The boutonniere (French for button hole) deformity is a result of an
injury to the finger extensor mechanism that causes a characteristic flexion
attitude of the proximal interphalangeal joint and distal interphalangeal
joint hyperextension. This article reviews the pertinent anatomy, the mech¬
anisms of injury, the diagnosis, and a systematic approach to the preven¬
tion and treatment of the boutonniere deformity.

Results of surgical treatment to extensor tendon injuries vary with the
degree of the associated injuries. Fractures, soft tissue loss, and neurovas
cular injuries adversely affect outcome. Early, controlled, active mobiliza¬
tion is indicated in severe injuries.

This article reviews the etiology of extensor pollicis longus rupture and
the pathogenesis of its occurrence. Different means are discussed for
restoration of function after rupture. Extensor indicis proprius tendon
transfer is recommended as the preferred method of treatment, and the
results of this procedure are presented.
Extensor Tendon Grafting on the Dorsum of the Hand in Massive
Tendon Loss 423

After severe injury to the dorsum of the hand, it may not be possible to
directly repair the extensor tendons. Patients in this article were treated
by initial skin replacement with pedicle flaps for coverage prior to extensor
tendon grafting. Multiple operative procedures were necessary, but resto¬
ration was uniformly successful.

The authors study the literature and the surgical treatment in chronic
nonrhematoid cases. Their technique employs one half of the extensor
digitorum communis tendon as a tether on the central extensor tendon,

A patient with a chronic boutonniere deformity often experiences more
difficulty because of the hyperextension deformity at the distal joint, rather
than the lack of extension at the proximal interphalangeal joint. It is in
these cases that restoration of flexion at the terminal joint by tenotomy of
the terminal extensor tendon is of most value.

Extensor tendon adhesions following laceration, fracture, or crush injuries
will prevent normal joint function. Tenolysis often will not address the

The use of immediate active short arc motion for the repaired extensor
tendon (in zones III to VII and thumb zones T IV and TV) is discussed.
Basic science, biomechanical, and clinical studies provide support for this
approach to postoperative rehabilitation for the repaired extensor tendon.
