local anes¬
thetics. The therapeutic effects of intra articular corticosteroids
remain more controversial, with only open studies supporting
their use. Further research is needed to address whether injections
followed by a more aggressive conservative program of exercise
and manual therapy during the period of relative analgesia in¬
crease long term benefit.
Lumbar Discography 743

Lumbar discography is used to evaluate internal disc anatomy
and identify clinically symptomatic discs. At the present time,
there is no other diagnostic study available that can more accu¬
rately determine if a lumbar disc is the pain generator responsible
for an individual s symptoms. Discography is useful in surgical
planning for lumbar fusions and can be helpful in evaluating
patients with persistent chronic low back pain. This procedure
can be performed safely by properly trained physicians using
strict aseptic technique.

Percutaneous lumbar disc procedures limit postoperative scar¬
ring, minimize morbidity, and reduce cost when compared with
open lumbar surgical techniques. The indications, techniques,
potential complications, and postoperative care of percutaneous
discectomy, chemonucleolysis, and annular denervation are dis¬
cussed.

The sacroiliac joint is a definite source of low back and lower
limb pain. This article discusses its anatomy, biomechanics,
pathophysiology, and noninvasive and invasive clinical evalua¬
tion. The spectrum of treatment approaches is discussed. Injection
techniques regarding the sacroiliac joint are reviewed in detail,
and a modified approach to intra articular injections is presented.

Despite widespread use, the mechanism of action of intra articu¬
lar glucocorticoid therapy remains unclear. Systemic absorption
occurs and may be associated with significant toxicity. Concerns
about direct articular cartilage damage from injected corticoster¬
oids are probably unfounded.

Joint and soft tissue injections have a role in the treatment of
localized pain and swelling and are potent diagnostic and thera¬
peutic tools. Conditions of the upper extremity such as carpal
tunnel syndrome, de Quervain s tenosynovitis, lateral epicondy
litis, and shoulder impingement syndrome are amenable to treat¬
ment with local injections. With appropriate technique, these
injections have been found to be safe and effective. This article
reviews the available literature on upper extremity injections
and presents the authors methods of evaluation, treatment, and
injection.

Arthrocentesis and corticosteroid injections serve an important
diagnostic and therapeutic role in the management of joint
and soft tissue inflammatory conditions. Joint fluid examination
allows for differentiation between inflammatory, noninflamma¬
tory, and septic processes, and for documenting the presence of
crystal induced arthritides. When initial conservative manage¬
ment is ineffective or when there are severe time constraints,
the combination of injectable glucocorticoid steroid and local
anesthetic solution is very helpful in providing relief and promot¬
ing healing. This article outlines general guidelines for safe and
effective injection techniques.

Local Injections in Pain Management: Trigger Point Needling

Five main types of injections for soft tissue pain management and
treatment of soft tissue damage are discussed. They are (1) trigger
point injections, (2) needling and infiltration of damaged and
tender areas, (3) somatic block, (4) needling and infiltration of
ligament sprains and bursitis, and (5) preinjection nerve blocks.
Two important aspects of injections must be emphasized. First,
needling combined with infiltration is the most effective modality
for immediate alleviation of pain and for complete removal of
pathology causing pain. Second, the injection represents only
the first step in management; special follow up treatment after
injections assures the best and fastest healing.
Management of Sympathetic Pain Disorders: Cervicothoracic
and Lumbar Sympathetic Blockade 871

The sympathetic nervous system is thought to play a role in many
painful disorders involving the face and extremities. Successful
diagnosis and treatment of these disorders is best accomplished
by an aggressive, multimodal approach involving medical, surgi¬
cal, and percutaneous therapies. Local techniques for sympa¬
thetic blockade are the cornerstone of these treatment programs
and are discussed in detail. The rehabilitation physician is encour¬
aged to become familiar with the indications, methods, and po¬
tential complications of these therapies.
The Use of Neurolytic Blocks for the Management of
Spasticity 885

The management of spasticity often requires a combination of
methods to maintain normal range of motion and improve func¬
tional capacity. Most practitioners are familiar with the use of
systemic spasmolytic agents, stretching, and splinting but may be
less likely to consider anesthetic or neurolytic neural blockade for
management of spasticity in a localized fashion. In this article,
both historical perspectives and new techniques of neural block¬
ade are discussed. Information presented includes indications for
use, choices for medication and site of injection, duration of
therapeutic effect, and adverse effects. The goal of the article is
to provide a rational, integrated analysis for the inclusion of
peripheral neural blockade and intramuscular neurolysis in the
treatment of spasticity.

Botulinum toxin injection is helpful for a number of dystonias
that the physiatrist may encounter. This article covers patient
assessment, proper handling of the drug, deciding which muscles
to inject, injection technique, and follow up. Botulinum toxin
injection, when used appropriately in the right patients, is a
highly effective medication with predictable responses and mini¬
mal adverse effects.
Acupuncture: History, Clinical Uses, and Proposed

Acupuncture is a traditional form of Chinese medicine that has
been used for more than 3000 years. Its philosophy is eloquent
and empowered by life s processes that move in a rhythmic and
harmonious way. Additionally, it is an orderly, comprehensible,
and clinically efficacious treatment method that needs further
exploration by Western physicians. Profoundly, as a modality, it
fits congruently into the basis of other physical medicine modal¬
ities. It is complementary to the physiatrist and other health care
practitioners caring for patients with pain complaints.
Prolotherapy: Present and Future Applications in Soft Tissue
Pain and Disability 917

In acute or chronic damage to soft tissue, the enthesis is the site
of damage. Degeneration rather than inflammation is the primary
change over time, with resultant insufficient connective tissue
strength. Given a high degree of sensory innervation and chronic
mechanoreceptor stimulation, the ability of entheses to function
as chronic nociceptors is substantial. Connective tissue laxity or
diminished tensile strength can be addressed with prolotherapy.
Applications to sprain, strain, overload injury, resistant bursitis
and tendinitis, articular cartilage irritation, trigger point disor¬
ders, fibromyalgia, and joint laxity are discussed.
