Endoscopy is a new modality of operating that is rapidly gaining accep¬
tance, particularly in aesthetic plastic surgery. This overview reviews
the applications of endoscopy for facial aesthetic surgery, especially the
forehead, as well as its applications to breast augmentation and abdomi
noplasty. Possibilities for future uses of endoscopy also are delineated.
Instrumentation and Setup for Endoscopic Plastic Surgery 591

The adoption of endoscopic surgical techniques adds an important new
tool for the plastic surgeon but adds new complexities of operative setup
and instrumentation. Familiarity in choice, design, function, and use of
endoscopic instrumentation will serve to expedite operative procedures
and minimize surgeon frustration. Similarly, the development of an orga¬
nized approach to operating room setup, including consideration of access
incision sites, will expedite endoscopic procedures. This article includes
an overview of currently available instrumentation for endoscopic plastic
surgical procedures. A generalized approach to operating room setup is
described, allowing the surgeon to tailor equipment and patient position
to individual procedures.
Endoscopic Forehead Lift: Aesthetics and Analysis 605

From an extensive review of the literature combined with an on going
analysis of aesthetic patients, two aesthetic characteristics of forehead
rejuvenation are evident. First, the aesthetics can be derived from the
following five sources: (1) classical art, (2) make up artists, (3) current
fashion models, (4) anthropometric measurements, and (5) youthful versus
aging changes. Second, the analysis often has the following progression:
(1) eyebrow level, (2) eyebrow shape, (3) periorbital aesthetics, and (4)
forehead aesthetics.

This article presents the development and current status of endoscopic
browlifting from its inception at the University of Alabama at Birmingham
(UAB) to the present techniques practiced at UAB and the Ochsner Clinic
in New Orleans. This article includes a review of the basic and clinical
research of materials and methods, a description of the procedure, and
clinical case studies.
The Biplanar Endoscopically Assisted Forehead Lift 633

The biplanar endoscopically assisted forehead lift offers advantages over
the pure endoscopic forehead lift for patients with marked transverse
forehead wrinkles, high vertical forehead height, or eyebrow asymmetry.
The two planes of dissection, via a prehairline incision, allow for redraping
of the forehead skin in the frontalis muscle, release of the periosteum at
the supraorbital rims, and differential plication of the frontalis muscle for
correction of brow asymmetry, with a tension free wound closure. Its
disadvantage is the prehairline incision, which, in most patients, however,
heals well.
Endoscopic Subperiosteal Browlift and Facelift 639

Endoscopic techniques for facial rejuvenation offer a variety of alternatives
that can be tailored to the patient s facial aesthetic and aging characteris¬
tics. The author approaches facial dissection in the central third of the face
primarily. In cases of severe rhytidosis or excess skin, a peripheral skin
excision with minimal undermining can be added. Step by step descrip¬
tions of the author s preferred techniques are illustrated. A classification
of subperiosteal lifts for better understanding of the complex methods of
rejuvenation is introduced.

lift. The basic technique, patient selection criteria, and advantages and
disadvantages are presented.

Prominent glabellar frown lines result from hyperactive corrugator mus¬
cles. A large number of patients are candidates for a minimally invasive
procedure to correct this problem. Because the anatomy and surgical
approach are familiar, this is an ideal entry level endoscopic operation.

Scarring potentially can detract from otherwise excellent aesthetic gains
achieved through a variety of surgical procedures of the breasts. Endo¬
scopic techniques can minimize scarring associated with surgery of the

breasts while decreasing morbidity rates. Augmentation, mastectomy, bi¬
opsy, capsulotomy, capsulectomy, implant inspection, reduction, masto
pexy, and various reconstructive procedures are all amenable to minimally
invasive techniques in selected patient groups. Current instrumentation
and techniques as well as future applications for endoscopic surgical
procedures of the breast are described.
Mammoscopy and Endoscopic Implant and Breast Tissue Evaluation 697

Mammoscopy is a very promising technique that allows the endoscopic
evaluation of breast implant integrity and documentation of the condition
of the breast implant in situ. Breast biopsy or nodular mass identification
also can be accomplished endoscopically. This article reviews the tech¬
niques and materials needed as well as the limitations of these endoscopic
procedures.
Endoscopic Abdominoplasty with Repair of Diastasis Recti and
Abdominal Wall Hernia 707

Endoscopic abdominoplasty is feasible, safe, and effective in the proper
surgical candidate. Excellent results can be expected when proper patient
selection criteria are followed. With future refinements in technique and
equipment, this procedure may be extended safely to those patients with
more severe deformities.

Endoscopy allows the surgeon to perform delicate procedures through
minimal incisions that can be made at strategically placed and remote
sites to avoid visible scars. This is an important goal in the field of
aesthetic surgery, in which scars are undesirable and may sometimes
compromise the final aesthetic result. This article presents the author s
experience with the use of endoscopic methods in abdominoplasty, masto¬
pexy, and breast reduction.
Endoscopic Harvest of the Omental and Jejunal Free Flaps 747

Experimental and clinical cases document that the omentum and jejunum
can be harvested successfully as free flaps using laparoscopic assistance.
The abdominal incision required in most of the cases was minimal, with
an inconspicuous donor site scar. The procedure has been well tolerated
by the patients, and the postoperative course has been unremarkable.
Although some modifications already have been made to the original
laparoscopic harvesting techniques, more cases are necessary before one
can reach definitive conclusions about whether the endoscopic approach
is significantly better than the open technique.
Endoscopic Technique for Free Flap Harvesting 755

Endoscopic surgery has been shown to reduce the morbidity rate for
a growing number of surgical procedures, including tissue harvest for
microvascular reconstruction. Minimally invasive surgical techniques are
being applied experimentally and clinically in the harvest of a variety of

tissues, including muscles, nerves, fascia, veins, and intra abdominal tis¬
sues. Microvascular surgery leads to the paradoxical situation in which
the major technical challenge and source of potential morbidity in a recon¬
structive procedure may not involve the region of the defect, but rather
the distant site from which tissue is being harvested for transfer. With
increased experience and instrumentation development, it is possible that
minimally invasive tissue harvest will become the future standard.

Endoscopic carpal tunnel release is a new technique that shows promise
in reducing postoperative pain and in shortening return to work times.
Endoscopic carpal tunnel release, however, is a technically exacting proce¬
dure that requires careful attention to detail to avoid known complications.
This article reviews the author s experience with the Agee technique.
Development of an Instrument for Endoscopic Nasal Surgery 781

A newly developed instrument specifically designed for endoscopic nasal
procedures has been used clinically in the treatment of the septum and
nasal dorsum with success. Dissection and resection of bone and cartilage
is performed with precision, and the magnified image is of extreme value
as an educational tool.

Functional endoscopic sinus surgery has affected significantly the treat¬
ment of infectious and benign tumors of the paranasal sinuses. With an
advanced understanding of the anatomy of the frontal recess and frontal
sinus, this area can be treated successfully with endoscopic surgery,
avoiding traditional open, obliterative techniques. This article discusses
the anatomy of the frontal recess and frontal sinus and describes four
illustrative cases.
