For over 20 years, nursing literature has discussed the gap be¬
tween the conduct of research and the use of its findings to im¬
prove practice. Factors relating to the nurse, the setting, the re¬
search, and the way research is presented have been identified as
barriers to research utilization. A combined approach that in¬
cludes specialty organizations that take the lead in knowledge
synthesis and dissemination, the creation of national on line
databases, and enhanced institutional supports and educational
efforts is recommended.

The proliferation of research in nursing is greater today than at
any other time in the history of nursing. In order for this re¬
search to be meaningful for practice, research utilization efforts
are imperative. This article reviews three prominent research uti¬
lization models spanning three decades of development. Future

challenges and suggestions for improving the process are identi¬
fied. Through research utilization efforts, nurses are given the

Evaluation is a key component of the research utilization process.
There is little consensus, however, regarding the components of
evaluation. Examples of evaluations of research based interven¬
tions are provided in this article, and the difference between
replication and evaluation is discussed.

Patients expect to receive health care that is based on the most
current knowledge available. Each practice discipline must deter¬
mine how to bring new knowledge to the attention of practition¬
ers in the field, and nursing is no exception. This article presents
strategies that can be used to support the research dissemination
goals of nursing. Examples of successful dissemination models
that are helpful in applying ideas to practice are provided.

Professional nursing organizations and nursing leaders value the
importance of research based nursing practice. The urgency for
nurses to use research in practice applies to all, regardless of in¬
stitution size. In this article, unique characteristics of small, rural,
community hospital settings are described in relation to research
utilization. The successful initiation and maintenance of a valued
research utilization program in a small, rural hospital is pre¬
sented, and suggestions on overcoming perceived barriers to the
facilitation of research utilization activities are reviewed.

A research utilization forum for clinical nurse specialists was ini¬
tiated in the early 1990s in two service settings. The primary pur¬
pose of this voluntary, small group strategy was to help master s
prepared clinicians gain knowledge and skills regarding the
systematic use of research findings. This article describes the fo¬
rum strategy and its conceptual base. It also reviews outcomes re¬
lated to program objectives, utilization competencies, and case
reports of subsequent clinical nurse specialist research utilization
behavior.
Latex Allergy Precautions: A Research Based Protocol 475

Recently, a number of case reports in the literature have height¬
ened the awareness of health care professionals to the prevalence
and seriousness of allergies to natural rubber latex. Reports of se¬
rious anaphylactic reactions in patients with no known history of
the allergy are particularly alarming. The Iowa Model for im¬
proving quality through research utilization provided a frame¬
work to develop a protocol for screening patients and caring for
those identified as high risk for a systemic reaction to latex. An
overview of this research based practice protocol and the process
used are presented.
The Use of Gauze Versus Transparent Dressings for Peripheral
Intravenous Catheter Sites 495

The purpose of this research utilization project was to determine
if a sufficient research base was present to support the use of
gauze rather than transparent dressings for peripheral intra¬
venous catheter sites. The research articles reviewed in this article
do not support a relationship between dressing type and compli¬
cations of peripheral intravenous catheters.
Evaluation and Implementation of a Research Based Falls

This article describes the trial and implementation of a research
based practice innovation. The Morse Fall Scale is a six item tool
used to identify patients at risk for falls. The Morse Fall Scale was

first piloted at a Veterans Affairs Medical Center with good re¬
sults. Subsequent implementation of this tool throughout the
hospital produced mixed results.
Implementation of the Agency for Health Care Policy and
Research Postoperative Pain Management Guideline 515

The Agency for Health Care Policy and Research reviewed and
synthesized the current pain research and developed a clinical
practice guideline to assist the practitioner. A community hospi¬
tal decided to adopt the postoperative pain management guide¬
line using a research utilization approach. Education of health
care professionals and development of a pain flow chart, pain
scales, and updated technology were completed. Preliminary
findings demonstrate a decreased pain intensity score postsurgi
cally and an increase in patient satisfaction with staff response to
pain relief.
Animal Assisted Therapy in the Intensive Care Unit: A Staff

This article discusses the potential of staff nurses to solve prob¬
lems in clinical practice through research and to promote re¬
search based nursing practice. An example is provided that
demonstrates how a staff nurse bridged the research to practice
gap by implementing animal assisted therapy in the critical care
setting. Patient satisfaction surveys indicate that critically ill pa¬
tients have benefitted from animal visits. Patients reported feel¬
ing more happy, calm, and loved and less lonely after the re¬
search based innovation was implemented.
Using a Research Based Assessment Scale in Clinical Practice 539

and Sue Bruch
Pressure ulcers are a major problem in the United States and
around the world. The key to preventing pressure ulcers is early
identification of at risk patients. This article describes the re¬
search base for risk assessment using the Braden Scale and proto¬
cols keyed to specific risk factor reduction as a means of decreas¬
ing the incidence of hospital acquired pressure ulcers. Two
examples of hospitals that implemented the Braden Scale and as¬
sociated preventive protocols are presented, along with outcome
data demonstrating a decrease in the incidence of ulcers and the
cost of care.
Adoption of a Research Based Practice for Treatment of
Pressure Ulcers 553

The process of research utilization was the vehicle for implemen¬
tation of a research based skin care protocol for the treatment of

pressure ulcers in an 830 bed, long term care facility. Elements of
the process that were put into practice in this setting are de¬
scribed. The evaluation of practice related to pressure ulcer treat¬
ment revealed that practitioners did adopt research based con¬
cepts. The factors that led to the positive outcome of knowledge
utilization are outlined in this article.
The Future of Research Utilization 565

The need for utilization of scientifically based knowledge in clini¬
cal practice was recognized long ago. This article discusses the
future of research utilization in nursing, emphasizing the role of
nurse administrators, health care organizations, educational pro¬
grams, and professional organizations. A futuristic view of re¬
search utilization is presented.
