A review of the epidemiology of cranial gunshot wounds in our cities is
presented in this article. The recent explosion of these devestating injuries
is placed in context, and the various factors that contribute to the acceleration
of this public health problem are discussed. Special attention is given to
the effect this problem has on children. A discussion of the authors own
experience in a large urban population is provided.

and their victims in the rural setting. The population is compared to and
contrasted with urban analogues. The potential prognostic value of clinical
and laboratory findings is considered. The results of such analyses may

Street gang violence has become a major public health problem in the United
States, especially in the inner cities. Firearms are the most commonly used
weapons in violent street gang activity. Walkup and drive by shootings
that result in injuries are two of the most common crimes committed by
members of violent street gangs. To prevent injury and death from gang
violence, the root causes of violent street gang activity must be alleviated,
and the relationship between firearms and gang violence must be addressed.

Missile wounding can only be understood in terms of the physical interac¬
tions between the missile and the tissue through which it passes. This article
discusses the physical correlates of missile wounding, energy of wounding,
missile causing brain wounds, prior experiments on brain wounds in anes¬
thetized animals, development of the Louisiana State University brain
wound model, animal preparation, results and discussion of findings, and
drug studies.

As more about pathophysiology of traumatic ischemic brain injury becomes
known, more effective means for pharmacologic intervention and neuronal
salvage will emerge, and optimisim is high that improvements in functional
survival will continue. This article discusses the prevention and treatment
of the secondary injury that follows the traumatic insult because this offers
the best chance for improving current methods for enhancing neurologic
recovery subsequent to a penetrating brain injury. The current and most
potentially rewarding avenues for intervention in the head injured patient
are discussed.

This article addresses the subject of cerebral blood flow in patients suffering
penetrating craniocerebral injuries. The discussion focuses on cerebral hemo¬
dynamic disturbances as assessed by either xenon 133 cerebral blood flow
measurements or transcranial Doppler (TCD) ultrasonography. The implica¬
tions of low cerebral blood flow and vasospasm induced ischemia on out¬
come are illustrated.

This article presents a brief overview of the imaging findings in craniocere¬
bral missile injuries (CMI). CT scanning has established itself as the primary
imaging modality for the complex injuries seen in CMI and its acute and

limited but sometimes important roles in the managment of these injuries.

the utility of the Glasgow Coma Scale (GCS) score at presentation, presence
or absence of coagulopathy, and radiographic evidence of the volume and
type of tissue damage.

A large proportion of patients who arrive at hospitals after gunshot wounds
to the head have GCS scores of 3, 4, or 5, indicating a very severe brain
injury. Death rates for these patients are high, and those who survive often
have devastating neurological deficits. Several prognostic indicators have
been identified that may identify patients who may be salvageable; however,
most of these patients will do poorly. Therefore, there must be societal
decisions about how to balance the rights of patients who expect care and
the responsibility of society to provide this care against the obligations of
patients not to be unreasonable about their demands, and the need for
society to allocate limited resources in the most fair way, and thus, who
should be treated.
Penetrating Injuries in the Vietnam War: Traumatic
Unconsciousness, Epilepsy, and Psychosocial Outcome 715

The WF Caveness Vietnam Head Injury Study includes over a thousand
men who survived penetrating head injuries during the Vietnam War and
on whom detailed medical and follow up data are available. This population
offers unique opportunities for the study of recovery from brain injury and
of brain structure function relationships. The authors briefly review long
term outcome in this cohort with respect to traumatic unconsciousness, post
traumatic epilepsy, and elements of psychologic and psychosocial function,
including returning to work.
Care and Variations in the Care of Patients with Gunshot Wounds to
the Brain 727

A national survey revealed significant variations in a number of aspects of
evaluation and surgical and nonsurgical treatment of patients with gunshot
wounds to the brain. There could be many reasons for this. It would seem
that there has probably not been sufficient research done to fully understand
all aspects of such injuries. It also may be that different ways of caring for
these patients provide equally satisfactory outcomes. Another problem that
arises is that even with good information, there is difficulty in changing
practice patterns that need to be understood better and made more effective.
Based on the author s review and ethical considerations, certain basic and
reasonable suggestions are made in this article.
The Prehospital and Emergency Department Management of

The care of the patient with a penetrating head injury begins in the prehospi¬
tal setting by paramedical and emergency medical technicians. The immedi¬
ate management of these patients is continued in the emergency department.
This article discusses these resuscitative and stabilizing measures and their
limitations. It is important for the neurosurgeon to have a thorough under¬
standing of the continuum of care that is provided until definitive therapy
can be initiated.

Military neurosurgery has long been a leader in developing treatment for
patients with penetrating head injuries. The evolution in treatment ap¬
proaches for these patients is reviewed in this article. Where applicable to
civilian trauma neurosurgery, recommendations are made regarding surgi¬
cal treatment and patient triage. The shift from routine radical debridement
to less aggressive surgical approaches in selected cases is emphasized. Atten¬
tion is given to principles in management of cerebrospinal fluid leaks.

of intracranial aneurysms. With the introduction of powerful neuroimaging

the brain. This article discusses recent literature, nonprojectile penetrating
traumatic aneurysms, iatrogenically induced traumatic aneurysms, materi¬

This article outlines the overall ethical role of the surgeon in rendering
health decisions in an efficacious manner. Following a brief history of the
role of the surgeon and support for a negotiation approach in health care,
a workable integrative negotiation (WIN) approach is presented. An applica¬
tion of the negotiation model in a health communication encounter is pre¬
sented for circumstances in patients and families with an acute traumatic
head injury. Finally, a prescriptive approach a surgeon should choose to
advance optimal civic health is offered in the hope that all can work together
for a healthier society.
