Rigid fixation for the repair and repositioning of bones of the craniofacial
skeleton has advanced dramatically during the past two decades. Presently
it is considered the standard of care for the management of craniofacial
skeletal trauma, for the orthognathic repositioning of the facial bones, and
reconstruction of congenital anomalies of the craniofacial skeleton. This
article reviews facial plating techniques and discusses future directions for
additional advances.
Calvarial Bone Grafting in Craniofacial Reconstruction 241

This article provides a comprehensive overview of calvarial bone grafting
and facial reconstruction. It reviews the history and basic sciences related
to calvarial bone grafting and bone healing. The authors techniques for
harvesting the different types of grafts are described, and a summary of
the literature highlights the wide variety of applications in craniofacial
reconstruction.

Orbital fractures are managed in a variety of ways, depending on the
extent of the defect and the presence of associated fractures. These two
factors are the primary determinates of which techniques and materials
will be used for the reconstruction. The materials available for reconstruc¬
tion of these defects include alloplastic implants, autologous bone grafts,
and metallic micromesh plating systems. Distinct advantages and disad¬
vantages are associated with each of these materials. This article discusses

their use in orbital reconstruction and demonstrates the techniques with

Alternative to the Lateral Rhinotomy 271

Presently, the most popular approach to lesions of the nasal cavities,
paranasal sinuses, and nasopharynx is the lateral rhinotomy. Although it
offers direct access, it leaves residual deformity in the form of a permanent
facial scar and offers only unilateral access. Midfacial degloving, with its
various modifications, is a reasonable alternative that allows adequate
surgical exposure without the residual deformity.
Approach to Rhinophyma 281

Rhinophyma is a form of nasal rosacea that occurs mainly in older men.
This condition compromises the patient s nasal function and cosmesis.
Medical and surgical therapies have been proposed. Carbon dioxide laser
resection was the treatment most commonly chosen in the early and mid
1980s. A new technique, which uses copious amounts of local anesthetic
agents, sharp dissection, and hemostasis with the argon beam coagulator,
is introduced in this article. The modality is simple and fast and gives
excellent results.
Growth Factors in Nerve Regeneration 287

The management of peripheral nerve injuries remains a clinical challenge,
and despite advances in surgical technique, return of normal function after
repair is uncommon. Knowledge of the cellular and molecular events
surrounding nerve regeneration has increased significantly. It is now pos¬
sible to enhance this process in animals using growth factors, and a
possible future therapeutic role for these substances is being contemplated.
This article describes the pathophysiology of motor nerve injury and
examines the results of surgical repair. Contemporary research involving
the use of growth factors to facilitate nerve regeneration is reviewed, and
preliminary data evaluating the efficacy of insulin like growth factor and
platelet derived growth factor for nerve regeneration in a rabbit facial
nerve model are presented.

Microtia is a very difficult surgical problem that is not encountered fre¬
quently. Surgeons often have few resources to direct them in how to
reconstruct an auricle. This article presents important preoperative infor¬
mation and examples of auricular reconstruction.
Laser Treatment of Cutaneous Vascular Lesions 307

Laser technology continues to improve, and so does the surgical armamen¬
tarium for treatment of cutaneous vascular lesions. In order to use this
technology fully, the laser surgeon must understand the specificity of
various wavelengths, the differing laser tissue interactions, the classifica¬
tion of cutaneous vascular lesions, and the therapeutic options for each le¬
sion.

Laser Assisted Surgery of the Aging Face 319

The high energy pulsed carbon dioxide laser with a high conductivity,
flexible 0.8 mm waveguide (fiber) has produced advances in laser assisted
surgery. This new technology enables surgeons to work rapidly through
small incisions and endoscopes, with diminished morbidity rates. The
combination of rapid char free cutting, excellent coagulation, and accurate
fiber delivery extends the usefulness of this carbon dioxide laser.
